[Rhodococcus equi, first case report of disseminated disease in Peru].

We present the case of a patient with human immunodeficiency virus (HIV) with a LTCD4 + 49 cells/mm3, who was admitted due to a seven-month period of weight loss, abdominal pain, chronic diarrhea and rubbery skin lesions. Myeloculture and blood cultures were positive for Rhodococcus equi. In addition, histological lesions in the skin and intestine compatible with this agent were observed, such as malacoplachy, granulomatous reaction and Michaelis-Gutmann bodies. Pulmonary involvement was ruled out by chest tomography. The patient received antibacterial therapy combined with clarithromycin, imi

Distribution and antimicrobial susceptibility of Rhodococcus equi from clinical specimens.

Rhodococcus equi, an unusual gram positive aerobic actinomycete, was first described as a respiratory pathogen of livestock in 1923. Reports of human clinical illness have emphasized R. equi as a cause of invasive pulmonary infection in severely immunocompromised patients and, recently, have implicated it as a cause of pneumonia, bacteremia and disseminated infection in HIV-infected patients. To determine the distribution of R. equi we evaluated 107 isolates referred to the Centers for Disease Control (CDC) during the period January 1973 through December 1990. The sites of these 107 isolates (

Antimicrobial susceptibility of Rhodococcus equi strains isolated from foals in Chile.

Rhodococcus equi is responsible for foal pneumonia worldwide, with a significant economic impact on the production and breeding of horses. In Chile, the first case was reported in 2000, and since then, its incidence has been increasing. Distinctive characteristics of R. equi as an intracellular pathogen in macrophages, emergence of virulence plasmids encoding surface lipoprotein antigens, and appearance of antibiotic resistance against macrolides and rifampicin have significantly complicated the treatment of R. equi pneumonia in foals. Therefore, in vitro susceptibility studies of first-line a

Rhodococcus equi foal pneumonia: Update on epidemiology, immunity, treatment and prevention.

Pneumonia in foals caused by the bacterium Rhodococcus equi has a worldwide distribution and is a common cause of disease and death for foals. The purpose of this narrative review was to summarise recent developments pertaining to the epidemiology, immune responses, treatment, and prevention of rhodococcal pneumonia of foals. Screening tests have been used to implement earlier detection and treatment of foals with presumed subclinical R. equi pneumonia to reduce mortality and severity of disease. Unfortunately, this practice has been linked to the emergence of antimicrobial-resistant R. equi i

Transfusion of hyperimmune plasma for protecting foals against Rhodococcus equi pneumonia.

The bacterium Rhodococcus equi causes pneumonia in foals that is prevalent at breeding farms worldwide. In the absence of an effective vaccine, transfusion of commercial plasma from donor horses hyperimmunised against R. equi is used by many farms to reduce the incidence of pneumonia among foals at farms where the disease is endemic. The effectiveness of hyperimmune plasma for controlling R. equi pneumonia in foals has varied considerably among reports. The purposes of this narrative review are: (1) to review early studies that provided a foundational basis for the practice of transfusion of h

Pleuropneumonia and mediastinitis caused by Rhodococcus equi in a dog: a case report.

Rhodococcus equi is a cosmopolitan bacterium found in the feces of several animals, and it is recognized as a major cause of bronchopneumonia in foals, as well as an opportunistic pathogen causing infections in humans and, rarely, in other species. This report describes a rare and atypical infection by Rhodococcus equi in a mixed-breed dog, associated with a pyogranulomatous pleuropneumonia and mediastinitis with abundant intralesional Gram-positive coccobacilli. The bacterium was isolated and identified as Rhodococcus hoagii (R. equi) using matrix-assisted laser desorption ionization-time of

Cutaneous Rhodococcus equi Infection in an Immunocompetent Host: A Case Report.

Rhodococcus equi is a zoonotic Gram-positive bacterium that can affect immunocompetent hosts and only rarely presents with cutaneous lesions resembling other granulomatous dermatoses, such as cutaneous mycobacterial infections, deep fungal infections, sarcoidosis, and foreign body granulomas. This report describes a 53-year-old immunocompetent woman who developed recurrent ulcerated nodules on the left lower extremity following recreational travel to southern Mexico. Initial diagnostic studies were nondiagnostic, and molecular analysis using polymerase chain reaction targeting the 16S rRNA gen

Seroepidemiological survey of Rhodococcus equi infection in adult horses in Mongolia.

ELISA antibody titers against Rhodococcus equi strain ATCC 6939 were measured in serum samples collected from 644 horses aged 1-21 years at 13 provinces and the capital of Mongolia, of which 631 (98%) samples showed values of approximately zero. The mean antibody titers in the 14 areas ranged from -0.08 to -0.01. Nine samples (1.4%) exhibited titers of 0.1-0.2, and surprisingly, four horses (0.6%) each aged 2, 5, 8, and 9 years demonstrated positive values in the range of 0.3-0.8. Considering the age resistance characteristic of this disease, the positive horses were hypothesized to exhibit no

Rhodococcus equi Endocarditis in Prosthetic Valve of a Heart Transplant Recipient.

BACKGROUND: Rhodococcus equi are gram-positive bacteria found in the soil and in animal manure. R. equi infection causes pneumonia and bacteremia predominantly in immunocompromised hosts. CASE SUMMARY: A 57-year-old male heart transplant recipient with a bioprosthetic aortic valve presented with 6 weeks of cough, diarrhea, and weight loss. He was found to have cavitary pneumonia and persistent R. equi bacteremia. Transesophageal echocardiogram demonstrated thickening of the bioprosthetic valve with mild regurgitation, and mobile echodensity was later seen on transthoracic echocardiogram. Disc

Ulcerative, granulomatous glossitis and enteritis caused by Rhodococcus equi in a heifer.

Rhodococcus equi infection in horses is common and is characterized by pyogranulomatous pneumonia and ulcerative enterocolitis. R. equi clinical disease in cattle, however, is rare and typically manifests as granulomatous lymphadenitis discovered in the abattoir. A 19-mo-old female Santa Gertrudis had a history of intermittent inappetence and weight loss for a 3-mo period before euthanasia. Gross and histologic examination revealed severe, chronic, ulcerative, and granulomatous inflammation in the tongue, pharynx, and small intestine. Also, the heifer had severe, granulomatous pharyngeal and m

Vertebral osteomyelitis due to Rhodococcus equi in a liver transplant recipient.

Rhodococcus equi is a rare but well-documented cause of cavitary pneumonia in immunocompromised patients. In this report the first case of R. equi infection manifesting as vertebral osteomyelitis is described. A 39-year-old liver transplant recipient presented with recurrent pneumonia and a pleura-based lung abscess and subsequently developed osteomyelitis of the lower thoracic spine. Surgical debridement and prolonged treatment with rifabutin and clarithromycin resulted in clinical cure. In the literature, 12 other cases of R. equi infection in solid-organ transplant recipients have been repo

Fatal Infection in an Alpaca (Vicugna pacos) Caused by Pathogenic Rhodococcus equi.

Rhodococcus (R.) equi is a pathogen primarily known for infections in equine foals, but is also present in numerous livestock species including New World camelids. Moreover, R. equi is considered an emerging zoonotic pathogen. In this report, we describe in detail a fatal rhodococcal infection in an alpaca (Vicugna pacos), to our best knowledge, for the first time. The alpaca died due to a septicemic course of an R. equi infection resulting in emaciation and severe lesions including pyogranulomas in the lungs and pericardial effusion. The onset of the infection was presumably caused by aspirat

Development of septic polysynovitis and uveitis in foals experimentally infected with Rhodococcus equi.

Rhodococcus equi is one of the most important causes of disease in foals. Infection is typically characterized by pyogranulomatous pneumonia although extrapulmonary infections occur occasionally. Uveitis and polysynovitis have been reported in foals naturally infected with R. equi and are thought to be the result of an immune-mediated process. However, the pathogenesis of these conditions is poorly understood. The objectives of this study were to document the occurrence of uveitis and polysynovitis after experimental infection with R. equi and to determine if these disorders are the direct res

Immunogenicity of an electron beam inactivated Rhodococcus equi vaccine in neonatal foals.

Rhodococcus equi is an important pathogen of foals that causes severe pneumonia. To date, there is no licensed vaccine effective against R. equi pneumonia of foals. The objectives of our study were to develop an electron beam (eBeam) inactivated vaccine against R. equi and evaluate its immunogenicity. A dose of eBeam irradiation that inactivated replication of R. equi while maintaining outer cell wall integrity was identified. Enteral administration of eBeam inactivated R. equi increased interferon-γ production by peripheral blood mononuclear cells in response to stimulation with virulent R. e

Penicillin-binding proteins of Rhodococcus equi: potential role in resistance to imipenem.

Rhodococcus equi is a gram-positive coccobacillus which, like other members of the order Actinomycetales, is increasingly reported as an opportunistic pathogen in patients with AIDS. The use of combinations of antibiotics that include imipenem (IMP) has been suggested for the treatment of patients infected with R. equi. An antagonism between IMP, meropenem, cefoxitin, ceftriaxone, moxalactam, and oxacillin and other beta-lactams, such as penicillin, amoxicillin, cephalothin, and ticarcillin, was detected in vitro both on Mueller-Hinton agar and in broth for all 10 IMP-susceptible R. equi strai

Cellulitis-related Rhodococcus equi in a cat harboring VAPA-type plasmid pattern.

Rhodococcus equi is a well-known intracellular facultative bacterium that is opportunistic in nature, and a contagious disease-causing agent of pyogranulomatous infections in humans and multihost animals. Feline rhodococcosis is an uncommon or unnoticed clinical condition, in which the organism is usually refractory to conventional antimicrobial therapy. The pathogenicity of the agent is intimately associated with plasmid-governed infectivity, which is attributed to the presence of plasmid-encoded virulence-associated proteins (Vap). Three host-adapted virulence plasmid types (VAPs) have been

Comparative genomic analyses reveal a lack of a substantial signature of host adaptation in Rhodococcus equi ('Prescottella equi').

Rhodococcus equi ('Prescottella equi') is a pathogenic actinomycete primarily infecting horses but has emerged as an opportunistic human pathogen. We have sequenced the genome of the type strain of this species, R. equi strain C7(T) , and compared the genome with that of another foal isolate 103S and of a human isolate ATCC 33707. The R. equi strains are closely related to each other and yet distantly related to other rhodococci and Nocardia brasiliensis. The comparison of gene contents among R. equi strains revealed minor differences that could be associated with host adaptation from foals to

Serum Electrolyte Changes in Foals Receiving Rhodococcus equi-Specific Hyperimmune Plasma.

BACKGROUND: Rhodococcus equi-specific hyperimmune plasma (Re-HIP) is routinely administered to neonatal foals to help prevent rhodococcal pneumonia. In addition to pathogen-specific immunoglobulins, frozen plasma products (FPPs) contain electrolytes that may exceed concentrations found in neonatal foal plasma. Because FPPs are also used to treat failure of passive transfer and sepsis, understanding their effects on neonate homeostasis is crucial. This study evaluated the effects of administration of Re-HIP on the serum concentrations of Na+, Cl-, K+, albumin, and globulin of healthy neonatal f
