Catastrophic acute retinal necrosis syndrome associated with diffuse orbital cellulitis: a case report.

BACKGROUND: To report a case of severe acute retinal necrosis syndrome (ARN) associated with initial features of orbital cellulitis. METHODS: A 49-year-old immunocompetent woman presented with unilateral painful orbital inflammation associated with deep visual loss due to ARN. Polymerase chain reaction (PCR) for detection of viral DNA was applied to ocular fluids. Immunohistochemistry for detection of viral antigens and histopathologic studies were performed on orbital biopsy specimens. RESULTS: HSV-2 DNA was detected by PCR in ocular fluids. Orbital biopsy disclosed non-specific inflammatio

Fulminant bilateral acute retinal necrosis syndrome associated with viral encephalitis: A case report.

Herpes simplex virus (HSV) is the most common cause of acute viral encephalitis. Acute retinal necrosis (ARN) is a rapidly progressing and potentially blinding eye disease that may be induced by HSV. The present case study reports the very rare case of a patient with herpes simplex encephalitis (HSE) combined with acute retinal necrosis (ARN). A 47-year-old woman was admitted to hospital with persistent high fever and somnolence for 5 days. Magnetic resonance imaging showed abnormal signals in the right medial temporal lobes, and HSV-1 was identified in the serum and cerebrospinal fluid. Five

[Management of acute retinal necrosis syndrome with cystoid macular edema: a case report].

Acute retinal necrosis is a rare but devastating and rapidly progressive viral retinitis. We report the case of a healthy 46-year-old woman who presented bilateral acute retinal necrosis caused by varicella zoster virus (VZV). Intravenous and intravitreal antiviral therapy was given for 6 weeks and followed by oral therapy. After 3 months, the patient presented a bilateral macular edema which was treated first by laterobulbar injections of corticosteroids, then successfully by interferon alpha-2a. This clinical disease, which can cause major visual function impairment, must be known in order t

Association of herpes zoster ophthalmicus with acquired immunodeficiency syndrome and acute retinal necrosis.

We conducted a review to investigate the prevalence of human immunodeficiency virus (HIV), or acquired immunodeficiency syndrome (AIDS), in patients with herpes zoster ophthalmicus, as well as the incidence of acute retinal necrosis after herpes zoster ophthalmicus. All charts of patients seen at our institution between 1987 and 1992 with a primary diagnosis of herpes zoster ophthalmicus were reviewed. Of 112 patients with herpes zoster ophthalmicus, 29 (26%) had HIV or AIDS. All these patients were younger than 50 years at the time of diagnosis. Five of 29 (17%) immunocompromised patients had

Mild type acute retinal necrosis syndrome involving both eyes at three-year interval.

A 44-year-old man was seen as having acute retinal necrosis syndrome involving both eyes at an interval of 3 years. The cement-like exudates typical to this disease were observed in the fundus of both eyes; they were self-limiting, finally resulting in retinochoroidal degeneration without retinal detachment before becoming confluent. The patient showed an elevation of the convalescent antibody titer to varicella-zoster virus in the aqueous humor during the second eye involvement. We suggest that there exists a self-limiting mild type of acute retinal necrosis syndrome of which the present case

[Acute retinal necrosis syndrome. Lausanne cases, review of the literature and new physiopathogenetic hypothesis].

PURPOSE: The purpose of this study was to analyse our ARN-patients, perform an extensive review of the literature, suggest a physiopathogenic hypothesis for the disease that should influence the therapeutical approach of these cases. PATIENTS AND METHODS: From 1985 to 1993, 15 HIV-negative cases of ARN were seen in our clinic. Eleven cases were analysed prospectively: the herpetic agent involved in each case was searched for by the determination of intraocular specific antibody production and a complete immunological work-up was performed. RESULTS: Our collective included 4 cases of bilatera

Good visual outcome in an immunocompromised patient with bilateral acute retinal necrosis syndrome: A case report.

Acute retinal necrosis (ARN) is an uncommon necrotizing, fulminant retinopathy caused by the herpes simplex virus types 1 or 2 or by the varicella zoster vírus with visually devastating consequences. Generally it occurs in patients who are systemically healthy, but occasionally occurs in immunocompromised host. We report a case of bilateral ARN in a patient with AIDS with a good final visual outcome.

Varicella-zoster virus retinitis in a patient with AIDS-related complex: case report and brief review of the acute retinal necrosis syndrome.

Retinitis reminiscent of the acute retinal necrosis syndrome was recognized in a patient with AIDS-related complex after he had experienced several episodes of a sacral, dermatomic zosteriform eruption. Varicella-zoster virus (VZV) was subsequently recovered from cell culture of retinal tissue. The literature on VZV retinitis, including that on acute retinal necrosis, is reviewed. Dissemination of VZV infection in AIDS is also reviewed. Features that differentiate the findings and course of VZV retinitis in patients with AIDS from those in otherwise healthy adults are noted and related to pote

Acute retinal necrosis (ARN) following chickenpox in a patient of Vogt-Koyanagi-Harada (VKH) syndrome using immunosuppressants.

A 36-year-old woman presented with diminution of vision and floaters in both the eyes. Both eyes had disc oedema, multiple pockets of neurosensory detachments along with vitritis. Fluorescein angiography and optical coherence tomography showed characteristic features of Vogt-Koyanagi-Harada (VKH) syndrome (figure 1). She was started on corticosteroid pulse therapy and immunosuppressants following which her VKH lesions resolved. However, she developed chickenpox after 2 weeks and after 1 month she developed discrete yellowish white retinitis patches in the periphery of the right eye which were

Detection of varicella zoster virus DNA and viral antigen in the late stage of bilateral acute retinal necrosis syndrome.

We describe the clinicopathologic and virologic findings in the right, blind eye of an immunocompetent 61-year-old woman. The eye was enucleated 32 months after the clinical onset of a bilateral acute retinal necrosis syndrome. Histopathologic study showed a diffuse, full-thickness, necrotizing retinitis with replacement of sensory retinal structures by glial tissue, occlusive retinal arteritis, granulomatous choroiditis, and optic neuritis with ischemic optic atrophy. Varicella zoster virus could be identified as the causative agent by DNA in situ hybridization and by immunohistochemical stai
