Reconfiguring Rehabilitation Services for Rural South Africans with Disabilities During a Health Emergency: A Qualitative Descriptive Study.

The COVID-19 pandemic and the subsequent hard lockdown in South Africa, implemented in March 2020, significantly disrupted disability and rehabilitation services. Persons with disabilities experienced limited access to essential Orthotic and Prosthetic services, particularly in rural provinces such as the Eastern Cape. This study aimed to explore how Medical Orthotists and Prosthetists reconfigured their services during and after the pandemic to inform disability-inclusive emergency responses. A descriptive qualitative study was conducted with 12 Medical Orthotists and Prosthetists practicing

A modified Continuous Quality Improvement approach to improve culturally and socially inclusive care within rural health services.

BACKGROUND: The sickest Australians are often those belonging to non-privileged groups, including Indigenous Australians, gay, lesbian, bisexual, transsexual, intersex and queer people, people from culturally and linguistically diverse backgrounds, socioeconomically disadvantaged groups, and people with disabilities and low English literacy. These consumers are not always engaged by, or included within, mainstream health services, particularly in rural Australia where health services are limited in number and tend to be generalist in nature. OBJECTIVE: The aim of this study was to present a n

The development and implementation of a rural consortium program to provide early, preventive school mental health services.

This article describes the development and implementation of a rural consortium of school-based programs for early detection and prevention of maladjustment. The program expanded considerably the reach of early services to young children. It also stimulated communication, interaction and support among professionals in participating districts. Program evaluation data provided evidence of its efficacy. Extensions of the consortium model to other rural districts are reported and the utility of the approach for addressing the mental health problems of children in under-resourced rural school distr

Integrated electronic health record facilitates a safer and more efficient rural outreach haematology service.

BACKGROUND: Rural Australian oncology patients are known to have inferior mortality rates compared to metropolitan patients, possibly related to access to appropriate healthcare services and treatments. Electronic systems improve the safety of chemotherapy administration and allow easily accessible patient information and data collection. AIMS: To integrate the electronic healthcare delivery systems at a metropolitan hospital and a rural outreach haematology clinic to facilitate streamlined and safe outpatient care. METHODS: The MOSAIQ v2.64(Elekta) system utilised at St Vincent's Hospital w

Rural-urban differences in suicide rates for current patients of a public mental health service in Australia.

Rural versus urban rates of suicide in current patients of a large area mental health service in Australia were compared. Suicide deaths were identified from compulsory root cause analyses of deaths, 2003-2007. Age-standardized rates of suicide were calculated for rural versus urban mental health service and compared using variance of age-standardized rates with 95% confidence intervals. There were 44 suicides and the majority (62%) were rural. Only urban patients used jumping from heights as a method of suicide (4/17; p = 0.02). Rural patients had 2.7 times higher rates of suicide, similar to

A method for identifying places in rural America at risk of not being able to support adequate health services.

CONTEXT: Public policymakers and their advisers struggle with the problem of specifying criteria by which health care providers in rural areas are eligible for special consideration in payment policies and for special grant programs. A means of designating places can provide a basis for assistance and can help target public resources for any providers who deliver services in those places. PURPOSE: This paper provides the details underlying a place-based approach to identifying rural areas that are at risk for not being able to provide requisite health services. METHODS: A population size cri

The association between health insurance status and utilization of health services in rural Northern Ghana: evidence from the introduction of the National Health Insurance Scheme.

BACKGROUND: Many households in low- and middle-income countries face financial hardships due to payments for health care, while others are pushed into poverty. Risk pooling and prepayment mechanisms help to lessen the impact of the costs of care as well as assisting to achieve universal health coverage (UHC). Ghana implemented the National Health Insurance Scheme (NHIS) for the promotion of access to health services for all Ghanaians. In this paper, we examined the association between health insurance status and utilization of outpatient and inpatient health services in rural poor communities.

Community-engaged approaches are essential for developing contextually appropriate solutions. METHODS: We used Group Concept Mapping, a participatory research method, to identify strategies for improving mental health service access for rural youth in three counties in northeastern Washington State. Adult community members participated in brainstorming, sorting, and rating potential solutions. Statements were rated on perceived impact an

Developing and implementing a large-scale summit to improve health services delivery for rural, remote and Indigenous populations.

Inequities persist in health services delivery for rural, remote and Indigenous populations internationally. These populations require attention to improve health outcomes for individuals, communities, providers and the health system. Strong collaboration and trusting relationships in rural and Indigenous communities in British Columbia (BC), Canada enabled rapid health services delivery changes in rural BC in response to the COVID-19 pandemic. In June 2020, the Rural Coordination Centre of BC and First Nations Health Authority co-hosted the BC Rural and First Nations Health and Wellness Summi

Explaining changes in educational disparities in competent maternal health care services in urban and rural areas in Ethiopia.

INTRODUCTION: Aggregate statistics of maternal health care services have improved in Ethiopia. Nevertheless, the country has one of the lowest Universal Health Coverage (UHC) service coverage indices, with slight improvement between 2000 and 2019. There are disparity studies that focus on a single dimension of inequality. However, studies that combine multiple dimensions of inequality simultaneously may have important policy implications for closing inequalities. In this study, we investigated education inequalities in the receipt of maternal health care services in rural and urban areas separ

Increasing access to safe abortion services in rural India: experiences with medical abortion in a primary health center.

INTRODUCTION: To increase access to safe abortion in rural India, the feasibility and acceptability of mifepristone-misoprostol abortion was assessed in a typical government run primary health center (PHC) in Nagpur district, Maharashtra State, that does not offer surgical abortion services and must refer off-site for emergency and backup services. MATERIALS AND METHODS: Consenting pregnant women (n=149) with <or=56 days amenorrhea seeking terminations received 200 mg mifepristone, and returned 48 h later for 400-microg sublingual misoprostol and 12 days later for abortion confirmation. Experiencing Housing Instability in Rural North America and Access to Health Care and Supportive Services: A Scoping Review.

OBJECTIVE: We conducted a scoping review of 45 peer-reviewed articles to characterize the intersection of housing instability and homelessness, care, and services in rural North America. We sought gaps in literature related to geographically based health disparities among people experiencing housing instability and identify practices that may promote better health outcomes. We offer practical implications for health care and service provision for people experiencing housing instability in rural areas. Cross-sectional assessment of government health center needs to implement long-acting reversible contraception services in rural Rwanda.

BACKGROUND: There is unmet need for family planning in Rwanda. We previously developed an evidence-based couples' family planning counseling (C)FPC program in the capital city that combines: (1) fertility goal-based family planning counseling with a focus on long-acting reversible contraceptive (LARC) for couples wishing to delay pregnancy; (2) health center capacity building for provision of LARC methods, and (3) LARC promotion by community health workers (CHW) trained in community-based provision of oral and injectable contraception. From 2015 to 2016, this service was integrated into eight
