A Tillaux Fracture and Concurrent Nondisplaced Salter-Harris Type III Fracture of the Distal Fibula: A Case Report.

The Salter-Harris classification system categorizes pediatric fractures in relation to the physis. A Salter-Harris type III fracture occurs from the physis extending to the epiphysis. Tillaux fractures are a type of Salter-Harris type III fracture that occurs due to incomplete fusion of the growth plate and includes the anterolateral tibial epiphysis. This specific fracture is unique to adolescents due to the anterior tibiofibular ligament's strength in relation to the growth plate, causing avulsion of the tibial fragment. The settings for a Tillaux fracture and a Salter-Harris type III fractu

Salter-Harris type III and IV epiphyseal fractures in the hand treated with tension-band wiring.

We present tension-band wiring in the treatment of Salter-Harris types III and IV avulsion fractures in the hand. By placing a small-gauge wire through the insertion of the ligament into the fracture fragment, accurate reduction and stability, allowing early mobilization, are achieved, avoiding many of the pitfalls and complications of other methods of internal fixation. The technique is simple and adheres to important physiological and biomechanical principles.

Displaced Salter-Harris Type I Distal Fibula Fractures: Two Case Reports and a Review of the Literature.

Salter-Harris type I (SH-I) fractures of the distal fibula are commonly encountered in pediatric orthopedics. We describe 2 unique cases of adolescents with completely displaced SH-I distal fibula fractures that were treated operatively. In the first case, a closed reduction attempt failed and the patient required open reduction and internal fixation of the distal fibula and syndesmosis. The syndesmotic ligaments were avulsed from the distal fibular metaphysis. In the second case, closed reduction of the distal fibula fracture was partially successful, but anatomic reduction could not be achie

Physeal Bar Formation After Salter-Harris III and IV Medial Malleolus Fractures in Children Aged ≤10 Years.

INTRODUCTION: Salter-Harris (SH) III and IV medial malleolar fractures in young children pose a risk of physeal bar formation and subsequent growth disturbance. The aim of this study was to compare surgical versus nonsurgical treatment and to identify factors predictive of physeal injury. METHODS: A retrospective cohort analysis examined patients aged ≤10 years treated for SH III/IV medial malleolar fractures between 2012 and 2024 at a single institution. Patients received either surgical (open reduction and internal fixation (ORIF) or percutaneous reduction and screw fixation (PRSF)) or nons

Concurrent Salter-Harris type II distal tibia fracture with ipsilateral talus and calcaneus fractures in a paediatric patient .

Paediatric bones exhibit greater elasticity due to their distinct biomechanical properties, enabling them to absorb and dissipate mechanical forces more effectively than adult bones. This inherent flexibility provides increased resistance to fractures under comparable loading conditions. However, despite this resilience, high-energy trauma can still result in complex injury patterns. Fractures involving the physis are of particular concern, as they carry a significant risk of growth disturbances and long-term deformities. We present a rare case of a girl in early childhood (preschool) who susta

Ultrasound-guided closed reduction of distal radius Salter-Harris I and II fracture in children.

BACKGROUND: Distal radial physeal fractures are the most common type of physeal fracture. However, few studies have been dedicated to the role of ultrasound-guided closed reduction in these fractures. This study aimed to investigate the utility of ultrasound-guided closed reduction in paediatric distal radial physeal fractures. METHODS: Consecutive patients undergoing ultrasound-guided closed reduction of fractures in our department between November 2017 and October 2019 were included. The adequacy of realignment according to the ultrasound and radiography was recorded. RESULTS: A total of 5

Delayed Presentation of Popliteal Artery Injury after Salter-Harris III Proximal Tibia Fracture.

INTRODUCTION: Proximal tibia physeal fractures in children are not very common but can be dangerous because they can harm popliteal fossa structures, especially the popliteal artery. Popliteal artery injuries (PAI) are most commonly the result of trauma to the lower extremity, including blunt force, hyperextension injuries, complex fractures, and knee dislocations that can compromise popliteal neurovascular structures. Case Presentation. A 14-year-old boy presents to the emergency department after being transferred from an outside hospital 24 hours after a left lower extremity hyperextension i

Two cases of missed Salter-Harris III coronal plane fracture of the lateral femoral condyle.

Coronal plane fractures of the lateral femoral condyle can be difficult to diagnose, especially in children with open physis. Two adolescents who sustained this uncommon Salter-Harris III fracture of the knee were misdiagnosed after initial clinical examination and standard x-rays. Oblique x-rays, computed tomography, and magnetic resonance imaging were valuable in arriving at the correct diagnosis and in decision making.

Risk Factors and Surgical Sequelae of Physeal Arrest in Pediatric Salter-Harris III and IV Medial Malleolus Fractures.

BACKGROUND: Pediatric medial malleolus fractures pose a risk for physeal bar formation and growth disturbances. This study aimed to determine the rate of physeal bar formation following Salter-Harris (SH) III or IV medial malleolus fractures and identify patient, fracture, and management factors predictive of bar formation. A secondary objective was to evaluate the rate of additional surgeries required in the event of physeal bar formation. METHODS: A retrospective review was performed of 161 patients (age 16 y or younger) with isolated medial malleolus or bimalleolar fractures. Fifty-six ske

OPERATIVE VERSUS NONOPERATIVE MANAGEMENT OF SALTER-HARRIS TYPE II DISTAL RADIUS FRACTURES IN CHILDREN: A RETROSPECTIVE COHORT STUDY.

BACKGROUND: Salter-Harris type II distal radius fractures are the most common physeal injuries in children. Although closed reduction and cast immobilisation are widely used, concerns about loss of reduction have led to increasing use of Kirschner wire fixation. The relative benefit of routine wire fixation following intraoperative reduction remains uncertain. METHODS: We performed a retrospective cohort study of 57 children aged 5-16 years treated for Salter-Harris type II distal radius fractures at a single centre between 2019 and 2024. All underwent manipulation under anaesthesia and were

A Retrospective Comparison of Above- vs Below-the-Knee Cast Treatment for Salter Harris-II Distal Tibia Fractures.

BACKGROUND: Distal tibia fractures are common in the pediatric patient population. Recent reports suggest that patients with closed low-energy distal tibial fractures treated with short leg casts (SLCs) have similar radiographic outcomes with improved functional outcomes compared to those treated with long leg casts (LLCs). However, to date there has not been a study comparing these treatment modalities for Salter-Harris (SH) II distal tibia fractures. The purpose of this study was to compare the radiographic and time to weightbearing outcomes between patients with SH-II tibial ankle fractures

Salter-Harris II fractures of the distal tibia: does surgical management reduce the risk of premature physeal closure?

INTRODUCTION: Premature physeal closure (PPC) is a common complication resulting from the management of a displaced Salter-Harris II (SH II) fracture of the distal tibia. The purpose of this study was to evaluate our institution's treatment approach to assess PPC and complication rates of fractures treated both surgically and nonsurgically. METHODS: We performed a retrospective review of all patients presenting with a displaced SH II fracture between 2004 and 2010. Initial treatment was closed reduction in the emergency department. Further treatment and subsequent categorization was based on

Combining Salter-Harris and Dias-Tachdjian could be better at determining the prognosis of distal tibial physeal fractures.

OBJECTIVES: The aim of this study is determining the factors that affect prognosis of distal tibial physeal fractures (DTPF) and analyzing whether Salter-Harris (SH) or Dias-Tachdjian (DT) classification is more predictive for outcomes. METHODS: Patients treated for DTPF were retrospectively analyzed. Fracture patterns were classified according to SH and DT. Treatment methods and fracture characteristics were noted. Distal tibial angles and joint irregularities were analyzed on patient's final ankle radiographs followed by American Orthopedic Foot and Ankle Score questionnaire. RESULTS: 75 p

A closed phalangeal neck fracture with 180-degree rotation of the phalangeal head and concurrent displaced Salter-Harris II fracture.

Closed type III phalangeal neck fractures with 180-degree rotation is a rare iatrogenic injury that occurs following failed attempts at closed reduction. Prior to closed reduction, the phalangeal head is in 90-degree rotation. Longitudinal traction during closed reduction then converts the deformity into 180-degree rotation. We present the first documented noniatrogenic case of phalangeal neck fracture with 180-degree rotation that was also associated with a displaced Salter-Harris II fracture at the same joint. The blood supply of the phalangeal head in that scenario is discussed along with p

Management options and outcomes for Salter-Harris type 3 and 4 fractures of the proximal phalanx of the hallux in children.

Epiphyseal fractures of the proximal phalanx of the hallux are rare in pediatric patients. While various management techniques exist, studies detailing outcomes, complications, and pathogenic mechanisms of Salter-Harris (S-H) 3 and S-H 4 fractures in children are limited. This study evaluates the radiographic, clinical, and functional outcomes of pediatric patients with these injuries and identifies potential risk factors for adverse results. A retrospective review was conducted on 11 pediatric patients diagnosed with S-H 3 and S-H 4 fractures. Demographics, injury mechanisms, and treatment mo

Surgical Management of a Salter-Harris Type I Distal Physeal Fracture of the Tibia in a Calf: A Case Report.

Fractures are common conditions in cattle, including tibial fractures. Physeal tibial fractures are more specific and less frequently met in field conditions. A calf with a Salter-Harris type I distal physeal fracture of the tibia was referred to the National Veterinary School of Toulouse (ENVT), France. Although the use of external fixators in the treatment of tibial fractures is common, distal physeal tibial fractures require a different and specific technique involving them. They were first used as a lever arm to reduce the fracture due to the severe displacement. A hock joint bypass was th

Case report: A rare Salter-Harris V metaphyseal fatigue fracture of the knee in an adolescent patient with obesity.

Stress fractures are rare, occurring in 1.5/100,000 high school athletes. High impact, repetitive loading participation in woman's sports, and being a white athlete have been identified as risk factors for stress fractures. Mostly treated conservatively, they are more common in the tibia (33%). Stress fractures requiring surgery, which are extremely rare, have been reported in the scaphoid, fifth metatarsal, and neck of femur. Herein, a 16-year-old adolescent patient with obesity presented with atypical knee pain after prolonged exercise. Advanced imaging revealed a stress fracture of the left

Salter-harris type 2 fracture of the proximal phalanx of the thumb with a rotational deformity: a case report and review.

Hand fractures are the most common site of injury in the pediatric population. They commonly involve the epiphyseal growth plates, and their standard classification is that of Salter-Harris (SH). Rotational deformities after SH fractures are rarely reported in literature. However, only 5 degrees of angulation can cause evident rotational deformity. This could be seen clinically and reconfirmed with radiologic evaluation. Up to this date, there are only a few cases of SH fractures with rotational deformity that have been described. We present 2 cases of SH type 2 with evidence of rotational def
