Retropharyngeal Abscess Presenting With Sleep Apnea Syndrome.

Retropharyngeal abscess (RPA) is the second most common deep neck space infection after peritonsiller abscess in pediatric population. Major signs and symptoms on physical examination include fever, hypersalivation, odynophagia, reduced oral intake, sore throat, swelling on the neck, torticollis, limitation in neck mobility, and voice changes. In this paper, the authors present a case of RPA with unusual and interesting presenting symptoms in a 10-month-old infant that exhibit new-onset and worsening snoring and sleep apnea. The purposes of this manuscript are to present the authors' experienc

DESCENDING NECROTIZING MEDIASTINITIS SECONDARY TO RETROPHARYNGEAL ABSCESS.

Descending necrotizing mediastinitis secondary to a nontraumatic retropharyngeal abscess is very rare. This form of mediastinitis in the era of potent antibiotics often ends up with lethal outcome. It usually occurs in immunocompromised patients and requires intensive multidisciplinary treatment approach. We report a case of nontraumatic retropharyngeal abscess complicated by descending necrotizing mediastinitis in a 70-year-old man with insulin dependent diabetes mellitus. The patient was admitted to our hospital after clinical and radiological diagnosis of retropharyngeal abscess. During tre

Retropharyngeal abscess complicating 'innocent' foreign body ingestion.

An adult patient presented to the emergency department with pharyngeal discomfort on swallowing, persisting several hours after lunch. Transnasal fibre-optic endoscopy performed by an otolaryngologist identified a hypopharyngeal foreign body, and the stalk of a dry leaf partially penetrating the mucosa was easily removed under general anaesthesia. Symptoms regressed completely and the patient was discharged. Two days later he presented again, reporting slight dysphagia without odynophagia or other associated symptoms. Meticulous physical examination by the same otolaryngologist revealed this t

National trends in retropharyngeal abscess among adult inpatients with peritonsillar abscess.

OBJECTIVES: To describe national trends in retropharyngeal abscess (RPA) complicating peritonsillar abscess (PTA) and to determine factors associated with RPA in patients with PTA. STUDY DESIGN: Cross-sectional analysis. SETTING: Nationwide Inpatient Sample, 2003-2010. SUBJECTS AND METHODS: PTA patients ≥18 years old, with or without RPA, were extracted according to ICD-9-CM codes. The cohort was analyzed with descriptive statistics and multivariate regression modeling to identify factors associated with RPA. RESULTS: Of the 91,647 (95% CI: 86,433-95,449) patients identified with PTA, 885

Differentiation of retropharyngeal calcific tendinitis and retropharyngeal abscess: a case series and review of the literature.

INTRODUCTION: Retropharyngeal calcific tendinitis (RCT) is a self-limiting aseptic inflammation of the tendon of the longus colli muscle, which can be clinically and radiologically misdiagnosed as abscess formation. This is a particular challenge for ENT specialists. However, articles about RCT are highly underrepresented in ENT journals and existing articles in ENT journals almost exclusively report overtreatment. METHODS: This study presents five patients, in which the diagnosis of RCT was delayed and of which one patient underwent incision and draining of a suspected retropharyngeal absces

Large retropharyngeal abscess: a case of mistaken identity.

We report an unusual case of nasopharyngeal carcinoma (NPC), presenting initially as a retropharyngeal collection. We discuss the investigation, diagnosis and management of NPC and furthermore review the literature of NPC, emphasising the varied presentation. A 44-year-old Caucasian male ex-smoker presented with a 10-day history of a painless left-sided neck lump; progressive dysphagia and unintentional weight loss. Examination demonstrated a large left posterior pharyngeal swelling with soft palatal effacement, confirmed on nasoendoscopy. A CT scan revealed a fluid collection in the retrophar

Hearing loss unusually caused by tubercular retropharyngeal abscess.

A 25-year-old man with complaints of neck pain for approximately 6 months and hearing loss accompanied by a feeling of obstruction of the left ear was presented. Findings from the oral cavity examination showed a retropharyngeal fluctuant, mildly tender mass. A conductive hearing loss of approximately 45 dB and a flat-shaped (type B) tympanogram were detected on the left ear. A magnetic resonance image of the neck demonstrated an abscess occupying the retropharyngeal space. Retropharyngeal space abscess was drained and sent both for pathologic and microbiologic analyses. Myringotomy and gromme

Descending necrotizing mediastinitis secondary to a retropharyngeal abscess.

A rare case of descending necrotizing mediastinitis (DNM) secondary to a nontraumatic retropharyngeal abscess is reported. Even in the era of antibiotics, the mortality of DNM is still around 40%. In spite of drainage of a localized neck abscess, and the administration of systemic antibiotics, the retropharyngeal abscess extended to the pericardial and pleural cavities. Several drainage procedures and thoracotomies were performed to treat the bilateral empyemas and purulent pericarditis. The patient was discharged on hospital day 52. Computed tomography was used to follow the progression of di

Deep Neck Infection: A Case of Retropharyngeal Abscess.

The incidence of retropharyngeal abscesses has been decreasing since the introduction of antibiotic therapy, and it is currently a rare diagnosis in adults, although there are some recent cases in the literature. Given its seriousness, if not treated promptly, the infection can progress rapidly and its complications can be fatal, making it a serious health problem. A 79-year-old woman presented at her primary care center with complaints of persistent odynophagia for about two weeks and rapidly progressive dysphagia in five days, initially for solids and later for solids and liquids. On observa

Retropharyngeal Abscess. Clinical Review of Twenty-five Years.

INTRODUCTION: Retropharyngeal abscess is a serious condition. Its rare occurrence, thus sharing symptoms with other processes, make it a diagnostic challenge for the clinician. Therefore, it is critical to make an early diagnosis to prevent delaying treatment and avoid complications. OBJECTIVES: To gain knowledge of the epidemiology, pathogenesis, clinical manifestations, the most commonly implicated microorganisms, the type of treatment used, morbidity and mortality of retropharyngeal abscesses at a tertiary institution over the last 25 years. METHODS: A retrospective study was conducted by

Adult Recurrence of Kawasaki Disease Mimicking Retropharyngeal Abscess.

Kawasaki disease (KD) is an acute, self-limited vasculitis of unknown etiology that occurs predominantly in young children (≤5 years of age). We herein report the case of an 18-year-old Japanese man with a history of incomplete KD during infancy; later, despite an initial diagnosis of retropharyngeal abscess, he was ultimately diagnosed with retropharyngeal edema associated with recurrent KD. Adult-onset or recurrent KD is an uncommon event, and retropharyngeal edema is a rare manifestation of this disease. Internists should be aware of the possibility of KD that mimics a retropharyngeal absce

Asymptomatic retropharyngeal abscess related to cervical Pott's disease.

Pott's disease is an uncommon manifestation of tuberculosis, which usually involves thoracic or lumbar vertebrae. The body of the vertebrae is most severely affected and a compression fracture is an almost inevitable consequence of the disease. A paravertabral abscess generally accompanies vertebral involvement. Tenderness over the involved vertebrae, weakness of the related muscles, and paraesthesia are the usual symptoms. In this article, we report a case of cervical Pott's disease presenting mainly with neurologic symptoms such as weakness, pain, numbness of both arms and hands, and an asym

An unusual cause of stridor: retropharyngeal cold abscess.

A 15-month-old female presented with a large swelling on the left lateral aspect of the neck and respiratory distress, and stridor due to incomplete obstruction of the upper airway. A lateral X-ray film and computerized tomography scan of the cervical spine showed a retropharyngeal abscess without vertebral involvement. Aspiration and contrast revealed it to be a bilobed tubercular abscess. Planned external drainage was done after 10 days under anti-tubercular drugs.

[Torticollis after unnoticed pharyngeal perforation: suspected retropharyngeal abscess].

Pharyngeal perforations due to foreign bodies are severe when retropharyngeal space infection develops into an abscess. Although the common clinical presentation is sore throat and fever, when the perforation remains occult, torticollis can be a significant symptom. Palatopharyngeal lesions, caused by rigid objects' impactation through the mouth, are common in childhood. Most are not important, but all have the potential hazard of pharyngeal perforation with development of a retropharyngeal abscess. It must be suspected specially when air is shown in the retropharyngeal space on a lateral radi

Retropharyngeal abscess after blunt trauma in an adolescent.

Retropharyngeal abscess is an unusual infection in teenagers, and it is rarely associated with blunt trauma. We present the case of 17-year-old male who developed signs and symptoms of retropharyngeal abscess soon after being struck by a car. The case illustrates that retropharyngeal abscess should be considered, even in an adolescent, if the patient has fever, neck stiffness, and sore throat with symptoms out of proportion to the oropharyngeal findings.
