Risk-reducing bilateral salpingo-oophorectomy in women with BRCA1 or BRCA2 mutations.

BACKGROUND: The presence of deleterious mutations in breast cancer 1 gene (BRCA1) or breast cancer 2 gene (BRCA2) significantly increases the risk of developing some cancers, such as breast and high-grade serous cancer (HGSC) of ovarian, tubal and peritoneal origin. Risk-reducing salpingo-oophorectomy (RRSO) is usually recommended to BRCA1 or BRCA2 carriers after completion of childbearing. Despite prior systematic reviews and meta-analyses on the role of RRSO in reducing the mortality and incidence of breast, HGSC and other cancers, RRSO is still an area of debate and it is unclear whether RR

Stillbirth due to placental hypoperfusion after salpingo-oophorectomy for an ovarian cyst.

BACKGROUND: Gravid oophorectomy past mid-pregnancy may be necessary, but the alterations of blood flow to supply the placenta may present risks to the mother and fetus. CASE: A salpingo-oophorectomy for a mucinous cystadenoma resulted in a postoperative hemorrhage of 2 L and fetal death. The placenta demonstrated a unique lesion that was consistent with global hypoperfusion of the placenta. CONCLUSION: The postoperative hemorrhage occurred despite good immediate operative hemostasis. Blood flow was shunted from the uteroplacental circulation due to the large utero-ovarian collateral circulat

Sexual functioning more than 15 years after premenopausal risk-reducing salpingo-oophorectomy.

BACKGROUND: Women with a BRCA1/2 pathogenic variant are advised to undergo premenopausal risk-reducing salpingo-oophorectomy after completion of childbearing, to reduce their risk of ovarian cancer. Several studies reported less sexual pleasure 1 to 3 years after a premenopausal oophorectomy. However, the long-term effects of premenopausal oophorectomy on sexual functioning are unknown. OBJECTIVE: This study aimed to study long-term sexual functioning in women at increased familial risk of breast or ovarian cancer who underwent a risk-reducing salpingo-oophorectomy either before the age of 46

Transvaginal Natural Orifice Transluminal Endoscopic Surgery (vNOTES) for prophylactic bilateral salpingo-oophorectomy.

BACKGROUND: Prophylactic bilateral salpingo-oophorectomy (BSO) is an important option for reducing the risk of developing ovarian and fallopian tube cancers in women with a hereditary ovarian cancer syndrome. Conventional laparoscopy is generally preferable since it is associated with less morbidity compared to laparotomy. Transvaginal Natural Orifice Transluminal Endoscopic Surgery (vNOTES) is an emerging surgical approach that offers several advantages over conventional laparoscopy including reduced postoperative pain, low rate of surgical site infections, fast patient recuperation and bette

Cholesterol profile in women with premature menopause after risk reducing salpingo-oophorectomy.

This cross-sectional study aimed to investigate the effect of premenopausal risk reducing salpingo-oophorectomy (RRSO) on the cholesterol profile of women at increased ovarian cancer risk and to assess possible effects of age at and time since RRSO. We included 207 women who underwent RRSO before menopausal age (52 years) attending the family cancer clinic of an academic hospital and 828 age-matched women from a general population cohort (PREVEND). Participants filled out a questionnaire on socio-demographic characteristics, lifestyle and medical history, had anthropometric measurements and pr

Risk-reducing salpingo-oophorectomy in patients with germline mutations in BRCA1 or BRCA2.

Women with mutations in BRCA1 or BRCA2 are at tremendously increased lifetime risk of both breast and BRCA-associated gynecologic (ovarian, fallopian tube, and primary peritoneal) cancer. Because of limitations in the efficacy of available screening and chemopreventive approaches for women with a mutation in one of these genes, surgical risk reduction, particularly risk-reducing salpingo-oophorectomy (RRSO), has become an important component of the management options in hereditary breast-ovarian cancer syndrome. This article will review the rationale and efficacy of RRSO for prevention of brea

Prophylactic bilateral salpingo-oophorectomy compared with surveillance in women with BRCA mutations.

OBJECTIVE: The objective of this study was to evaluate clinical factors associated with choosing prophylactic bilateral salpingo-oophorectomy (BSO) over surveillance in women with a BRCA1 or BRCA2 mutation. METHODS: Between 1996 and 2005, 139 women who tested positive for a BRCA1 or BRCA2 mutation were identified. Thirty-three women were excluded due to a personal history of ovarian or fallopian tube cancer before genetic testing, resulting in 106 women for the final analysis. The characteristics of women who underwent prophylactic BSO were compared with those choosing surveillance. RESULTS:

A live birth after bilateral salpingo-oophorectomy for seromucinous borderline ovarian tumor.

Borderline ovarian tumors primarily affect younger women and are associated with favorable prognosis, making fertility-sparing surgery a viable option. However, owing to recurrence and malignant transformation risks, careful monitoring is required. Seromucinous borderline ovarian tumors, a rare subtype that is often associated with endometriosis, lack standardized guidelines for fertility-sparing surgery or infertility management following bilateral salpingo-oophorectomy with uterine preservation. We present a case of bilateral seromucinous borderline ovarian tumors stage IC3 managed with bila

BRCA carriers, prophylactic salpingo-oophorectomy and menopause: clinical management considerations and recommendations.

Women who inherit a mutation in either the BRCA1 or BRCA2 gene have greatly elevated lifetime risks of ovarian cancer, fallopian tube cancer and breast cancer. Preventive surgical removal of the ovaries and fallopian tubes (salpingo-oophorectomy) is recommended to these women, often prior to natural menopause, to prevent cancer. The ensuing hormone deprivation may impact on health and quality of life. Most of these women experience menopausal symptoms shortly after surgery; however, there may also be longer term consequences that are less well understood. In this review, we highlight recent st

Manic episode after total abdominal hysterectomy with bilateral salpingo-oophorectomy: case report.

OBJECTIVE: Times of rapid decline in reproductive hormones have been associated with mood episode onset, and the menopausal transition confers an increased risk of depression. Mood state in women with bipolar disorder during the rapid decline in reproductive hormones resulting from surgical menopause has not been reported. METHODS: The case of a 46-year-old woman with bipolar disorder presenting to her psychiatrist after total abdominal hysterectomy with bilateral salpingo-oophorectomy is described. RESULTS: A manic episode was diagnosed, with onset 10 days after surgical menopause. CONCLUS

vNOTES Assisted Vaginal Bilateral Salpingo-Oophorectomy in Postmenopausal Women With Large Ovarian Fibroma.

The authors describe two cases where women with large ovarian fibromas (6 and 9 cm, respectively) underwent a transvaginal Natural Orifice Transluminal Endoscopic Surgery (vNOTES) assisted bilateral salpingo-oophorectomy. Retrieval was facilitated by transvaginal cold knife morcellation in the Pouch of Douglas under direct vision following vNOTES assistance without abdominal incisions. This is the first time vNOTES has been described for excision of solid ovarian tumors of these dimensions; the authors discuss its advantages compared to conventional laparoscopy, together with challenges of ret

Long-term outcomes after unilateral salpingo-oophorectomy: A registry-based retrospective cohort study.

BACKGROUND: Opportunistic bilateral salpingo-oophorectomy (BSO) is recommended in women who have undergone a hysterectomy due to gynecological carcinomas and/or in women with genetic indications, especially for women who do not intend to conceive. However, there is ongoing debate about whether BSO should be recommended in premenopausal women, due to the early cessation of estradiol because of BSO which is linked to several health concerns, including coronary artery disease (CAD) and osteoporosis. This study aims to explore whether ovarian cancer can be prevented by unilateral salpingo-oophorec

Long-term effects of premenopausal risk-reducing salpingo-oophorectomy on bone mineral density.

UNLABELLED: Women at high familial risk for ovarian cancer are recommended to undergo risk-reducing salpingo-oophorectomy (RRSO), leading to surgical menopause and short-term bone loss. However, long-term studies and osteoporosis screening recommendations are lacking. Eighteen years after premenopausal RRSO, women had lower bone mineral density compared with women who underwent a postmenopausal RRSO. PURPOSE: To prevent ovarian cancer, BRCA1/2 germline pathogenic variant carriers are recommended to undergo premenopausal risk-reducing salpingo-oophorectomy (RRSO). Premenopausal RRSO leads to i

Heterotopic adrenal tissue in a specimen after unilateral salpingo-oophorectomy: a case report.

BACKGROUND: Heterotopic adrenal tissue is an extremely rare finding. The most common site is the genitourinary tract and pelvis, more frequently in male than female children. In our report, we discuss an ectopic adrenal tissue detected incidentally in a perimenopausal woman, which is even more unusual. CASE PRESENTATION: A 54-year-old Eastern European perimenopausal female patient was referred for surgical treatment due to a suspected 60 mm paratubal cyst. Her medical history was unremarkable and risk of ovarian malignancy algorithm score were normal. Laparoscopic left salpingo-oophorectomy w
