[So-called "pseudorheumatoid" nodules in childhood (author's transl)].

This report concerns 4 patients, 2 to 2,5 years of age, with subcutaneous "pseudorheumatoid" nodules in the occipital region. None of the children suffered from any real rheumatic or rheumatoid disease, nor were there any biochemical signs of those diseases detectable. The course of the nodules was benign, they disappeared within several years, and no later rheumatic disease occurred. Extirpation and histology was performed in all 4 cases, the microscopic diagnosis being uniformly "rheumatic granuloma". The own observations are confronted with the literature. It is concluded that, inspite of t

Pseudorheumatoid subcutaneous nodules and the poststreptococcal state. A case report.

A 10-year-old girl with an acute illness fulfilling the Jones criteria for rheumatic fever is described. She had subcutaneous nodules, arthritis, an elevated ESR, and serologic evidence of a recent streptococcal infection. Her clinical course was benign and at ten weeks, the nodules disappeared, the joint pains had subsided, and the ESR was normal. Thus, there was little to support a diagnosis of rheumatic fever. We hypothesize that the patient had pseudorheumatoid subcutaneous nodules coincidental with poststreptococcal arthritis. A revision of the Jones criteria disallowing subcutaneous nodu

Cutaneous Nodules and Inflammatory Arthritis: Two Illustrative Cases of Rare Mimics of Rheumatoid Arthritis.

Background: Rheumatoid arthritis is a relatively common rheumatic disease that can present with inflammatory arthritis and subcutaneous nodules. Multicentric reticulohistiocytosis and fibroblastic rheumatism are rarer entities that also present with these features. Methods: Two cases, one of each of fibroblastic rheumatism and multicentric reticulohistiocytosis, are described highlighting characteristic clinical, radiographic, and histologic findings. A narrative review of the literature on these rarer conditions, compared with rheumatoid arthritis, is provided with a focus on articular and cu

Deep granuloma annulare in adults.

Subcutaneous nodules with the histopathologic features of rheumatoid nodules can occur in both rheumatic and nonrheumatic diseases. When there is no evidence of systemic disease, they often are classified as deep granuloma annulare. While there are numerous reports of such nodules occurring in children without systemic disease, there are few reports of similarly affected adults. Two adults are described with multiple subcutaneous nodules that were overlying joints and were histologically similar to rheumatoid nodules. Neither patient had other clinical or laboratory manifestations of rheumatic

[Pseudorheumatoid nodules in childhood. A case report].

Subcutaneous nodules of the rheumatic type can occur in patients without other evidence of rheumatic disease. Nodules are characterized by subcutaneous location with predilection for pretibial regions and scalp, occasional large size, spontaneous regression and frequent recurrence. Sometimes lesion biopsy may be usefull for confirmation of the clinical diagnosis; etiology is unknown, therapy is unnecessary. One asymptomatic child with benign rheumatoid nodules is described by the Authors.

[A rare form of rheumatic lung disease].

Rheumatic changes of the lung are not usual findings, but may accompany rheumatoid arthritis in the form of interstitial fibrosis or pleurisy. The extent of the rheumatic changes depends on the duration of the illness. Necrobiotic nodules in the lung are found very rarely indeed, but if so then usually in the lower lobe of the lung. A case of necrobiotic nodules in the upper lobe of the lung is described.

Histopathology of auricular appendages in rheumatic heart disease.

Forty-four left auricular appendages removed from patients presenting clinically as rheumatic heart disease (RHD) were subjected to histopathologic examination. The aim was to identify the changes so far reported in RHD. It was found that Aschoff's nodules, though specific for RHD, are much less frequent than the other changes reported. Viewing the histopathologic alterations in the perspective of clinical data is important in identifying the rheumatic process.

[Does methotrexate induce "rheumatoid nodules"? Case report and discussion of the literature].

A 58-year-old woman with chronic polyarthritis rapidly developed a large number of rheumatic nodules under treatment with methotrexate. A comparison with other cases reported to date showed that the sudden onset of such nodules after initiation of treatment, and their disappearance again after discontinuation of the drug make the causal relationship highly likely. Although the predominantly anti-inflammatory actions of methotrexate control the chronic polyarthritis in almost all cases so far reported, they are unable to prevent the primarily immunological tissue necrosis (rheumatic nodules) an

Disease activity and subcutaneous nodules are associated to severe periodontitis in patients with rheumatoid arthritis.

Rheumatoid arthritis (RA) was significantly associated with increased overall risk of periodontitis, both chronic, inflammatory pathologies leading to connective tissue breakdown and bone destruction. To identify clinical and/or serological variables routinely evaluated during follow-up of people with RA which are associated with the severity of their periodontal disease. An observational, cross-sectional study was carried out, which included RA patients according to ACR/EULAR 2010 criteria having chronic periodontal disease. RA clinical parameters (disease duration, erythrocyte sedimentation

Chronic active hepatitis presenting with rheumatoid nodules and arthritis.

Chronic active hepatitis is often accompanied by extrahepatic rheumatic symptoms of mild or moderate severity. We report a 16-year-old girl with this disorder who presented with severe polyarthritis and rheumatoid nodules mimicking juvenile rheumatoid arthritis. Rheumatoid nodules have not previously been described in chronic active hepatitis, and the severity of the rheumatic symptoms in our patient led to a delay in the recognition of the underlying liver disease. Our report provides support for an autoimmune etiology in some patients with chronic active hepatitis, notes the occurrence of rh

A Case Report: Acute Rheumatic Fever or Something More?

Acute rheumatic fever (ARF) is an autoimmune response that may occur after a group A Streptococcus (GAS) infection. Subcutaneous nodules are considered a rare manifestation of acute rheumatic fever with an incidence of 0%-10%. We present a case study of a 13-year-old girl who presented to us with subcutaneous nodules and articular involvement described as a non-migratory polyarticular joint pain involving the small joints of the hands, wrist, elbows, knees, and ankles for three months with poor response to the non-steroidal anti-inflammatory drug (NSAID) Ibuprofen. Accompanied with the presenc

Rheumatoid nodules. Review of the spectrum of associated conditions and proposal of a new classification, with a report of four seronegative cases.

The literature relating to prevalence, characteristics, differential diagnosis, histologic features, pathogenesis, complications, and prognosis of rheumatoid nodules is tabulated and reviewed. The broad spectrum of conditions associated with rheumatoid nodules is discussed. A new classification of these clinical entities, reflecting disease characteristics and prognosis, is proposed. This classification is based on age, association or lack thereof with rheumatoid arthritis or rheumatic features, the presence or absence of rheumatoid factors, and the location of the rheumatoid nodules. Four cas

[The pathologists: Aschoff, Klinge and Gräff].

The article presents three pathologists who have made important contributions to the understanding of rheumatic diseases. Ludwig Aschoff's (1866-1942) work formed the foundation and was for a long time at the centre of the discussion on pathology of rheumatic diseases. Impetus was added from a rheumatological perspective by the discovery of the Aschoff nodule as an indicator of rheumatic myocarditis. Thus, newly manifested rheumatic carditis is only diagnosed when rheumatic nodules are found. Fritz Klinge (1892-1974), following in Aschoff's footsteps, broke new ground in rheumatology in German
