Hematogenous pneumococcal meningitis in the infant rat: description of a model.

Sensorineural deafness occurs in 20%-30% of children after Streptococcus pneumoniae meningitis. An infant rat model of S. pneumoniae meningitis was developed to study the pathogenesis of inner ear invasion by S. pneumoniae. S. pneumoniae type 6 was administered intraperitoneally (inoculum: 1-10 x 10(8) cfu) every 24 h for 3 days to 5-day-old rats. Bacteremia (12 [50%] of 24) and meningitis (11 [46%] of 24) were detected most frequently 4 days after the three doses. The mean cerebrospinal fluid (CSF) white blood count for rats with positive CSF cultures was 7271/mm3 (range, 81-20,475). Hematoxy

Effects of glycerol on the inner ear fluid electrolytes of guinea pigs--oral and intravenous administration.

Under sodium pentobarbital anesthesia (25-35 mg/kg), serum, CSF and inner ear fluids from scala tympani perilymph, scala vestibuli perilymph, and scala media endolymph were collected from normal guinea pigs after the oral administration of glycerol (50%, 12 ml/kg) or the intravenous injection of glycerol (1.0 ml/kg). The sodium and potassium concentrations were assessed by microflame photometry. The electrolyte dynamics of the inner ear fluids were compared after these two routes of administration. Serum: There was no significant change in Na or K levels after either route of administration. C

Cochlear Implantation in a Case of Relapsing Polychondritis With Profound Hearing Loss and Labyrinthine Ossification.

Relapsing polychondritis is an autoimmune disorder causing inflammation of cartilaginous structures, sensory epithelium, and cardiovascular system. Hearing loss is a rare and dreadful complication of this pathology. We report a case of relapsing polychondritis in a 38-year-old female who developed gradually progressive bilateral profound hearing loss. She did not have any improvement with medical management. Cochlear implantation was performed to rehabilitate her hearing. As the scala tympani was obliterated, a scala vestibuli insertion was performed. A complete insertion was possible with a c

Modulation of cochlear tuning by low-frequency sound.

An intense, low-frequency tone (about 30 Hz) modulates the sensitivity of the inner ear to high-frequency stimulation. This modulation is correlated with the displacement of the basilar membrane. The findings suggest that the modulation may also affect cochlear tuning. We have investigated modulation of cochlear tuning by low-frequency sound in the guinea pig. Applying indirect methods of measurement (narrow-band analysis of compound action potentials and compound-action-potential tuning curves), the results suggest a shift of the excitation pattern along the basilar membrane towards higher-fr

Incomplete Partition Type II Cochlear Malformations: Delineating the Three-Dimensional Structure from Digitized Human Histopathological Specimens.

HYPOTHESIS: There are clinically relevant differences in scalae anatomy and spiral ganglion neuron (SGN) quantity between incomplete partition type II (IP-II) and normal cochleae. BACKGROUND: IP-II is a commonly implanted cochlear malformation. Detailed knowledge of intracochlear three-dimensional (3D) morphology may assist with cochlear implant (CI) electrode selection/design and enable optimization of audiologic programming based on SGN maps. METHODS: IP-II (n = 11) human temporal bone histological specimens were identified from the National Institute on Deafness and Other Communication Di

Labyrinthitis ossificans: histopathologic consideration for cochlear implantation.

Labyrinthitis ossificans may be a hindrance to cochlear implantation by making electrode insertion difficult. We performed a histopathologic study of 24 temporal bones with labyrinthitis ossificans from multiple causes. The organ of Corti was graphically reconstructed and the degree of obstruction was estimated for each millimeter of the cochlea. Correlations were calculated between the degree of new bone formation and the cause, patient's age and sex, and time from the original temporal bone insult. Our results demonstrate that complete cochlear ossification is rare. The scala tympani in the

Morphometric Measurements of the Incomplete Partition Type II (IP-II) Cochlea and Implications on Cochlear Implantation.

HYPOTHESIS: The objective of this study is to obtain comprehensive morphometric measurements of the incomplete partition type II (IP-II) cochlea to provide a better understanding of intracochlear anatomy and important considerations for electrode selection and insertion. BACKGROUND: IP-II is the most common bony inner ear malformation that often requires cochlear implantation. Currently, there is significant controversy on electrode selection due to a lack of research that can provide reliable, high-resolution measurements. METHODS: Three-dimensional reconstructions of the cochlea were made

Old theme and new reflections: hearing impairment associated with endolymphatic hydrops.

The cause(s) of the hearing impairment associated with Menière's disease are not understood but are undoubtedly associated with the inner ear endolymphatic hydrops. Two major hypotheses have been proposed and widely received: endolymphatic overpressure followed by leaky membranes and subsequently the mixing of high K+ endolymph with perilymph. Our recent data on an experimental model of endolymphatic hydrops have provided grounds for renewed reflections on the pathology. Indeed our data might be interpreted without involving either of the above hypotheses and suggests that the symptoms of Meni

Effects of localized neurotrophin gene expression on spiral ganglion neuron resprouting in the deafened cochlea.

A cochlear implant may be used to electrically stimulate spiral ganglion neurons (SGNs) in people with severe sensorineural hearing loss (SNHL). However, these neurons progressively degenerate after SNHL due to loss of neurotrophins normally supplied by sensory hair cells (HCs). Experimentally, exogenous neurotrophin administration prevents SGN degeneration but can also result in abnormal resprouting of their peripheral fibers. This study aimed to create a target-derived neurotrophin source to increase neuron survival and redirect fiber resprouting following SNHL. Adenoviral (Ad) vectors expre

In Vivo Imaging of Cochlear Implant Electrode Orientation Relative to Scalar Tilt.

OBJECTIVE: Visualization of cochlear implant (CI) electrode contact position offers insights into electrode-cochlea biomechanics. Prior in vivo imaging studies have assessed electrode distances to a fixed modiolar axis, but electrodes' orientation within the cochlear canal and their relationship to the scala tympani's dynamic anatomical tilt have not been investigated. This study employed in vivo flat-panel computed tomography (FPCT) imaging to examine the flexibility and dynamic behavior of lateral wall arrays. METHODS: This retrospective cohort study included eight MED-EL FLEX28 CI users. F

Enhanced oval window and blocked round window passages for middle-inner ear transportation of gadolinium in guinea pigs with a perforated round window membrane.

To elucidate the communication between the middle and inner ear, and the fluid dynamics of the inner ear with the perilymphatic fistula (PLF) of the round window membrane (RWM). The PLF of the RWM was created in nine guinea pigs. Gadolinium diethylenetriamine pentaacetic acid bismethylamide (Gd-DTPA-BMA) was delivered into the middle ear and followed in the inner ear using a 4.7 Tesla MRI. Pressure was delivered to the external ear canal of PLF ear in an attempt to enhance the inner ear uptake of Gd-DTPA-BMA. The immediate loading of Gd-DTPA-BMA in the scala tympani of the basal turn was ablat

Proinflammatory cytokine expression in the endolymphatic sac during inner ear inflammation.

The inner ear is capable of rapidly mounting an immune response that can ultimately lead to cochlear degeneration and permanent hearing loss. The role of the endolymphatic sac in this immune process is not clear. In order to investigate the cytokine expression of cells within the endolymphatic sac, a secondary inner ear immune response to keyhole limpet hemocyanin (KLH) was created in mice. The animals were sacrificed 3-48 h and 7 days following initiation of the immune response. The cochleas and endolymphatic sacs were assayed by immunocytochemistry for IL-1beta, TNFalpha, and IL-6. Evaluation of a silicon-substrate modiolar eighth nerve implant in a guinea pig.

The availability of thin-film multichannel electrodes provides new possibilities for implantation and direct stimulation of the modiolar portion of the auditory nerve. Electrodes in direct contact with the auditory nerve should be functional at lower thresholds and might require fewer remaining neurons for stimulation compared to electrodes in the scala tympani. This strategy would also provide close contact with neural elements subserving a greater frequency range. Implantation of the eighth nerve may also be advantageous in profoundly deaf subjects who lack an implantable scala tympani. In t

High-resolution 3-D T2-weighted imaging in the diagnosis of labyrinthitis ossificans: emphasis on subtle cochlear involvement.

BACKGROUND: Meningitis is the most common cause of acquired sensorineural hearing loss (SNHL) in children. Labyrinthitis ossificans develops in a significant number of patients with meningitis-related SNHL. Reduced T2 signal on MRI within the membranous labyrinth is often noted in the fibrous and ossifying stages of labyrinthitis ossificans. OBJECTIVE: The purpose of this study is to demonstrate the distribution and extent of involvement of the cochlea in children being evaluated for labyrinthitis ossificans; using high-resolution 3-D T2-weighted imaging, and to evaluate for subtle involvemen

Biohybrid cochlear implants: neural interfaces, regenerative pathways, and translational benchmarks.

Cochlear implants (CIs) restore hearing by electrically stimulating the auditory nerve, yet the millimeter-scale electrode-neuron separation limits spatial selectivity and temporal fidelity. Rather than relocating the electrode array, we propose a biohybrid strategy that preserves the conventional scala tympani insertion and closes this gap biologically by interposing living neural constructs between the array and the modiolus. This approach leverages the Canaliculi Perforantes of Schuknecht (CPS) to deliver spatially confined biochemical, mechanical, and electrical cues from the scala tympani
