A Literature Review Investigating the Use of Topical Janus Kinase Inhibitors for the Treatment of Vitiligo.

BACKGROUND: Vitiligo is a disfiguring disease that frustrates both patients and clinicians due to its difficulty to treat. Topical Janus kinase inhibitors (TJKI) may offer an alternative treatment option that are safe and well-tolerated for all anatomic sites. OBJECTIVE: The objective of this review was to evaluate the published clinical reports regarding the use of TJKI for the treatment of vitiligo and to summarize the formulations, dosing strategies, efficacy, and safety of this emerging therapeutic class. METHODS: This is a review of the National Library of Medicine (via PubMed) and Scop

Systematic literature review of topical local anaesthesia or analgesia to donor site wounds.

BACKGROUND: Topical local analgesic and anaesthetic agents have been used both pre- and immediately post-harvest on split-thickness skin graft (STSG) donor site wounds (DSW). There is no systematic review of their effectiveness in providing post-harvest analgesia, or of the possible toxic effects of systemic absorption. This study is designed to address the question of which agent, if any, is favoured over the others and whether there are any safety data regarding their use. METHODS: Systematic literature review of randomised controlled trials of topical agents applied to STSG DSWs, with a vi

Mucosal side effects in patients treated with topical imiquimod-A scoping review of the literature.

Imiquimod 5% is approved for topical treatment of actinic keratosis (AKs), superficial basal cell carcinoma and condylomata acuminata, the 3.75% formulation for the treatment of AKs and genital warts. Imiquimod has also been used off-label in various other skin conditions (eg, Bowen's disease, lentigo maligna, vulvar intraepithelial neoplasia). As a toll-like receptor 7/8 (TLR7/8) agonist imiquimod induces a local inflammatory response by increased production of cytokines, co-stimulatory molecules, activation of Nk-cells and antigen-specific T-cells. In addition to imiquimod-associated adverse

Iatrogenic Cushing's syndrome and topical steroid therapy: case series and review of the literature.

Topical corticosteroids are considered first-line therapy in patients with chronic inflammatory oral mucosal diseases; among them, clobetasol propionate is one of the most widely used in oral medicine. Under physiological conditions, the transmucosal application is characterized by a significantly greater absorption than the skin application. Contrary to many publications about the side effects of topical corticosteroids in dermatology, few studies have investigated the systemic effects due to local application of these drugs on oral mucosa. Although topical steroid therapy for the management

Treatment of vulvar Paget disease with topical imiquimod: a case report and review of the literature.

BACKGROUND: Extramammary Paget's disease is a cutaneous neoplasm that presents as erythematous crusted patches and plaques reminiscent of contact dermatitis or inverse psoriasis that can be a challenge to treat in a tissue-sparing manner. The most commonly involved site for this rare disorder is the anogenital region. Surgery is considered as the gold standard therapy. In the last years, the topical use of imiquimod cream in the treatment of this condition has been reported. MAIN OBSERVATIONS: We present a case of a 59-year-old woman with primary extramammary Paget's disease of the vulva, in

Topical antibiotic treatment for uncomplicated skin and skin structure infections: review of the literature.

Uncomplicated skin infections account for almost 200 million physician-office visits in the USA annually. Treating these infections is estimated to cost in excess of US$350 million each year. The primary etiology of these infections is Staphylococcus aureus, over 60% of which is estimated to be methicillin resistant across the USA. Therapeutic options include incision and drainage in combination with antimicrobial therapy, which may be oral, topical or occasionally parenteral. Because the effectiveness of the current oral options, such as beta-lactams and other classes, is being eroded due to

[Topical treatment of bullous pemphigoid with tacrolimus. Case report with brief literature review].

Bullous pemphigoid (BP) is a cutaneous autoimmune disease predominantly affecting older patients which can cause death either due to severe clinical manifestations or due to the side effects of systemic immunosuppressive treatment. Topical treatment with corticosteroids is an established alternative to systemic treatment. However, prolonged application is accompanied by side effects such as skin atrophy. Recently, the immunomodulatory calcineurin antagonists tacrolimus and pimecrolimus have been introduced for topical treatment of skin diseases. Tacrolimus has been reported to be effective in

Balanitis Xerotica Obliterans Successfully Treated with Topical Tacrolimus: A Case Report and Literature Review.

Balanitis xerotica obliterans (BXO) is a chronic, progressive, inflammatory, and sclerosing disease of the male genitalia. If left untreated, it can lead to urinary and sexual dysfunction and reduced quality of life. We describe a 15-year-old Caucasian male with a 3-month history of progressive thickening and whitening of the prepuce and penile glans. He reported mild pruritus, embarrassment of the prepuce retraction, painful cracking of the foreskin, and painful erection. Dermatological examination showed mild erythema and hypopigmentation of the foreskin and penile glans, a sclerotic white r

Off-Label Use of Topical Ruxolitinib in Dermatology: A Systematic Literature Review and Current Perspectives.

JAK inhibitors are used to treat various inflammatory skin diseases. However, systemic formulations are associated with an increased risk of major adverse events. Ruxolitinib 1.5% cream is a selective topical JAK1 and JAK2 inhibitor, which has recently been approved by EMA and MHRA for treating non-segmental vitiligo, while being FDA-approved for both vitiligo and atopic dermatitis. Recent literature has reported the off-label use of topical Ruxolitinib for several skin conditions, but data are mostly limited to single case reports and series and few prospective studies, with mixed results. Keratosis pilaris rubra successfully treated with topical sirolimus: Report of a case and review of the literature.

Keratosis pilaris rubra (KPR) is a subtype of keratosis pilaris (KP) presenting with numerous "grainlike" follicular papules in a background of confluent erythema most often affecting the face and upper extremities with persistence beyond puberty. Treatment has remained challenging with inconsistent benefit from topical therapies such as emollients, keratolytics, corticosteroids, and retinoids, though case reports documenting success with pulsed dye laser therapy have been found. We present a case of KPR in a 15-year-old boy who was successfully treated with topical sirolimus 1% cream.

Literature review and meta-analysis of topical carbonic anhydrase inhibitors and ocular blood flow.

The purpose of this literature review and meta-analysis was to determine what, if any, effects topical carbonic anhydrase inhibitors have on ocular hemodynamics in humans. A literature review and meta-analysis was conducted to investigate the effects of topical carbonic anhydrase inhibitors on ocular blood flow. Thirty-five articles were evaluated according to the inclusion criteria with 13 manuscripts meeting requirements for statistical analysis. Each study's effect size, defined as the change in blood flow measures after treatment with topical carbonic anhydrase inhibitors, was estimated us

Peripheral precocious puberty due to inadvertent exposure to topical oestradiol gel and review of the literature.

IntroductionTopical oestradiol gel is now widely used in hormone replacement therapy (HRT), but the risk of inadvertent secondary transfer to children is under-recognised. While accidental exposure to testosterone gels has been documented, oestradiol gel transfer as a cause of precocious puberty in prepubertal girls is rare.We undertook a 20-year literature review and discuss the case of a 5-year-old girl presented with Tanner stage 3 breast development, pubic hair stage 2, growth acceleration and mood changes. Laboratory evaluation revealed markedly elevated oestradiol levels with suppressed

Quinolone immediate hypersensitivity due to topical ophthalmic preparations: a case report and review of literature.

Ophthalmic quinolone preparations are widely used for the treatment of eye diseases; however, there are only a few reported cases of immediate hypersensitivity due to such medications. Here, we present a case of immediate hypersensitivity induced by an ophthalmic administration of quinolones in a 20-year-old female with bacterial conjunctivitis. The results of skin prick and basophil activation tests suggest that it involved a type Ib reaction associated with Mas-related G-protein coupled receptor X2. The present case cautions us to be aware of quinolone immediate hypersensitivity caused by to
