The Independent Effect of Nerve Injury and Tendon Injury on Rotator Cuff Fatty Infiltration.

BACKGROUND: Muscular fatty infiltration (FI), atrophy, and fibrosis commonly occur after chronic rotator cuff tear and are barriers to successful rotator cuff repair. It is unclear if these phenomena result from tendon injury alone, neurological injury, or a combination. PURPOSE: To determine the role of suprascapular nerve mediated FI with and without chronic rotator cuff tear in a rabbit model. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 22 rabbits underwent bilateral shoulder procedures. Extremities were randomized to surgical procedures: suprascapular neurotomy, supra

Clinical efficacy of different shoulder joint drug injections for rotator cuff injuries: A network meta-analysis.

BACKGROUND: Rotator cuff injury is a common clinical disorder of the musculoskeletal system. It is one of the most common causes of shoulder pain and shoulder joint dysfunction. Intra-articular injection therapy is one of the common conservative treatments for rotator cuff injury in clinical practice, but scholars have been controversial about the different drugs used for injection therapy and their clinical effectiveness, and there is no complete agreement on the choice of different injectables, and the existing original studies are mostly direct comparisons in 1 arm, lacking indirect compari

[Evaluation of a simple test for shoulder function in patients with surgically treated rotator cuff injuries].

INTRODUCTION: To assess overall shoulder function a simple examination of the range of motion and strength may not be sufficiently representative. because the shoulder in its daily use is often required for repetitive activities, fatigue at an early stage could be a sign of disturbed shoulder function. Therefore the shoulder function test (SFT) was created to incorporate the factor time into shoulder assessment. With this study the SFT was evaluated for its clinical use in the prospective follow-up of a series of patients operated on for rotator cuff tears. METHODS: Using forward flexion, the

[Early physiotherapy in lower trapezius transfer for massive and irreparable rotator cuff injuries. A case report].

Massive and irreparable lesions of the rotator cuff are frequent between people over 60 years old. Recently, inferior trapezium transference with Achilles tendon allograft has demonstrated very good results to restore shoulder's function. For this kind of surgery, it is recommended an absolute immobilization in shoulder abduction at 90° and maximal external rotation between 6-8 weeks. A 57-year-old woman underwent surgery with a lower trapezius transfer to repair the rotator cuff due to a massive and irreparable tear. She started aquatic physiotherapy 15 days after surgery and conventional phy

IMU-based rotator cuff injury recognition with varying configurations and combinations.

INTRODUCTION: Assessing functional movements is important for evaluating shoulder impairments, as these movements directly reflect patients' capacity to perform daily activities. Intelligent rotator cuff injury (RCI) recognition is beneficial for clinical management. METHODS: This study aimed to develop accurate and cost-effective RCI recognition models by integrating machine learning (ML)/deep learning (DL) algorithms with upper limb kinematic data collected in functional movement, and to explore optimal motion task combinations and model configurations for clinical practice. Defining Endogenous Mitochondrial Transfer in Muscle After Rotator Cuff Injury.

BACKGROUND: Rotator cuff muscle degeneration leads to poor clinical outcomes for patients with rotator cuff tears. Fibroadipogenic progenitors (FAPs) are resident muscle stem cells with the ability to differentiate into fibroblasts as well as white and beige adipose tissue. Induction of the beige adipose phenotype in FAPs has been shown to improve muscle quality after rotator cuff tears, but the mechanisms of how FAPs exert their beneficial effects have not been fully elucidated. PURPOSE: To study the horizontal transfer of mitochondria from FAPs to myogenic cells and examine the effects of β

Progress in studying the impact of hyperlipidemia and statins on rotator cuff injury and repair.

This review delves into the intersection of two prevalent conditions, hyperlipidemia and rotator cuff injuries, both of which bear substantial healthcare burdens. Our investigation begins with an exploration of rotator cuff injuries, common musculoskeletal disorders that severely impair shoulder functionality and quality of life. These injuries are notably pervasive among sports enthusiasts and the older adult, with an incidence rate estimated at 5-10% in the general population. Despite their widespread occurrence and the diverse, multifactorial etiological factors, effective treatment strateg

Defining the relationship between rotator cuff injury and proximal humerus fractures.

The purpose of our study was to correlate radiographic characteristics with rotator cuff tendon injury on magnetic resonance imaging after fractures of the proximal humerus. We prospectively obtained magnetic resonance imaging on 30 patients with proximal humerus fractures after classifying each fracture radiographically using Neer and AO systems and determining the displacement of the greater tuberosity. Twelve patients (40.0%) had either complete ruptures or avulsions of at least one of the rotator cuff muscles. No abnormality was identified in the rotator cuff musculature in nine patients (

MR imaging of rotator cuff injury: what the clinician needs to know.

The rotator cuff muscles generate torque forces to move the humerus while acting in concord to produce balanced compressive forces to stabilize the glenohumeral joint. Thus, rotator cuff tears are often associated with loss of shoulder strength and stability, which are crucial for optimal shoulder function. The dimensions and extent of rotator cuff tears, the condition of the involved tendon, tear morphologic features, involvement of the subscapularis and infraspinatus tendons or of contiguous structures (eg, rotator interval, long head of the biceps brachii tendon, specific cuff tendons), and

Temporal Gene Expression Changes in Rotator Cuff Tendon Injury and Repair.

Rotator cuff tendon injuries are common, particularly among the elderly. However, promising biomarkers to assess tendon degeneration and healing remain limited. This study aimed to identify dynamic biomarkers by analyzing temporal gene expression and histological changes in a chronic rotator cuff tear model. Using Sprague-Dawley rats, a chronic injury was induced by detaching the supraspinatus tendon and inserting a silastic drain to inhibit spontaneous healing. Tendon samples were collected at multiple time points after injury and after repair for gene expression profiling and histological an

[Transposition of scapular muscles in the treatment of injuries of the rotator cuff of the shoulder (RCS)].

An experience of 13 cases of short rotator muscles advancement is reported. These procedures were found to be available in treatment of rotator cuff avulsions with retraction, when anatomical reinsertion or shoe-lace suture after Mclaughlin appeared to be impossible. The authors had performed advancement of the supraspinatus alone in 10 cases and simultaneous advancement of supra-, infraspinatus and teres minor in 3 cases. Only one poor result was obtained that can't but confirm a most high efficacy of the procedures described.

Delivery of GAS5 by LNP promotes tendon-bone healing in rotator cuff injury.

It is observed in clinical practice that worse healing in rotator cuff tear (RCT) is closely associated with osteoporosis in patients. Herein, from the statistical analysis, we further confirmed that the RCT is significantly increased in postmenopausal women who suffer from decreased bone mineral density (BMD), suggesting intervention of osteoporosis as a potential strategy in the management of RCT in aged women. Our previous studies showed that Growth arrest-specific 5 (GAS5), a long non-coding RNA (lncRNA), could promote bone formation, and alleviate osteoporosis. Herein, we demonstrated an

Factors associated with pseudoparalysis in patients with extensive chronic and atraumatic rotator cuff injury.

BACKGROUND: Patients with massive chronic atraumatic rotator cuff injuries can be asymptomatic or present severe shoulder dysfunction, a condition known as pseudoparalysis. Our hypothesis is that integrity of the subscapularis, hypertrophy of the teres minor, and a complete tear of the long head of the biceps tendon are associated with improved active forward flexion range in such patients. Therefore, the objective of this study was to evaluate factors associated with pseudoparalysis. METHODS: This was a single-center cross-sectional study that included patients with massive chronic atraumati

Mesenchymal stem cell-derived exosomes: therapeutic implications for rotator cuff injury.

Rotator cuff injuries are a common clinical condition of the shoulder joint. Surgery that involves reattaching the torn tendon to its humeral head bony attachment has a somewhat lower success rate. The scar tissue formed during healing of the rotator cuff leads to poor tendon-related mechanical properties. To promote healing, a range of genetic interventions, as well as cell transplantation, and many other techniques have been explored. In recent years, the therapeutic promise of mesenchymal stem cells (MSCs) has been well documented in animal and clinical studies. Some data have suggested tha
