Meta-analysis of primary care delivered buprenorphine treatment retention outcomes.

Background: Currently, the capacity to provide buprenorphine treatment (BT) is not sufficient to treat the growing number of people in the United States with opioid use disorder (OUD). We sought to examine participant retention in care rates of primary care delivered BT programs and to describe factors associated with retention/attrition for participants receiving BT in this setting. Objectives: A PRISMA-guided search of various databases was performed to identify the articles focusing on efficacy of BT treatment and OUD.Method: A systematic literature search identified 15 studies examining ret

Guidelines for preventing urinary retention and bladder damage during hospital care.

AIMS AND OBJECTIVES: To develop evidence-based guidelines for adult patients in order to prevent urinary retention and to minimise bladder damage and urinary tract infection. BACKGROUND: Urinary retention causing bladder damage is a well known complication in patients during hospital care. The most common treatment for urinary retention is an indwelling urinary catheter, which causes 80% of hospital-acquired urinary tract infections. Appropriate use of bladder ultrasonography can reduce the rate of bladder damage as well as the need to use an indwelling urinary catheter. It can also lead to a

Quantifying Staff Preferences for Workforce Retention Strategies in Remote Australian Primary Health Care Services.

AIM: To outline key criteria when choosing how to measure remote area primary health care workers' retention preferences and compare six different methodological approaches against these criteria. CONTEXT: Australians living in remote communities experience poorer health outcomes. Many health services serving remote populations have persistent workforce shortages, high staff turnover and short retention which adversely affect access to timely, appropriate health care, care continuity and cultural appropriateness of care. Improved understanding of staff preferences for different retention init

Closing the hepatitis C treatment gap: United States strategies to improve retention in care.

The hepatitis C virus (HCV) treatment landscape is shifting given the advent of direct-acting antivirals and a global call to action by the World Health Organization. Eliminating HCV is now an issue of healthcare delivery. Treatment is limited by the complexity of the HCV care continuum, expensive therapy and competing health burdens experienced by an underserved HCV population. The objective of this literature review was to assess strategies to improve retention in HCV care, with particular focus on those implemented in the United States. We identified barriers in HCV care retention and propo

Pattern of linkage and retention in HIV care continuum among patients attending referral HIV care clinic in private sector in India.

Continued engagement throughout the HIV care continuum, from HIV diagnosis through retention on antiretroviral therapy (ART), is crucial for enhancing impact of HIV care programs. We assessed linkage and retention in HIV care among people living with HIV (PLHIV) enrolled at a private HIV care clinic in Pune, India. Of 1220 patients, 28% delayed linkage after HIV diagnosis with a median delay of 24 months (IQR = 8-43). Younger people, women, low socioeconomic status, and those diagnosed at facilities other than the study clinic were more likely to delay linkage. Those with advanced HIV disease

Retention in HIV care before and after implementation of a case management program in Malawi.

BACKGROUND: Retaining persons living with HIV (PLHIV) on antiretroviral therapy (ART) and preventing loss-to-follow-up (LTFU) is an ongoing challenge. In Malawi, the Elizabeth Glaser Pediatric AIDS Foundation (EGPAF) implemented a facility-based case management initiative targeting PLHIV at risk of discontinuing care. Using a risk assessment tool, the program provided personalized support through psychosocial counselors, adherence officers, and expert clients. Our evaluation assessed the impact of this initiative on retaining newly initiated ART clients. METHODS: We conducted a pre-/post quasi

Sexual Risk Compensation and Retention in PrEP Care in Korea: An HIV PrEP Demonstration Study.

BACKGROUND: Pre-exposure prophylaxis (PrEP) with tenofovir disoproxil fumarate and emtricitabine (TDF-FTC) is effective in preventing human immunodeficiency virus (HIV), however, its feasibility has not been evaluated in the Republic of Korea (Korea). Therefore, this study aimed to assess the feasibility of PrEP in men who have sex with men (MSM) in Korea. This is the first demonstration study in Korea, in which PrEP medication, laboratory tests, and clinic visit fees were provided without charge to participants. METHODS: HIV-negative MSM were prescribed daily TDF-FTC and followed up at an ou

Retention in HIV care between testing and treatment in sub-Saharan Africa: a systematic review.

BACKGROUND: Improving the outcomes of HIV/AIDS treatment programs in resource-limited settings requires successful linkage of patients testing positive for HIV to pre-antiretroviral therapy (ART) care and retention in pre-ART care until ART initiation. We conducted a systematic review of pre-ART retention in care in Africa. METHODS AND FINDINGS: We searched PubMed, ISI Web of Knowledge, conference abstracts, and reference lists for reports on the proportion of adult patients retained between any two points between testing positive for HIV and initiating ART in sub-Saharan African HIV/AIDS car

A new analytical framework of 'continuum of prevention and care' to maximize HIV case detection and retention in care in Vietnam.

BACKGROUND: The global initiative 'Treatment 2.0' calls for expanding the evidence base of optimal HIV service delivery models to maximize HIV case detection and retention in care. However limited systematic assessment has been conducted in countries with concentrated HIV epidemic. We aimed to assess HIV service availability and service connectedness in Vietnam. METHODS: We developed a new analytical framework of the continuum of prevention and care (COPC). Using the framework, we examined HIV service delivery in Vietnam. Specifically, we analyzed HIV service availability including geographic

Differences in buprenorphine initiation and retention for opioid use disorder between primary care and substance use disorder specialty care settings.

INTRODUCTION: Buprenorphine, an evidenced-based medication for opioid use disorder (OUD), can be prescribed in any medical setting, including primary care (PC) and substance use disorder (SUD) specialty clinics. While research has identified patient characteristics associated with buprenorphine initiation and retention within both settings, between-settings evaluations have been limited. This evaluation examined differences in patient characteristics associated with buprenorphine initiation and retention between PC and SUD settings within the Veterans Health Administration. METHODS: Retrospec

Retention in care for persons with opioid use disorder transitioning from sublingual to injectable buprenorphine.

INTRODUCTION: In the current overdose epidemic, effective treatments for opioid use disorders (OUD), including innovations in medication delivery such as extended-release formulations, have the potential to improve treatment access and reduce treatment discontinuation. This study assessed treatment retention in a primary care-based, extended-release buprenorphine program. METHODS: The study recruited individuals (n = 92) who transitioned from sublingual buprenorphine to extended-release buprenorphine (BUP-XR) in 2018-2019. The study defined the primary outcome, treatment retention, as three o
