Human herpesvirus 7 in dermatology: what role does it play?

Human herpesvirus 7 (HHV-7) was discovered in 1989 as a new member of the beta-herpesvirus subfamily. Primary infection occurs early in life and manifests as exanthema subitum, or other febrile illnesses mimicking measles and rubella. Thus, HHV-7 has to be considered as a causative agent in a variety of macular-papular rashes in children. In addition, HHV-7 was found in some cases of other inflammatory skin disorders, such as psoriasis. There are controversial data on the detection of HHV-7 in pityriasis rosea, but so far there is not enough evidence for a pathogenetic association of HHV-7 wit

Surveillance and prevention of infection in clinical xenotransplantation.

SUMMARYXenotransplantation, the transplantation of living organs, tissues, or cells between species, carries the potential to address the global shortage of human organs for patients with end-stage organ failure. Recent advances in genetic engineering have improved prospects for clinical xenotransplantation by reducing immune and inflammatory responses to grafts, controlling coagulation on endothelial surfaces, and modifying viral risks, including the porcine endogenous retrovirus (PERV). Management of infectious risks posed by clinical xenotransplantation requires meticulous attention to the

The diagnostic complexities of human herpesvirus 6 (HHV-6) infections.

Human herpesvirus 6 (HHV-6) is a common virus which is typically acquired early in childhood, and like other herpesviruses, can induce lifelong latency. While healthy individuals are not at risk of disease, immunosuppressed individuals such as those receiving hematopoetic stem cell transplantation are at substantial risk of morbidity and non-relapse mortality. Swift diagnosis and treatment remain crucial for the timely management of infections and reduce the severity of complications. A range of diagnostic approaches to detect HHV-6 have been established, the most common of which include PCR a

Human herpesvirus 7 induced aseptic meningitis in an immunocompetent adult presenting with unusual neurological symptoms: a case report.

BACKGROUND: Viral meningitis is a frequent cause of central nervous system (CNS) inflammation, typically presenting with headache, fever, photophobia, and neck stiffness. While enteroviruses and herpes simplex virus are the most common etiologies, Human Herpesvirus 7 (HHV-7), a rarely recognized cause, is increasingly implicated in aseptic meningitis, particularly in immunocompetent adults. HHV-7 is a T-lymphotropic virus that primarily affects CD4 + T lymphocytes, and it is globally prevalent, with over 95% of adults seropositive for the virus. Though traditionally associated with mild childh

Acute transverse myelitis following an opsoclonus-myoclonus syndrome: An unusual presentation.

Opso-myoclonus syndrome (OMS) is a very rare and severe condition. Ataxia, opsoclonus, myoclonus and/or behavioral and sleeping disturbances define that autoimmune disorder syndrome which is paraneoplastic or triggered by an infection. Here, we report a 3 year-old immunocompetent boy who developed an atypical OMS which was later complicated by an acute transverse myelitis. Screening for neuroblastoma was negative and extensive infectious screening revealed an active HHV-6 infection confirmed by blood and cerebrospinal fluid PCR. A parainfectious disease was suggested and immunosuppressive trea

Pathogenesis of Severe Neutropenia in Patients With Primary Human Herpesvirus 6B Infection.

BACKGROUND: Although myelosuppression caused by human herpesvirus 6B (HHV-6B) reactivation in transplant recipients has been extensively investigated, the pathophysiological mechanisms of severe neutropenia in primary HHV-6B infection remain unclear. PROCEDURE: Fifty-four patients with primary HHV-6B infection were evaluated. Hematological examinations and blood sampling were conducted on days 1-4 (pre) and 5-10 (post) after the onset of illness. Severe neutropenia was defined as a neutrophil count less than 500 cells/μL. Patient characteristics, clinical data, and cytokines and chemokines le

HHV-6 infection in patients with HIV-1 infection and disease.

Human herpesvirus 6 (HHV-6) is among the most widespread of the human herpesviruses. In immunocompetent children, it causes exanthem subitum, febrile episodes without skin rash, and non-Epstein-Barr and non-cytomegalovirus infectious mononucleosis. HHV-6 has also been associated with clinical disease in bone marrow and solid organ transplant recipients. Its potential role in HIV-1-associated clinical syndromes is now being recognized and evaluated. In this review, we describe the virus, the pathogenesis of HHV-6-associated disease, and the diagnostic tests used to differentiate active from lat

Detection of viral pathogens in high grade gliomas from unmapped next-generation sequencing data.

Viral pathogens have been implicated in the development of certain cancers including human papillomavirus (HPV) in squamous cell carcinoma and Epstein-Barr virus (EBV) in Burkitt's lymphoma. The significance of viral pathogens in brain tumors is controversial, and human cytomegalovirus (HCMV) has been associated with glioblastoma (GBM) in some but not all studies, making the role of HCMV unclear. In this study we sought to determine if viral pathogen sequences could be identified in an unbiased manner from previously discarded, unmapped, non-human, next-generation sequencing (NGS) reads obtain

[Human Herpesvirus-6B Encephalitis].

Human herpesvirus 6B (HHV-6B) is a β-herpesvirus that establishes latency following primary infection during early childhood. In immunocompromised individuals, particularly those who have undergone allogeneic hematopoietic stem cell transplantation, HHV-6B reactivation occurs in 30-80% of cases, with notably high rates observed after cord blood or HLA-mismatched transplants. HHV-6B encephalitis poses a serious and often life-threatening neurological complication for these transplant recipients. The mortality rate associated with this condition remains approximately 30-50%, and many survivors e

Human herpesvirus 6 and central nervous system disease in oncology patients: A retrospective case series and literature review.

BACKGROUND: Human herpesvirus 6 (HHV-6) can reactivate with immunosuppression and cause central nervous system (CNS) dysfunction. Much of the literature describes cases after hematopoietic stem cell transplantation (HSCT), ranging from encephalitis to a post-transplant acute limbic encephalitis syndrome (PALE). Outside of HSCT, studies of HHV-6 encephalitis are limited to case reports. OBJECTIVES: This study was designed to review HHV-6 CNS infection, and evaluate all patients admitted to MD Anderson Cancer Center between March 2016 and December 2018 with detectable HHV-6 DNA in the cerebrosp

Is human herpesvirus 7 the causative agent of pityriasis rosea?--A critical review.

BACKGROUND: Conflicting results on the association of pityriasis rosea and human herpesvirus 7 infection have been reported by different investigators. AIM: To review the level of evidence for such an association. METHODS: Medline was searched with unlimited data entry and 13 reports were retrieved. The data were analyzed for a causative association according to the criteria of Fredericks and Relman, which take into consideration latent infection and the reactivation of viruses characteristic of herpesviruses, and the roles of sequence-based detection methods. RESULTS: None of the criteria

Intracerebral opportunistic infections caused by immunosuppressants after orthotopic liver transplantation: Report of two cases and literature review.

Central nervous system (CNS) infections in adults are rare because of normal immunity and the existence of the blood brain barrier, which prevents the invasion of pathogenic microorganisms. Liver transplant recipients are at an increased risk of opportunistic infections (OI) due to immunosuppressive therapy compared to those with normal immunity. Early diagnosis and timely implementation of treatment are critical for the successful treatment of these infections. We present two cases of intracerebral OI after orthotopic liver transplantation (OLT), with different clinical presentations. Patient

Spontaneous remission of West syndrome following a human herpesvirus 7 infection in a Chinese infant: A case report.

RATIONALE: West syndrome (WS) is an age-dependent epileptic encephalopathy that is characterized by intractable epileptic seizures, hypsarrhythmia, and observed through electroencephalogram (EEG) and significant neurodevelopmental regression. The spontaneous remission of epileptic seizure is clinically rare and has not previously been reported in a Chinese infant. Herein, we reported a Chinese infant with WS whose seizures disappeared following a human herpesvirus 7 (HHV-7) infection. PATIENT CONCERNS: The male Chinese infant was born at the gestational age of 36 weeks with a birth weight of

Xenotransplantation and porcine cytomegalovirus.

Porcine microorganisms may be transmitted to the human recipient when xenotransplantation with pig cells, tissues, and organs will be performed. Most of such microorganisms can be eliminated from the donor pig by specified or designated pathogen-free production of the animals. As human cytomegalovirus causes severe transplant rejection in allotransplantation, considerable concern is warranted on the potential pathogenicity of porcine cytomegalovirus (PCMV) in the setting of xenotransplantation. On the other hand, despite having a similar name, PCMV is different from HCMV. The impact of PCMV in
