PURPOSE: To describe the technique and examine the value of salivary stone extraction using a minimally invasive, radiologically guided approach as an alternative to salivary gland surgery for the treatment of benign salivary gland obstruction.
METHODS: Eighty-six cases of sialolithiasis (83 patients) were treated by stone removal using a Dormia basket under local anesthesia and fluoroscopic guidance. Postoperative assessment was made clinically at review, by sialogram and by questionnaire.
RESULTS: Of 86 cases of sialolithiasis treated, in 55 (64%) it was possible to remove all stones. In 1

Sialolithiasis represents the most common disorders of salivary glands in middle-aged patients. It has been hypothesized that the retrograde migration of bacteria from the oral cavity to gland ducts may facilitate the formation of stones. Thus, in the present study, a microbiome characterization of salivary calculi was performed to evaluate the abundance and the potential correlations between microorganisms constituting the salivary calculi microbiota. Our data supported the presence of a core microbiota of sialoliths constituted principally by Streptococcus spp., Fusobacterium spp. and Eikene

Sialolithiasis is one of the common diseases of the salivary glands. It was speculated that, in the process of calculi formation, degenerative substances are emitted by saliva and calcification then occurs around these substances, and finally calculi are formed. However, the exact mechanism of the formation of calculi is still unclear. In this study, we identify some possible etiologies of calculi formation in salivary glands through biophysical analysis. Calculi from 13 patients with submandibular sialolithiasis were investigated by transmission electron microscopy, scanning electron microsco

BACKGROUND: Extracorporeal lithotripsy is now used routinely for the treatment of salivary duct stones. The question arose whether electrohydraulic intracorporeal lithotripsy, which is applied in urology and gastroenterology, might also be useful in the treatment of this disease. Before its possible clinical application the influence of electrohydraulic intracorporeal shock waves on salivary stones in vitro and any influence on the tissue in the head and neck region (in vivo) had to be investigated.
MATERIALS AND METHODS: In vitro experiments. Fifty-eight salivary stones and 11 extirpated hum

Before clinical application of an extracorporeal piezoelectric lithotripter to treat sialolithiasis, tissue reaction during shockwave application was examined in vitro and in experiments with animals. Application of shockwaves to human tissue in vitro showed neither macroscopic nor microscopic effects. In animal experiments, the acute experiment (16 rabbits, Chinchilla-Bastard) revealed minor bleeding in the parenchyma of the parotid gland, while the chronic experiment (14 rabbits, Chinichilla-Bastard) revealed no morphologic tissue damage to the parotid region of the rabbit, as a result of pi

BACKGROUND: The effort to use minimally invasive strategies in the treatment of human sialolithiasis in principle seems to allow the clinical application of pneumatic lithotripsy because of the small diameters of the probes involved. While this kind of therapy is already used successfully in urology, we investigated the method during in vitro and animal experiments.
MATERIALS AND METHODS: The treatment of stones and tissue utilized the pneumatic lithotripter "Lithoclast". This system works with ballistic energy and is similar to a biologic "pneumatic hammer". Thirty salivary calculi were trea

A model was developed in which high-resolution real-time sonography was compared with radiological examinations for determining their sensitivity in diagnosing salivary gland stones. For this purpose an artificial salivary gland medium was compared with human salivary gland tissue for echogenicity. It was shown that both the artificial and the true salivary gland medium possessed comparable acoustic properties. Forty salivary gland stones were examined ultrasonographically in both media with 5 and 7.5 MHz linear scanners and a 5 MHz sector scanner, following which a plain X-ray was done, while

Large calculi of the salivary glands are rare. They may go undetected for many years. The authors report three cases of giant submandibular gland calculi. In one patient, the calculus was an incidental finding. In all patients, the mass was in the substance of the gland; in one, the configuration of the mass was unusual and it had eroded through the floor of the buccal cavity forming an orocervical fistula (the first such report) and in another the mass occupied a small portion of Wharton's duct. The histopathologic findings in all three cases were of nonspecific chronic inflammation. All the

This study correlated radiographic observations and histologic findings of submandibular glands with the diagnosis of salivary calculus and/or chronic sialoadenitis. During a 15-year period eighty-eight patients satisfied clinical requirements by having a radiographic examination performed prior to gland extirpation. Salivary calculi were present in 83% of the patients. The diagnostic accuracy of radiographs regarding salivary calculi in the submandibular gland system was 92%. Radiolucent calculi constituted 20% of the total number of cases with concrements. Failure to disclose concrements was

The clinical and histopathological characteristics of 49 previously unpublished cases of sialolithiasis of minor salivary glands are presented. The lesions are usually solitary, small, submucosal nodules, which are hard or firm and freely movable in the surrounding tissue and on rare occasions may be multiple. They are most often seen in the buccal mucosa and the upper lip of patients in the 5th, 6th or 7th decade, but are rarely found in the lower lip and palate. The authors found that clinicians usually failed to include calculi in the differential diagnosis and further, they concluded that

Adenoid cystic carcinomas (ACC) of the head and neck are relatively rare tumors, consisting of approximately 10-15% of all salivary gland neoplasms. ACC, a slow-growing aggressive malignant tumor of salivary gland commonly seen in the submandibular, sublingual, minor salivary glands is seldom found in the parotid. Calculus, the common cause of salivary gland dysfunction is usually identified in submandibular salivary gland because of its duct anatomy and physiochemical characteristic serous secretion. We report an unusual case of co-existent presentation of ACC with salivary calculi in the par

An in vitro study was performed to examine the application of a laser beam for the fragmentation of salivary stones. In accordance with the absorption and reflection spectra of sections of salivary stones, optimal fragmentation was achieved using a pulsed excimer laser with a pulse width of 60 ns and a wave-length of 308 or 351 nm. Further studies on particle size were conducted and a new "sialoendoscope" technique was developed. Twelve patients suffering from a sialolithiasis of the submandibular gland were treated. Under continuous endoscopic monitoring a laser-induced shock wave lithotripsy

In this study the authors, making use of a non-surgical therapeutic method like extracorporeal shock wave lithotripsy, aim to show the advantages of this method compared with surgery, in the therapy of lithiasis of the salivary glands. Seven patients suffering from calculi of the salivary glands were treated. The authors obtained the best result in each patient and in 5 of these obtained the complete disappearance of sialolithiasis. The authors, considering that there is an ever higher number of requests for this treatment, put extracorporeal lithotripsy in the foreground in the therapy of sia

We report our results comprising a series of 34 cases of sialolithiasis that were approached sialendoscopically with conventional instruments (ie, basket and/or forceps) and for which stone removal was unsuccessful and so were thereafter treated with pneumatic lithotripsy. Intraductal pneumatic lithotripsy was able to fragment 30 of 34 (88%) salivary calculi in total. No major complications occurred. All 30 treated glands returned to normal function without any symptoms. We believe that the success of sialendoscopy is closely related to the success of salivary stone fragmentability.

We studied 43 patients (25 women and 18 men) who had salivary calculi removed from the hilum of the submandibular gland. Preoperatively they had clinical and radiographic examinations, and glandular function was measured scintigraphically in 38 patients. Postoperative follow-up was based on history, clinical examination, structured questionnaire, and scintigraphy. Stone(s) were removed successfully in 42 patients (97%). During the follow-up of a mean of 24 months (range 4-47), 37 patients were symptom-free and 2 patients had mild obstructive symptoms that did not require intervention. The othe

Aggregated crystalloids and calculi, up to 600 to 700 micron in diameter, found in the parotid gland tissue taken from a patient with chronic recurrent parotitis were examined histopathologically and microanalytically. In part of the periphery of calculi which had lamellated structure, layers of crystalloids in continuation with calculi were observed; and diffuse deposition of von Kossa-positive fine granules was seen in the innermost area of the crystalloid-layers. By electron probe x-ray microanalysis, phosphorus, sulphur and calcium were principally detected within the innermost area of the

In a prospective study, results of the extracorporeal shock wave lithotripsy of salivary stones were evaluated. The study was performed with the currently available technical possibilities of electromagnetic systems to get indication criteria for this procedure. Using the Modulith SL 20-lithotripter of Storz Medical AG, 33 concrements in 25 patients were treated in 89 lithotripsy sections. The stone sizes, localized by ultrasound, ranged from 3 to 13 mm diameter (phi 6.9 mm). After application of local anaesthesia, per session on an average 1300 shock waves with raising energy levels were appl

Sialolithiasis is the formation of calculi or sialoliths in the salivary gland. It is the most common benign condition of the salivary gland. Sialolithiasis can occur in all salivary glands. The submandibular gland is most commonly affected followed by the parotid gland. Calculi commonly measure less than 10 mm. Calculi of more than 15 mm are termed giant salivary gland calculi and are infrequently reported in the literature. Here, we report a case of unusually large submandibular gland calculus of 5 cm in greatest dimension which caused erosion of the oral cavity.
