[A macular hole in the eye of a young patient with retinitis pigmentosa].

PURPOSE: To describe the histological and cytological findings in the epiretinal membrane around a macular hole. CASE: A 24-year-old male patient with retinitis pigmentosa had had night blindness since childhood. A macular hole(Stage 2) in his left eye was noted when he was 18 years old. He visited our department when he was 22 years old. The visual acuity on the left was 0.5 at the first examination and two years later it was 0.2 with enlargement of the macular hole. Pars plana vitrectomy was applied, and we removed the thick yellow epiretinal membrane around the hole. The macular hole was c

Prophylactic scleral buckling in perforating ocular injuries involving the posterior segment.

Twenty patients suffering from perforating injury involving the posterior segment of the eye without retinal tear or detachment underwent posterior vitrectomy. In 12 of these where traction of the retina was suspected, prophylactic scleral buckling was performed as well. Only two of these patients developed delayed retinal detachment. Six of the eight patients who did not undergo scleral buckling at the time of the vitrectomy developed this complication. This finding was statistically significant (P = .015) and supports the use of prophylactic scleral buckling in the treatment of eyes with per

Vitrectomy in the treatment of ocular perforating injuries.

Using pars plana (closed) vitrectomy with related procedures we achieved improved vision in certain catagories of perforating ocular injuries. These procedures were particularly useful in removing retained nonmagnetic foreign bodies and correcting retinal detachments associated with traction bands and opaque vitreous. Limited success was obtained with longstanding injuries, double perforations from shotgun injuries, and large corneoscleral lacerations. Early vitreous surgery produced the best results.

Pathology characteristics of ocular von Hippel-Lindau disease with neovascularization of the iris and cornea: a case report.

INTRODUCTION: Retinal hemangioblastoma is one of the most common tumors in von Hippel-Lindau disease. In addition to the classical pathological characteristics of von Hippel-Lindau disease, we report, for what we believe to be the first time, a severe and rare ocular complication characterized by neovascularization in the cornea and iris. CASE PRESENTATION: A 41-year-old white man with a long history of retinal hemangioblastoma presented with neovascularization of his iris and cornea as well as corneal perforation. His right eye was blind and painful, leading to a decision of enucleation. On

Amniotic membrane transplantation in the treatment of macular hole secondary to neuroretinitis.

This case report aims to demonstrate the use of amniotic membrane transplantation (AMT) as a treatment for a refractory full-thickness macular hole (FTMH) that developed secondary to neuroretinitis. A 14-year-old girl presented to our hospital with a complaint of blurred vision persisting for one week. The patient was diagnosed with neuroretinitis due to cat scratch disease with a FTMH that developed secondarily. A pars plana vitrectomy, internal limiting membrane (ILM) peel, and ILM transplantation were performed. The FTMH did not close completely. The third surgery was performed using the AM

Ocular perforation in utero.

Midtrimester amniocentesis has rarely been reported to cause ocular damage. Findings in the literature include corneal perforation, a vascularized corneal leukoma, a non pigmented epithelial cyst of the anterior chamber, eyelid coloboma, microphthalmos, retinal hole, axial myopia and retinal detachment. We describe the case of a paralimbal scleral perforation with iris prolapse, distortion of the pupil, aphakia due to resorption of the lens and a chorioretinal scar possibly caused by an amniocentesis needle.

Macular Hole Formation Six Months after Hemorrhage Displacement for Submacular and Henle Fiber Layer Hemorrhage due to Retinal Arterial Macroaneurysm Rupture.

A 78-year-old woman presented with sudden vision loss and central scotoma. Visual acuity in the right eye was 20/222, with submacular hemorrhage (SMH) and Henle fiber layer hemorrhage (HFLh) due to retinal arterial macroaneurysm (RAM) rupture. She underwent SMH displacement, including cataract surgery, vitrectomy, intravitreal injection of tissue-plasminogen activator, and air tamponade. Three months postoperatively the SMH and HFLh had disappeared and visual acuity had improved to 20/200. Six months postoperatively, a macular hole had developed. We performed an inverted internal limiting memb

Longitudinal Observation of Two Giant Peripheral Atrophic Retinal Holes Left Untreated.

In this report, two rare cases of large atrophic peripheral retinal holes are described. Both patients presented during a routine visit without reporting any ocular symptoms. The holes did not exhibit significant risk factors for progression to rhegmatogenous retinal detachment: No signs of posterior vitreous detachment, vitreoretinal tractions, or retinal degeneration were visible. For such asymptomatic cases, international guidelines recommend a strict follow-up schedule; however, the unusual size of the holes raised significant concerns about the management of these retinal breaks. The adva

Clinical implications of heavy silicone oil (Densiron) endotamponade after pars plana vitrectomy: A systematic review and meta-analysis.

Rhegmatogenous retinal detachments (RRDs) with inferior retinal breaks (IRBs) pose significant surgical challenges, lacking a universally accepted treatment standard. Heavy silicone oils (HSOs), specifically Densiron® 68 and Densiron XTRA, have emerged as promising endotamponade agents because of their specific gravity, which is greater than that of water, allowing effective tamponade in the inferior retina. Unlike earlier HSO formulations, these newer variants show reduced rates of intraocular inflammation while maintaining favorable anatomical outcomes, even when compared with concomitant sc

Fresh retinal detachments--the role of scleral buckling.

Current concepts and methods of scleral buckling for fresh rhegmatogenous retinal detachments are reviewed in the light of alternative or additional approaches to retinal reattachment surgery. Increasingly, in difficult detachments, temporary internal air tamponade is replacing scleral buckling as a means of achieving initial closure of superiorly-located retinal breaks, though buckling often retains a role in thereafter maintaining break-closure by withstanding persisting vitreoretinal traction. Vitrectomy is also being employed more frequently in fresh detachments in order to relieve vitreor

[Prognosis of penetrating keratoplasty in secondary endothelial atrophy of diverse etiology].

Endothelial atrophy following cataract extraction is an increasingly important indication for perforating keratoplasty (PK). Other significant indications for PK are the treatment of endothelial atrophy following vitreous/retinal surgery for perforating injuries or primary retinal pathology. We evaluated the first year post-operative course of fourteen patients that underwent PK: 7 patients with silicon-oil induced keratopathy (group I), and 7 patients with endothelial atrophy following anterior segment surgical procedures (group II). On follow-up, group II exhibited a significant improvement

[Optic disc pit with maculopathy – a case report].

The article presents a case of an 18-year old man with bilateral optic disc pit associated with serous macular detachment in the left eye. Optic disc pit is a rare congenital abnormality of the optic nerve head, which affects 1:11 000 people, with no gender predilection. Optic disc pits are usually incidental findings on fundus examination. In ophthalmic examination, optic disc pit presents as an oval, gray, white or yellowish depression in the optic disc, commonly involving temporal quadrants, but may be situated in any sector. Approximately 25–75% of eyes with optic disc pits have a poor vis

[Minimalizing surgery of retinal detachment].

The prerequisites for minimizing detachment surgery are (1) dispensing with drainage by using elastic plug materials limited to the area of the tear and (2) improved diagnostic methods for detecting retinal tears. Various procedures are being developed on this basis in which the extent of surgery is dictated by the size of the tear alone, and no longer by the extent of the detachment. These procedures include (1) the cryosurgical detachment operation for treating several tears with individual, permanent plugs and (2) the balloon operation for single-tear detachments, with a temporary plug; bot

Inadvertent triple globe penetration during peribulbar anesthesia.

Inadvertent globe perforation following peribulbar anesthesia can lead to unpleasant experiences if not identified early and managed appropriately. We present the case of a 75-year-old female who came with decreased vision in the left eye (LE) following cataract surgery under peribulbar block. Her visual acuity in the right eye (RE) was 6/24 and LE was 6/75. Fundus examination of LE showed vitreous hemorrhage with localized subretinal hemorrhage along the inferotemporal arcade suggestive of globe perforation.
