The authors describe a semiconservative approach for bilateral fracture-dislocation of the sacrum, an extremely rare injury. Traction is advocated; it seems to lead to a good functional result after 20 years. Other authors either propose internal fixation or benign neglect, also with a good outcome. Every case must be approached on an individual base.

A series of five cases of sacral aneurysmal bone cyst is presented. There were three females and two males, the youngest patient being 11 years old, the oldest 41 years old. All patients had pain of long duration with varying signs of nerve root compression. Three patients had micturitional problems. Four patients had a palpable mass which was tender. All had standard radiographs. Tomograms, computed tomography (CT) scans or MRI (magnetic resonance imaging) were obtained in some patients. In all patients a definite histologic diagnosis was established. Four lesions healed after resection (two

PURPOSE: Sacral tumours, both benign and malignant, often necessitate surgical removal (sacrectomy) to achieve optimal outcomes. However, this procedure disrupts the pelvic ring's stability, potentially leading to pain and limited mobility.
METHODS: This article explores innovative approaches to reconstruct the sacrum and restore function in primary and secondary sacral tumours.
RESULTS: Beyond traditional bone graft-based spino-pelvic fixation, the paper delves into minimally invasive alternatives like robotic-assisted surgery which may be used especially as a palliative procedure in destru

INTRODUCTION AND IMPORTANCE: Bone hydatidosis is a rare parasitic infection caused by Echinococcus granulosus, with a frequency of 1-2 %, it has a high morbidity and mortality rate due to its long clinical latency and lack of symptomatic specificity. This often leads to delayed diagnosis and discovery, usually at the stage of complications.
CASE PRESENTATION: We present a case of sacral hydatidosis in a 71-year-old patient with no prior medical history. The symptomatology was a pelvic pain that had been ongoing for 5 months with constipation and difficulty with defecation. The clinical and ra

Fragility fractures of the sacrum (FFS) are caused by low-energy trauma in the elderly population. Due to the nuanced symptomatology, many FFS remains unrecognized and the prevalence is underestimated. The clinical presentation varies, typically presenting with weightbearing low back pain without even remembering of a previous trauma. Radiographs are usually insufficient for the diagnosis and second level imaging modalities are required. In particular, magnetic resonance demonstrated the highest diagnostic accuracy. Treatment should aim to guarantee early mobilization and weightbearing, effici

BACKGROUND: An osteoblastoma is a rare benign bone tumor characterized by formation of osteoid tissue and primitive bone and occurs more often in men than in women. They are often secondary to an osteoid osteoma and can be located at any site on the skeleton. Lesions generally involve the posterior elements of the spine, such as the pedicle and the lamina.
CASE PRESENTATION: This study reports the case of a 25-year-old female who suffered from an osteoblastoma of the right sacrum with repeated swelling and pain in the right lumbosacral region for approximately 6 months. Computed tomography (C

INTRODUCTION: Several important steps occur in order for the sacrum to develop properly. Embryological derailment can result in several different anatomical variants.
CASE REPORT: Herein, we report, to our knowledge, the first case of a duplicated sacrum in the sagittal plane.
CONCLUSIONS: Clinicians and radiologists should be aware of such a finding if found during imaging. We would hypothesize that such a variant occurred near the 29th day of development and probably arose from an error in the HOX gene. To our knowledge, this represents the first reported case.

Myxopapillary ependymomas rarely present as a primary intrasacral lesion, and extensive sacral osteolysis is unusual. The authors report a case series of 6 patients with these complex tumors causing extensive sacral destruction, who underwent resection, lumbopelvic reconstruction, and fusion. The operative procedure, complications, and outcome are summarized after a mean follow-up of 3.55 years (range 18-80 months).

Although several morphological variations of sacrum have been reported in western populations, little attention has been paid to this anatomic issue in eastern people, and classification of sacral variability in particular. In this research of sacral morphology in Chinese people, we investigated and measured thoroughly and systematically 203 specimens of intact dry Chinese adult sacra. Morphological features of sacral variations were observed by visual inspection, and correlation parameters of variability were measured with a vernier caliper. The incidence of sacral variations was calculated.

BACKGROUND: Leptomeningeal carcinomatosis occurs in patients with cancer at the rate of approximately 5%; it develops particularly in patients with breast cancer, lung cancer, melanoma, leukemia, or malignant lymphoma. We describe a rare case of leptomeningeal carcinomatosis in which spinal intradural squamous cell carcinoma with no lesions in the cerebral meninges and leptomeninx, was the primary lesion.
METHODS: A 64-year-old man complained of sacral pain. Although the patient was treated with analgesics, epidural block and nerve root block, sacral pain persisted. Since acute urinary retent

STUDY DESIGN: This report describes a 51-year-old woman with a sacral metastatic meningioma that originated from an intracranial meningioma.
OBJECTIVES: To describe an unusual presentation of a metastatic meningioma in the sacrum.
SUMMARY OF BACKGROUND DATA: Extracranial metastases of meningioma are very rare. The phenomenon of metastasis may have more to do with the ability to invade the wall of a blood vessel than with the mitotic activity of a tumor. Therefore, metastases of the meningioma can occur even with a benign histologic picture in the original intracranial meningioma.
METHODS: A

Twenty-two patients with primary tumors of the sacrum were surgically treated between 1983 and 1997. Seventeen male and 5 female patients were followed up for a mean of 53.6 months (range 12-203 months). The histopathologic diagnoses were giant cell tumor (GCT) in 7 patients, chordoma in 4 patients, aneurysmal bone tumor in 3 patients, chondrosarcoma in 2 patients, osteoblastoma in 2 patients, synovial sarcoma in 2 patients, Ewing's sarcoma in 1 patient, and simple bone cyst in 1 patient. Currettage and thermo- or chemocauterization was applied to 8 patients, a subtotal sacrectomy was done in

A case of monostotic fibrous dysplasia involving both alae of the sacrum is reported. Only 2 cases of monostotic sacral involvement were published previously. The lesion was detected in a 42-year-old man suffering from lumbosacral pain after minimal trauma. Radiographic studies revealed cystic images on both alae of the sacrum, with internal condensations and some liquid contents. A trephine biopsy did not produce enough diagnostic data, and a posterior surgical approach was elected. Curettage and refilling with allograft were performed. The pathologic anatomy study diagnosed a fibrous dysplas

STUDY DESIGN: A radiographic study of the sagittal sacral deformity in spondylolisthesis.
OBJECTIVES: To characterize and classify the pathoanatomy of sagittal sacral deformation in spondylolisthesis.
SUMMARY OF BACKGROUND DATA: Spondylolisthesis has been extensively described and reviewed in the literature. Deformity of the entire sacrum in spondylolisthesis potentially could affect the natural history, treatment options, and outcome. The sagittal contour of the entire human sacrum has never been quantitatively studied in spondylolisthesis.
METHODS: A literature search was performed and da

Displaced fractures of the sacrum are often associated with mechanical instability and neurologic deficits that cause late musculoskeletal problems and pain. We describe the radiologic and neurologic characteristics of 12 displaced fractures of the sacrum, (in 5 men and 7 women; age at injury, 17-61 years) as well as the management and results. Seven patients had unilateral longitudinal fractures (ULF), and 5 had bilateral longitudinal fractures (BLF) with a transverse fracture. All patients had mechanical instability (rotational and/or vertical). Seven (58%) had neurologic deficits. ULFs were

BACKGROUND: Insufficiency fracture of the sacrum has been recognized as a major cause of lower back pain, especially in elderly osteoporotic patients with or without underlying malignancy and/or prior pelvic radiotherapy. Its diagnosis cannot be readily made from its nonspecific clinical features. Consequently some cases may be mistaken for bone metastases, causing patients to receive inappropriate diagnostic measures and management. Recognition of the spectrum of imaging features is the basis of correct diagnosis and treatment.
METHODS: The imaging studies of 14 cases (12 women and 2 men; ag

Stress fractures of the lower extremity and sacrum occur in a variety of patients, ranging from young, healthy athletes to elderly persons with underlying illnesses. Knowledge of the activities and risk factors associated with these fractures may heighten clinical suspicion and help direct an appropriate evaluation. Diagnosis is usually based on characteristic features of the history and physical examination accompanied by radiologic findings. Bone scintigraphy remains the standard, but the specificity of computed tomography scanning or magnetic resonance imaging may be required to differentia
