Cutaneous and systemic infection with Scedosporium apiospermum.

We present a case of cutaneous and systemic infection with Scedosporium apiospermum occurring in a patient with systemic lupus erythematosus. Scedosporium apiospermum is a rare cause of systemic mycosis but is associated with a high rate of mortality in immunocompromised hosts. Because the patient presented in an advanced state of infection, supportive measures were unsuccessful and definitive diagnosis could not be made until postmortem examination. We discuss the presentation, pathology, diagnosis, and treatment strategies for Scedosporium infection.

Nasal infection with Scedosporium apiospermum in a dog.

CASE HISTORY: A 2-year-old female Siberian Husky was presented with a 6-month history of sneezing and mucous discharge from the right nostril. CLINICAL FINDINGS: Reduced airflow through the right nostril was evident. Radiographs showed subtle loss of detail of turbinates within the right nasal chamber. Rhinoscopy revealed swollen and erythematous turbinates and a white mass within the caudal aspect of the right nasal cavity. Histopathologically, there was a heavy mixed inflammatory infiltrate in the submuscosa of the right turbinate, and the presence of fungal hyphae and spores in the white m

Airborne outbreak of nosocomial Scedosporium prolificans infection.

We describe six inpatients with acute non-lymphocytic leukaemia who developed invasive infection with Scedosporium prolificans resistant to amphotericin B, flucytosine, ketoconazole, fluconazole, and itraconazole. All six patients died. Phenotypic and genotypic assessment of samples from clinical material and ambient air from the isolation rooms where the patients were being treated showed that the epidemic was caused by a single strain. After implementation of aerial control measures, there were no further infections with this organism. We conclude that fatal multidrug-resistant S prolificans

Infections due to Scedosporium apiospermum and Scedosporium prolificans in transplant recipients: clinical characteristics and impact of antifungal agent therapy on outcome.

BACKGROUND: Unique characteristics, impact of therapy with antifungal agents, and outcome of infections with Scedosporium species were assessed in transplant recipients. METHODS: The patients comprised a total of 80 transplant recipients with Scedosporium infections, including 13 patients from our institutions (University of Pittsburgh Medical Center [Pittsburgh, PA], University of Maryland [Baltimore], Duke University Medical Center [Durham, NC], Emory University [Atlanta, GA], and Hospital Gregorio Maranon [Madrid, Spain]) and 67 reported in the literature. The transplant recipients were co

Susceptibility and diversity in the therapy-refractory genus scedosporium.

Scedosporium species show decreased susceptibility to the majority of systemic antifungal drugs. Acquired resistance is likely to disseminate differentially with the mode of exchange of genetic material between lineages. Inter- and intraspecific diversities of Scedosporium species were analyzed for three partitions (rDNA internal transcribed spacer gene [ITS], partial β-tubulin gene, and amplified fragment length polymorphism profiles), with the aim to establish distribution of resistance between species, populations, and strains. Heterogeneity of and recombination between lineages were determ

[Fungemia caused by Scedosporium prolificans in myelodysplastic syndrome].

We report a case of fungemia caused by Scedosporium prolificans, an emerging pathogen. An 83-year-old man with myelodysplastic syndrome (MDS) and agranulocytosis was admitted for pneumonia in January 2009. He was treated with meropenem, minocycline, and gamma-globulin for pneumonia and G-CSF and platelet transfusion for MDS. Although he recovered from pneumonia as neutrophil count increased, intermittent fever continued. On hospital day 17, blood culture yielded fungal colonies indicating S. prolificans. Voriconazole was started immediately, but the man's general condition deteriorated with ce

Fungal rhinosinusitis caused by Scedosporium apiospermum in a cat.

A 3-year-old neutered male Bengal cat with a history of chronic mucopurulent bilateral nasal discharge and sneezing was diagnosed with severe fungal rhinosinusitis. A diagnosis was obtained after computer tomography imaging, histopathological examination and fungal culture. The mold Scedosporium apiospermum was identified as the aetiological agent. To our knowledge, this case is the first description of a rhinitis or sinusitis caused by this agent in a cat. Aggressive surgical debridement combined with topical and systemic antifungal therapy was performed. Unfortunately, the treatment resulted

Scedosporium scleritis or keratitis or both: case series.

PURPOSE: To report case series of infectious scleritis or keratitis or both caused by Scedosporium fungal species. METHODS: Medical records of patients presenting with infectious scleritis or keratitis or both caused by Scedosporium were retrospectively reviewed at a tertiary care hospital. Nine culture-proven cases of Scedosporium corneal and scleral infection that were treated medically or surgically or both were enrolled for the study. The main parameters evaluated were the predisposing factors, clinical presentation, and the success of various treatment modalities. RESULTS: The most comm

Voriconazole in fungal keratitis caused by Scedosporium apiospermum.

OBJECTIVE: To describe the first case, to the best of our knowledge, of posttraumatic Scedosporium apiospermum (ScA) keratitis successfully treated with systemic and topical voriconazole. CASE SUMMARY: A 19-year-old man was admitted to the hospital with an incisive wound of his left eye and the cornea totally sectioned after trauma with a cutter used in gardening. Initial empirical treatment was followed by systemic and topical voriconazole, and the eye did not have to be enucleated. Five months after the trauma, a penetrating keratoplasty and chamber intraocular lens implantation was perform

Cutaneous Scedosporium apiospermum infection in an immunocompromised patient.

Scedosporium apiospermum infection occurred in the left forearm of a patient who was taking oral prednisolone for pulmonary fibrosis. The infection appeared to follow a scratch from a blackcurrant bush. This is the first reported case in the United Kingdom of a cutaneous infection from Scedosporium apiospermum in an immunocompromised patient.

Endocarditis and hip arthritis associated with Scedosporium inflatum.

A 30-year-old man had endocarditis and hip arthritis associated with the recently recognized fungus, Scedosporium inflatum. Inhibition, and possibly cure, of endocarditis occurred with medical therapy alone. Data from in vitro susceptibility testing did not predict clinical response. This case also illustrates the need to standardize fungal susceptibility testing and to remain vigilant for emerging pathogens.

Scedosporium-induced keratitis: insights from a case study.

INTRODUCTION: Pseudallescheria boydii with its asexual form, Scedosporium apiospermum, is now recognized as an important emerging opportunistic pheoid-hypomycosis. Usually results in invasive infections in immunocompromised patients but rarely can infect the eyes. Herein, we report a challenging case of Pseudallescheria boydii keratitis in a lady with traumatic injury. CASE REPORT: A 38-year-old female presented with alleged history of penetrating injury to right eye with stick by her spouse for which she was started on antibiotic eye drops from nearby local hospital and became symptomaticall

Scedosporium apiospermum invasive sinusitis presenting as extradural abscess.

INTRODUCTION: Chronic invasive fungal rhinosinusitis (CIFR) is a rare entity generally observed in immunodepressed subjects. The pathogen most frequently identified is Aspergillus spp. Imaging generally reveals invasive pseudoneoplastic features. We report a case of Scedosporium apiospermum (S. apiospermum) CIFR with an atypical clinical and radiological presentation. CASE REPORT: A 72-year-old immunocompetent man presented with chronic headache, neck pain and bilateral limitation of lateral gaze. Imaging revealed an isolated left sphenoidal lesion with marked bone changes and an extradural a
