Physical therapy for reducing arousal and mechanical restraint among in-patients with mania.

BACKGROUND: From 2003 to 2013 the number of patients exposed to mechanical restraint in Danish psychiatric hospitals was increased. Since, a great effort has been done to reduce mechanical restraint. OBJECTIVES: The aim of this study was to assess whether physical therapy could lower the level of arousal and reduce mechanical restraint among patients with mania. METHODS: 170 patients admitted with mania at Aarhus University Hospital were offered physical therapy during the study period from 2017 to 2018. Patients reported their experienced level of arousal before and after physical therapy,

[Vision on and use of physical restraints and 'smart technology' in nursing homes in Flanders].

AIM: The STAFF-project investigates in what way 'smart technology' can offer an alternative for physical restraints in nursing homes. A survey is realized aimed at gaining more insight into the vision on and the use of physical restraints and 'smart technology'. METHOD: Two partly overlapping structured questionnaires were developed and sent to nursing home staff in Flanders (Belgium). One hundred fifty six administrators (managers or assistant-managers) and 238 caregiving staff (nurses, nursing aids, paramedical staff and other) completed the online questionnaire. RESULTS: In general there

Use of physical restraints in nursing homes: a multicentre cross-sectional study.

BACKGROUND: Although many countries have implemented strict legal rules, the prevalence of physical restraints in nursing homes seems to remain high. In Switzerland, data related to the frequency of physical restraints are scarce and little is known about associations with resident and nursing home characteristics. The aim of this study was to investigate the prevalence and types of physical restraints in nursing homes in two Swiss cantons and to explore whether resident-related and organisational factors are associated with the use of physical restraints. METHODS: We conducted a multicentre

Staffing levels and the use of physical restraints in nursing homes: a multicenter study.

There is an unclear relation between staffing levels and the use of physical restraints in nursing homes (NHs). A survey design was used in 570 older adults (median age = 86; 77.2% women), living on 23 wards within seven NHs. Restraint use was high (50% of residents, of which 80% were restrained on a daily basis). Multivariate analysis was conducted at the level of the individual wards. Neither staff intensity nor staff mix was a determinant of restraint use. Bathing dependency, transfer difficulties, risk for falls, frequent restlessness/agitation, and depression were independent predictors o

Disparities in Physical Restraints and Antipsychotic use in Hospitalized Patients with Limited English Proficiency.

BACKGROUND: Physical restraints and antipsychotics should only be used as a last resort in hospitalized individuals at risk of harm to themselves or others. Since verbal reorientation and de-escalation are first-line management in these scenarios, patients with limited English proficiency (LEP) may be at higher risk of physical restraint and antipsychotic use. We aimed to examine the relationship between English proficiency, physical restraints, and antipsychotic use in hospitalized adults. METHODS: Retrospective cohort study of adults discharged from an academic medical center between 1/2019

Restraint asphyxia. An analysis of the circumstances and mechanism of death in agitated, physically restrained individuals.

Recent years saw frequent media reports of young people who die while they are being arrested by the police. Death in these circumstances affects people who are agitated and restrained with the use of force, with their autopsies indicating no unequivocally traumatic cause of death. The goal of this study was to identify the mechanism and circumstances of sudden deaths in agitated individuals who are being restrained. Ten cases evaluated at our center since 2010 were included in this study: nine involved forensic postmortem examination and one involved casefile analysis. In each case there was

Impact of the Hospital-Acquired Conditions Initiative on Falls and Physical Restraints: A Longitudinal Study.

BACKGROUND: The Centers for Medicare & Medicaid Services (CMS) implemented the Hospital-Acquired Conditions (HACs) Initiative in October 2008; the CMS no longer reimbursed hospitals for fall injury. The effects of this payment change on fall and fall injury rates are not well described, nor its effect on physical restraint use. OBJECTIVE: The aim of this study was to examine the effects of the 2008 HACs Initiative on the rates of falls, injurious falls, and physical restraint use. DESIGN/SETTING: This was a nine-year retrospective cohort study (July 2006-December 2015) involving 2,862 adult

Factors associated with the use of physical restraints for agitated patients in psychiatric emergency rooms.

OBJECTIVE: To examine factors associated with physical restraint in psychiatric emergency rooms. METHOD: We extracted variables likely to predict use of physical restraints from a large randomised trial undertaken in three psychiatric emergency rooms in Rio de Janeiro. We fitted a Bayesian binary multivariate model using only variables clearly preceding the restraints. RESULTS: Of 301 agitated, aggressive people admitted to emergency rooms, 73 (24%) were restrained during the first 2 h of admission. In Rio, younger people (OR=1.03 for each year younger), exhibiting intense (OR=2.53) or extre

Physical restraint in the intensive care units: a narrative review of current practices and outcomes.

BACKGROUND: Physical restraint remains common in the intensive care unit (ICU), but it is increasingly questioned following recent high‑quality evidence, particularly the R2D2‑ICU trial, which challenges the assumption that reducing restraint inherently improves short‑term neurological outcomes. MAIN BODY: Epidemiological data reveal significant variability in the prevalence of restraint use, which is influenced by a complex interplay of patient characteristics, caregiver factors, and systemic elements. The evidence on its effectiveness for mainly preventing the pulling out of tubes is questi
