Cryptococcal meningitis can present to the otologist with hearing loss and vestibular dysfunction. A man with cryptococcal meningitis presenting with bilateral profound sensorineural hearing loss and vestibular dysfunction is described. The difficulty in arriving at the diagnosis and consequences of misdiagnosis are discussed. Histological and clinical studies suggest that the cochlear nerve is likely to be damaged in patients who have deafness associated with this disease. Retrocochlear damage may result in cochlear implantation having a poor outcome.

In this report, performance intensity functions for monosyllabic materials (PI-PB) were obtained on two groups of subject: 20 patients with eighth nerve pathology and 15 individuals with Meniere's disease. The subjects with retrocochlear involvement exhibited marked deterioration in word recognition as the speech intensity was increased beyond the level required for a maximum discrimination score. Such a dramatic breakdown, referred to as the "rollover effect," was not seen to occur among the subjects with cochlear disorders. Hence, the PI-PB function appears to be a valuable tool for use in t

PURPOSE: Sudden idiopathic deafness is a sensorineural hearing loss with no recognized causes at the time of onset. The impairment site is usually localized in the cochlea, but some cases of retrocochlear lesions (e.g., cerebellopontine angle tumors, degenerative neural diseases, neuraxial ischemic lesions) can induce sensorineural deafness. The medical management of patients presenting with sudden deafness aims at detecting a causal mechanism, and at administering emergency therapeutic drugs. The diagnosis of idiopathic sudden deafness can be definitely made when no causes are found. Usually,

Hearing loss affects one in six people in the UK and is a significant disease burden. In addition to communication problems, there is also an association with depression and dementia. Clinical assessment with targeted history and examination can identify the characteristics and cause of hearing loss, and complementary audiological testing can confirm its type and severity. Retrocochlear screening is recommended for sudden, rapidly progressive or asymmetric sensorineural hearing loss. Medical or surgical therapies may be indicated in cases of conductive hearing loss, while hearing assistive dev

The audiometric data of 41 patients with Paget's disease of the bone show that hearing loss exceeds the age-related presbycusis in patients with deformities of the temporal bone as demonstrated by X-ray (27%). The usual type of hearing loss is a sensorineural one. 80% of these patients showed hair cell damage, 32% of them a retrocochlear lesion, depending on the degree of deformity of the temporal bone. Conductive hearing loss of the otosclerosis type was found in only 20%. The Paget-related hearing loss can be regarded as a consequence of the deformation occurring both at the bone surrounding

Acoustic reflex latency is the time interval between onset of an intense auditory stimulus and onset of middle-ear muscle contraction. The hypothesis was proposed that a retrocochlear lesion involving the afferent (VIIIth nerve) portion of the reflex arc should result in a prolongation of the reflex latency. A clinical procedure, the Acoustic Reflex Latency Test (ARLT), its protocol, methods and equipment were developed in order to test this hypothesis clinically. Absolute latency values and interaural latency differences (ILD) were established on 4 populations which included normals, cochlear

OBJECTIVES: To examine the evidence for a cochlear, retrocochlear, or central site of lesion for deafness in two cochlear implant recipients with Kearns-Sayre Syndrome (KSS).
DESIGN: Speech perception data and electrically evoked Auditory Brainstem (eABR) and Middle Latency Responses were obtained in two patients with KSS and compared with a group of non-KSS implant recipients.
RESULTS: Speech perception data and electrophysiological responses for the patients with KSS were similar to those obtained in non-KSS patients.
CONCLUSIONS: Results are consistent with an initial cochlear site of le

The ear is constituted of an apparatus for transmission, the external ear and the middle ear, and of an apparatus for perception, the inner ear or cochlea and the acoustic nerves. A disorder can occur at each of these levels, leading to different types of hearing loss: defective transmission through a disorder of the external canal and/or by disorder of the tympanic-middle ear bone apparatus; endocochlear hearing loss through disorder of the ciliary cells of the Corti ganglion and/or by metabolic disorder or by pressure modification (such as Meniere's disease) of the endolymph compartment of t

Analysis of the latency, amplitude, and rise characteristics of the stapedius reflex as demonstrated on a strip recorder revealed abnormalities in patients with vestibular neuronitis and other vertigo syndromes. The findings in vestibular neuronitis, acute labyrinthitis, perilymphatic fistulas, Meniere's disease, cerebrovascular insufficiency, and metabolic vertigo are described. Alterations in the stapedius reflex included increase in latency, decrease in amplitude, stepping of the rise, and absence of the reflex. This study suggests that absence of the reflex can be associated with vertigo a

The majority of magnetic resonance imaging (MRI) studies in neurotology concern the evaluation of retrocochlear pathologies or temporal bone lesions. Recently, quite a number of investigators have attempted to use imaging diagnosis to obtain a diagnosis and manifest the pathological findings of Meniere's disease. However, there is no evidence that endolymphatic hydrops in patients with Meniere's disease can be identified by imaging techniques. In this study we could depict Reissner's membrane clearer than before with the use of image processing. At the present time, we cannot apply the 2.11T M

Noninvasive meatal electrocochleography simultaneously recorded with brain stem auditory evoked potentials were performed on 24 adult patients with unilateral Meniere's disease. Data from the affected ear were compared with those of the nonaffected ear and with those of ten healthy normally hearing adults. The most striking finding was the significant deviation from the norm of the nonaffected ears' action potential duration. The combination of simultaneously recorded surface meatal electrocochleography and brain stem auditory evoked potentials complement the diagnostic battery of Meniere's di

We present the case of a 56 years old female controlled in our ENT Department because of right sudden hearing loss, tinnitus and vertigo. The endoscopic exam was normal. Audiogram showed a severe neurosensorial hypoacusia and PEATC confirmed the so called hypoacusia suggesting a retrocochlear origin of it. As imaging tests an otic-craneal CT and IRM were asked for, which informed of the presence of a cystic lesion versus polyp in sphenoidal sinus of 15 x 10 mm without other alterations or significant pathology.

Auditory brain stem evoked responses (ABRs) were recorded from six selected adult patients with various audiometric configuration, histories, and diagnoses. Although none of the patients were diagnosed medically as having retrocochlear disease, ABR latencies and waveform morphology were considered abnormal in some cases. By contrast, other patients with similar hearing loss configuration yielded normal ABRs. These observations emphasize the potential difficulty encountered in the interpretation of ABRs when hearing loss exists. Further, because ABRs and all other special auditory tests are not

The standard of reference for diagnosing and characterizing hearing loss is audiologic testing. The results of audiologic testing inform the imaging algorithm and the differential diagnosis for the underlying cause. Pure-tone audiometry tests the ability to hear tones across different frequencies, and the results are displayed as an audiogram. Tympanometry measures tympanic membrane compliance as a function of pressure to generate a tympanogram. Acoustic reflex testing helps differentiate third window lesions from other causes of conductive hearing loss. Clinical and audiologic assessment of s

There are two main kinds of hearing disorders: sensorineural and conductive. Sensorineural hearing loss is a common disorder that results from damage to the inner ear in over 95% of all cases; therefore, retrocochlear hearing disorders are rare and cannot be differentiated from sensory losses by clinical symptoms alone. Unilateral hearing loss entails many problems in hearing during the entire life of the affected patients. Conductive deafness has a readily determined etiology in most cases. In contrast, unilateral sensorineural hearing loss requires more refined and extensive investigation. T

In neurofibromatosis type 2 (NF2) bilateral vestibular schwannomas (VS) or their treatment usually results in bilateral hearing loss. Cochlear implantation (CI) was traditionally not used in these patients due to concern that retrocochlear disease would render the implant ineffective. This paper describes the auditory outcomes of CI in 13 patients with NF2 and includes patients with untreated VS and patients undergoing VS removal with cochlear nerve preservation. The non-user rate was 7.7%. Of the active users, median CUNY score was 98%, median BKB score in quiet was 90% and median BKB score i

Two cases of progressive bilateral hearing loss associated with superficial hemosiderosis of the central nervous system are reported. This is a rare disease caused by repetitive hemorrhage in subarachnoid spaces with hemosiderin deposits on the brain surface and cranial nerves. MRI provided the etiologic diagnosis in both cases based on typical low-density signals from the brain, the brainstem, and the cerebellar surfaces on T1 and T2 sequences. In one case a fourth ventricle ependymoma, which was the probable cause of hemosiderosis, was also discovered. Based on a literature review, we discus

Auditory brainstem response (ABR) is a good screening test for cerebellopontine angle (CPA) tumors and other retrocochlear diseases. The pathologic correlates of many cases with positive ABR remain unknown. A two-staged clinical study was performed to help us understand the significance of a false-positive ABR. In the first stage, data from 66 patients with abnormal ABR were reviewed. Nineteen patients had identifiable causes for the ABR abnormality, whereas 47 patients had no lesions identified. In the second phase of the study 21 of the 47 patients were re-evaluated with a minimum follow-up

The audiological findings in 125 patients with surgically confirmed acoustic neuromas are presented. Following the classification by Pulec et al. (1971) we found 20 medium-size and 105 large tumours; small tumours were not represented. A clear connection between the degree of hearing loss and tumour size was noted, while there was no correlation between duration of history and tumour size. An attempted evaluation of the different audiological tests (ABLB and Metz recruitment test, speech discrimination scores, tone decay, reflex decay, and Békésy tracings) that were applied to the patients, ha
