A Promising Opportunity to Address Food Insecurity and Promote Equity: The Crucial Role of Federally Qualified Health Centers.

OBJECTIVE: To explore how Federally Qualified Health Centers (FQHCs) are addressing food insecurity by understanding their existing and needed services, policies, and community partnerships using a social determinants of health perspective to promote health equity. DESIGN: A mixed-methods, exploratory case study research design consisting of two phases was conducted in 2023. Phase I involved a survey of FQHC leaders in Ohio. From these data, eight FQHC cases were identified for Phase II qualitative interviews of staff and patients. SETTING: Ohio Federally Qualified Health Centers. PARTICIPA

Determinants of Implementation and Sustainability of Transitional Opioid Programs Within Rural and Urban Safety Net Hospitals.

BACKGROUND: Hospitalization presents a critical juncture to engage patients with opioid use disorder, but safety net hospitals are less likely to offer transitional opioid programs (TOPs), which initiate treatment and connect patients to care after discharge. This study explored determinants of TOP adoption in 4 diverse safety net health systems. METHODS: We interviewed key participants affiliated with safety net health systems and partner community-based organizations about TOPs within their organization. We used inductive coding informed by grounded theory to identify emerging themes relate

Implementation of a lung cancer screening program at an urban safety-net hospital: enabling solutions via stakeholder engagement.

Lung cancer screening (LCS) has been shown to reduce lung cancer mortality; however, numerous obstacles impede the successful development and expansion of LCS programs, particularly in underserved communities where uptake is limited. Neighborhood disadvantage plays a significant role, as numerous studies have elucidated the influence of race, ethnicity, income, and other measures of social disadvantage on LCS, as well as lung nodule surveillance programs. Successful development and expansion of LCS and nodule programs in inner-city hospitals can be an important means of improving health equity

Systemic Challenges to Lung Cancer Screening in the Safety-Net Setting: Care and Informatics Fragmentation.

BACKGROUND: Despite being the leading cause of cancer death, <20% of eligible, at-risk Americans undergo lung cancer screening. Lung cancer risk is concentrated in lower-income populations, but Medicaid-insured populations consistently have the lowest screening rates across cancers. To improve lung cancer screening rates and outcomes, we must understand the systemic challenges of orchestrating a complete episode of multidisciplinary care, from radiographic screening to treatment, in the resource-limited, safety-net environment. METHODS: We performed a qualitative case study of purposively sam

Transitioning From Race-Specific to Race-Neutral Reference Equations for Pulmonary Function Test Interpretation at a Large Safety Net Hospital System.

In April 2023, the American Thoracic Society (ATS) published an official ATS statement entitled "Race and Ethnicity in Pulmonary Function Test Interpretation" recommending the adoption of race-neutral reference equations for pulmonary function test results interpretation. However, lack of a clear roadmap to implement this recommendation effectively remains a challenge. This article outlines how our large safety net hospital systematically transitioned from race-specific to race-neutral reference equations. Our approach, guided by the Kotter change model, can serve as a framework for other inst

Video versus audio telehealth in safety net clinic patients: Changes by rurality and time.

BACKGROUND: Understanding the mix of video versus audio telehealth modality is critical to informing care for low-income safety net clinic patients. Our study examined whether telehealth modality and continued use of telehealth varied by rurality and whether that changed over time. METHODS: Encounters from adults in the OCHIN national network of primary care safety net clinics were identified by encounter type (in-person vs telehealth) and telehealth modality (video vs audio) from 4/1/2021 to 3/31/2023. Our main outcome was an interaction between patient rurality (defined using Rural Urban Co

Historical and clinical factors associated with positive urine toxicology screening on labor and delivery.

OBJECTIVE: Illicit drug use in pregnancy may lead to adverse outcomes. Although the American College of Obstetricians and Gynecologists recommends that all pregnant women be screened for substance use by questionnaire or conversation, it remains unclear how well these methods identify women with illicit drug use. Drug use may also be suspected based on clinical complications, such as fetal demise or placental abruption. There are currently no formal recommendations to guide targeted laboratory testing in women perceived to be at risk based on historical or clinical factors. Our objective was t

Assessing The Inclusion Of Federally Qualified Health Centers In Medicare Advantage Networks.

Medicare Advantage (MA) disproportionately enrolls low-income and racial and ethnic minority populations that may benefit from access to federally qualified health centers (FQHCs). Using 2022 FQHC provider and MA network data, we examined the inclusion of FQHCs in MA provider networks. On average, these networks included 57 percent of FQHCs in their service areas, but 30 percent of contracts had narrow FQHC networks, defined as including fewer than one-quarter of available FQHCs. Asian/Pacific Islander, Hispanic, and Black beneficiaries were more likely to be enrolled in a narrow-network contr

Developing user-friendly ambulatory referrals: a quality improvement study in GI referral services at a large academic safety net hospital system.

BACKGROUND: In the United States, more than a third of patients are referred to specialists each year; however, most of these referrals do not lead to completed appointments. At the Grady Health System (GHS), our large safety net hospital system, the initial gastroenterology (GI) referral process suffered from multiple inefficiencies, creating barriers to care. We aimed to improve GI referrals with both a triage and a direct-to-endoscopy program to relieve systemic barriers to GI care at GHS especially around colorectal cancer screening. METHODS: Given wait times for GI services and employee d

Social Determinants of Health Associated With the Need for Urgent Versus Elective Cholecystectomy at an Urban, Safety-Net Hospital.

Background: Benign gallstone disease is the most frequent indication for cholecystectomy in the United States. Many patients present with complicated disease requiring urgent interventions, which increases morbidity and mortality. We investigated the association between individual and population-level social determinants of health (SDoH) with urgent versus elective cholecystectomy. Patients and Methods: All patients undergoing cholecystectomy (2014-2021) for benign gallstone disease were included. Demographic and clinical data were linked to population-level SDoH characteristics using census t

340B Participation and Safety Net Engagement Among Federally Qualified Health Centers.

IMPORTANCE: The 340B program provides discounts on outpatient drugs to certain hospitals and federally supported clinics (covered entities) that can be used to generate revenue to fund safety net care. While numerous studies have found no association between 340B and safety net care provision for most hospital covered entities, less is known about whether federally qualified health centers (FQHCs), the largest group of covered entities after hospitals, use the program to enhance safety net care. OBJECTIVE: To assess whether a proxy for 340B revenue was associated with increased safety net car

The Burden of Patients With Lower Limb Amputations in a Community Safety-net Hospital.

BACKGROUND: The functional disability after amputation is tremendous and imposes a high economic burden on patients and health systems. The current literature on the costs of amputation has been limited to the index hospitalization or a short time window around the amputation procedure, which covers a small percentage of the total costs. METHODS: We conducted a retrospective cohort study of patients who underwent lower extremity amputations at a single urban public level 1 trauma hospital. Resource utilization and healthcare costs 1 year before and 1 year after the index amputation were exami

Estimating the Size and Cost of the STD Prevention Services Safety Net.

The Patient Protection and Affordable Care Act is expected to reduce the number of uninsured people in the United States during the next eight years, but more than 10% are expected to remain uninsured. Uninsured people are one of the main populations using publicly funded safety net sexually transmitted disease (STD) prevention services. Estimating the proportion of the uninsured population expected to need STD services could help identify the potential demand for safety net STD services and improve program planning. In 2013, an estimated 8.27 million people met the criteria for being in need

Factors related to implementation of outpatient palliative care for advanced cancer patients in a safety-net health system: a qualitative study using the theoretical domains framework.

BACKGROUND: Clinical practice guidelines strongly recommend oncology clinicians refer all advanced cancer patients for early intervention by a specialized palliative care (PC) team during treatment. Studies indicate that cancer patients receiving care in safety-net systems experience low rates of outpatient PC referral and delays in accessing care. Yet, few studies have examined factors influencing early PC implementation in settings predominantly serving underserved populations. This study qualitatively evaluated the multilevel (patient-, provider-, clinic-level) factors that influence early

Getting to best practice: a practice-oriented checklist for monitoring clinic-level fidelity to hypertension interventions in a large safety-net healthcare system.

BACKGROUND: Practical tools are needed to monitor and support progress toward high-fidelity implementation of evidence-based practices designed to change patients' health behaviors and improve clinical outcomes. This article describes the development and baseline use of a clinic-level fidelity checklist for a multilevel intervention bundle of nine hypertension evidence-based practices in primary care. METHODS: Collaborating with partners from a large U.S. safety-net healthcare system, we developed a checklist with categorical clinic-level fidelity ratings ("little or none," "partial," "comple

A contactless cure: Leveraging telehealth to improve hepatitis C treatment at a safety-net hospital.

Hepatitis C virus (HCV) causes significant mortality worldwide. HCV is highly curable but access to care is limited for many patients. The Grady Liver Clinic (GLC), a primary care-based HCV clinic, utilizes a multidisciplinary team to provide comprehensive care for a medically underserved patient population in Atlanta, Georgia. The GLC added a telehealth option for HCV treatment at the start of the COVID-19 pandemic. We describe the outcomes of utilizing telehealth in this population. We performed a retrospective chart review of patients who initiated HCV treatment from March 2019 to February
