Does Samoa Have Adequate Policies to Reduce Obesity and Obesity-Related Disease?

In obesity, abnormal or excess body fat (adiposity) impairs health, increases the risk of long-term medical complications, and results in decreased lifespan. There are many long-term medical complications to consider, such as heart disease, cancer, stroke, type 2 diabetes mellitus, and non-alcoholic fatty liver disease, among others. These are all difficult and expensive conditions to treat in a small developing country like Samoa. The prevalence of obesity in Samoa has steadily increased since the 1960s, and today, 53% of Samoa's adult population are obese. People living in the small island c

Spatially Explicit Environmental Factors Associated with Lymphatic Filariasis Infection in American Samoa.

Under the Global Program to Eliminate Lymphatic Filariasis (LF) American Samoa conducted seven rounds of mass drug administration (MDA) between 2000 and 2006. Subsequently, the territory passed the WHO recommended school-based transmission assessment survey (TAS) in 2011/2012 (TAS-1) and 2015 (TAS-2) but failed in 2016, when both TAS-3 and a community survey found LF antigen prevalence above what it had been in previous surveys. This study aimed to identify potential environmental drivers of LF to refine future surveillance efforts to detect re-emergence and recurrence. Data on five LF infecti

Notes from the Field: Outbreak of Zika Virus Disease - American Samoa, 2016.

During December 2015-January 2016, the American Samoa Department of Health (ASDoH) detected through surveillance an increase in the number of cases of acute febrile rash illness. Concurrently, a case of laboratory-confirmed Zika virus infection, a mosquito-borne flavivirus infection documented to cause microcephaly and other severe brain defects in some infants born to women infected during pregnancy (1,2) was reported in a traveler returning to New Zealand from American Samoa. In the absence of local laboratory capacity to test for Zika virus, ASDoH initiated arboviral disease control measure

Acute rheumatic fever and rheumatic heart disease among children--American Samoa, 2011-2012.

Acute rheumatic fever is a nonsuppurative, immune-mediated consequence of group A streptococcal pharyngitis (strep throat). Recurrent or severe acute rheumatic fever can cause permanent cardiac valve damage and rheumatic heart disease, which increases the risk for cardiac conditions (e.g., infective endocarditis, stroke, and congestive heart failure). Antibiotics can prevent acute rheumatic fever if administered no more than 9 days after symptom onset. Long-term benzathine penicillin G (BPG) injections are effective in preventing recurrent acute rheumatic fever attacks and are recommended to b

Dapsone Resistance in Leprosy Patients Originally from American Samoa, United States, 2010-2012.

Skin biopsies from US leprosy patients were tested for mutations associated with drug resistance. Dapsone resistance was found in 4 of 6 biopsies from American Samoa patients. No resistance was observed in patients from other origins. The high rate of dapsone resistance in patients from American Samoa warrants further investigation.

Acute hemorrhagic conjunctivitis caused by enterovirus type 70: an epidemic in American Samoa.

From December 1981 to February 1982, an estimated 22,000 cases of acute hemorrhagic conjunctivitis (AHC) caused by enterovirus type 70 (EV 70) occurred among Samoan and non-Samoan residents of American Samoa. The overall attack rate was estimated to be 68%. Samoans of all ages resident in traditional housing and of large family size were at greatest risk of acquiring AHC, while non-Samoan adults resident in western style housing were at lowest risk. Epidemiologic aspects of AHC acquisition were also different for the Samoan and non-Samoan communities; index cases in Samoan households were freq

Bridging worlds: polycentric collaboration and the integration of traditional governance in Samoa's disaster management.

Effective multi-agency collaboration (MAC) is essential for disaster risk reduction; however, its implementation remains profoundly challenging in contexts where modern state structures intersect with traditional governance systems. Grounded in polycentric governance theory, this study investigates the barriers to MAC in Samoa, a Small Island Developing State characterized by the coexistence of formal agencies and the indigenous fa’a-Samoa governance system. Using a qualitative case study design, semi-structured interviews were conducted with 15 senior officials across Samoa’s disaster managem

Validation of the Latin American and Caribbean Food Security Scale (ELCSA) for use in Samoa.

BACKGROUND: Measuring food security accurately has implications for policies and programming designed to address both hunger and obesity risk among children. OBJECTIVES: We aimed to examine the validity of the Latin American and Caribbean Food Security Scale (ELCSA) for use in Samoa. METHODS: The ELCSA was administered verbally to 454 Samoan mothers who were participants in an ongoing longitudinal cohort study (the Ola Tuputupua'e Study). Internal consistency and construct validity were examined with Rasch modeling, which generated item severity and item infit statistics. Associations betwee

Evaluating Molecular Xenomonitoring as a Tool for Lymphatic Filariasis Surveillance in Samoa, 2018-2019.

Molecular xenomonitoring (MX), the detection of filarial DNA in mosquitoes using molecular methods (PCR), is a potentially useful surveillance strategy for lymphatic filariasis (LF) elimination programs. Delay in filarial antigen (Ag) clearance post-treatment is a limitation of using human surveys to provide an early indicator of the impact of mass drug administration (MDA), and MX may be more useful in this setting. We compared prevalence of infected mosquitoes pre- and post-MDA (2018 and 2019) in 35 primary sampling units (PSUs) in Samoa, and investigated associations between the presence of

Laboratory studies of Ochlerotatus samoanus in association with leaf axils of Freycinetia (Pandanaceae) in Samoa.

Three species of Freycinetia (Pandanaceae) (F. reineckei, F. storkii, and F. hombronii) were tested for oviposition preference of Ochlerotatus samoanus, a vector of filariasis in Samoa. Laboratory tests indicated that F. reineckei was preferred by this mosquito for oviposition. Eggs were preferentially deposited on a peeled or a moist dried leaf. The percentage hatch was highest when eggs were kept moist for longer than 4 days before submersion. Hatching was complete in less than 6 h. These studies allowed us to successfully rear Oc. samoanus in the laboratory, facilitating future studies on t

Morphological variation of the frontal sinus in Melanesian (Fiji) and Polynesian (Western Samoa) populations.

The aims of this study were to investigate gender differences in frontal sinus morphology and to estimate differences between Fijian and Western Samoan populations. Cephalograms of 118 Fijians (59 male, 59 female), 80 W. Samoans (33 male, 47 female) and 89 Japanese (60 male, 29 female) were used in this study. The thickness of the frontal sinus and the convexity of glabella in males were found to be larger than those of females in each population. The angle of Sg-N-G in the Fijians was larger than that of the Western Samoans and Japanese. In order to compare the frontal sinus morphology among

An epidemic of acute hemorrhagic conjunctivitis in American Samoa caused by coxsackievirus A24 variant.

Between May 25 and July 5, 1986, an epidemic of acute hemorrhagic conjunctivitis affected an estimated 47% of the population on American Samoa. Coxsackievirus A24 variant was isolated from 18 of 22 patients. This is the first documented outbreak of acute hemorrhagic conjunctivitis due to coxsackievirus A24 variant outside of Southeast Asia and the Indian subcontinent. When this outbreak was compared with an outbreak on the island in 1981-1982 caused by enterovirus 70, conjunctival hemorrhage or injection and the severity of hemorrhage were less prevalent among cases in 1986, while upper respir

Targeted research to improve invasive species management: yellow crazy ant Anoplolepis gracilipes in Samoa.

Lack of biological knowledge of invasive species is recognised as a major factor contributing to eradication failure. Management needs to be informed by a site-specific understanding of the invasion system. Here, we describe targeted research designed to inform the potential eradication of the invasive yellow crazy ant Anoplolepis gracilipes on Nu'utele island, Samoa. First, we assessed the ant's impacts on invertebrate biodiversity by comparing invertebrate communities between infested and uninfested sites. Second, we investigated the timing of production of sexuals and seasonal variation of

Scabies prevalence after ivermectin-based mass drug administration for lymphatic filariasis, Samoa 2018-2019.

BACKGROUND: Scabies is a common skin infestation caused by the Sarcoptes scabei mite. Ivermectin, one of three drugs used in mass drug administration (MDA) for lymphatic filariasis, is also effective for treating scabies. Ivermectin-based MDA was first conducted in Samoa in August 2018, with ivermectin being offered to those aged ≥5 years. Here, we report scabies prevalence in Samoa after MDA. METHODS: We conducted household surveys 1.5-3.5 months (Survey 1) and 6-8 months (Survey 2) after the 2018 MDA in 35 primary sampling units. We conducted clinical examination for scabies-like rash and u

"We're Trapped": Maternal Well-Being and Caregiving for Children With Developmental Disabilities in American Samoa.

Mothers raising children with developmental disabilities in American Samoa navigate caregiving within fa'a Samoa (the Samoan way of life), where family, culture, and spirituality are deeply intertwined. Yet, the lived experiences of these mothers remain largely undocumented. This study explored how mothers make meaning of caregiving and its implications for holistic well-being within this unique cultural and geographic context. Using interpretative phenomenological analysis with culturally congruent focus group methods, 15 mothers participated across three focus groups. Data were interpreted t

Swarming behavior of Aedes polynesiensis (Diptera: Culicidae) and characterization of swarm markers in American Samoa.

We characterize the swarming behavior of male Aedes polynesiensis (Marks) in American Samoa. Instead of swarming around a blood host, males used the base of certain trees as a marker. Repeated sampling proved nondestructive and allowed us to investigate the impact of static (e.g., tree species) and dynamic (e.g., barometric pressure) characters on the likelihood of swarm presence and intensity. Tree circumference and oviposition activity (number of Ae. polynesiensis reared from oviposition cups) were significant positive predictors of the number of males in a swarm. Tree circumference and diam

Evaluation of three traps for sampling Aedes polynesiensis and other mosquito species in American Samoa.

The efficacy of the recently developed BG-Sentinel mosquito trap baited with BG-Lure (a combination of lactic acid, ammonia, and caproic acid) was evaluated in American Samoa against the omnidirectional Fay-Prince trap and the Centers for Disease Control and Prevention (CDC) light trap, both baited with carbon dioxide. The BG-Sentinel trap captured the greatest number of the important filariasis and dengue vector Aedes (Stegomyia) polynesiensis at all 3 collection locations; however, its catch rate was not significantly different from that of the Fay-Prince trap at 2 of the 3 trapping location
