Supplemental photocoagulation on the buckle for prevention of surgical revision after scleral buckling procedures.

Supplemental photocoagulation on the buckle appears to be a useful therapeutic modality in selected cases of persistent retinal detachment. In a retrospective review, 71 (63%) of 113 eyes were successfully treated with photocoagulation, thus avoiding the need for surgical revision. Median response time between treatment and postoperative subretinal fluid absorption in the successfully treated eyes was two days, with the majority responding within one day. The one favorable prognostic factor was the use of a soft silicone segmental sponge. Unfavorable prognostic factors included the following:

Recurrent hemolacria: a sign of scleral buckle infection.

PURPOSE: The purpose of this study was to report recurrent hemolacria, or "bloody tears," as a sign of scleral buckle (SB) infection. METHODS: This is an interventional case series of three eyes of three patients with hemolacria after SB placement. RESULTS: Two men and one woman were treated for recurrent hemolacria after SB placement for a rhegmatogenous retinal detachment. Two patients had an encircling silicone sponge placed, one 6 years and the other 3 years before presentation. The third patient had a segmental solid silicone element placed 3 months before presentation. Two of the patie

Scleral buckle infections due to atypical mycobacteria.

Six cases of scleral buckle infections due to atypical mycobacteria were studied to determine their clinical and microbiologic features and the response of these infections to treatment. All infections were treated with exoplant removal and administration of a variety of antibiotics. Symptoms and clinical signs of infection resolved rapidly after treatment in all cases, and visual acuity returned to the best preoperative level in 5 of 6 cases. There are no clinical features to distinguish mycobacterial infections from other causes of scleral buckle infection, although scleral rupture occurred

Functional muscle insertional changes and strabismus after scleral buckling.

We report 3 cases of esotropia following scleral buckling surgery for retinal detachment, a relatively undocumented complication. All patients exhibited alterations at the site of the rectus muscle insertions. In all cases, the distance from the limbus to the functional muscle insertion was increased, highlighting the importance of considering the functional muscle insertion in strabismus surgery after scleral buckling.

Persumed sympathetic Ophthalmia after scleral buckling surgery: case report.

BACKGROUND: Scleral buckling (SB) is usually considered an extraocular operation premeditated to have a low risk of sympathetic ophthalmia (SO). Here we report a rare case of presumed SO in a young female patient following SB. CASE PRESENTATION: A nineteen-year-old female patient was referred for visual loss in her left eye due to macula off inferior long-standing rhegmatogenous retinal detachment (RRD). The best corrected visual acuity (BCVA) was 20/400 in the left eye. SB with 360 degrees encircling band, an inferior segmental tire with one spot cryoretinopexy at the break site, and subreti

Segmental scleral buckling without sling sutures around the muscles.

PURPOSE: To introduce a modified technique for segmental buckling through a small in situ conjunctival opening without rectus muscle traction for minimally invasive repair of uncomplicated rhegmatogenous retinal detachment. METHODS: This technique was performed in 15 eyes of 15 selected patients with primary retinal detachment. Conventional traction sutures placed beneath rectus muscles were replaced by an absorbable translimbal traction suture at lesion quadrant. The conjunctival incision was made 8-9 mm posterior and parallel to the limbus. After cryopexy, a minimal explant was fixed with o

Scleral buckling surgery in stage 4 retinopathy of prematurity.

BACKGROUND: Technical advances in neonatal intensive care have significantly increased the number of very low birth-weight babies that survive the perinatal period. Some of these infants develop severe retinopathy of prematurity that may lead to retinal detachment. METHODS: Between November 1988 and January 1994, 28 eyes from 15 preterm babies underwent scleral buckling for stage 4 retinopathy of prematurity at a mean age of 4.2 months. Cryotherapy was performed preoperatively on 12 eyes and intraoperatively in the remaining 16 eyes. The mean follow-up period was 35 months. RESULTS: Scleral

Primary vitrectomy without scleral buckling for rhegmatogenous retinal detachment.

BACKGROUND: Pars planta vitrectomy has evolved as an alternative method in the treatment of more complicated rhegmatogenous retinal detachments. We report a series of patients who underwent primary vitrectomy with gas tamponade without the use of additional scleral buckling. METHODS: A retrospective study of 53 patients with a follow-up of 6-45 months (mean 17.8 months) was carried out. Preoperative findings included unusual, multiple or large breaks, vitreous haemorrhage, proliferative vitreoretinopathy and bullous retinal detachment. Preoperative visual acuity was between light perception a

Scleral buckling for retinal detachment. Predictors for anatomic failure.

PURPOSE: The influence of various preoperative, intraoperative, and postoperative factors on retinal reattachment after scleral buckling was examined. METHODS: A study of 601 eyes of 577 consecutive patients who underwent conventional scleral buckling procedures was conducted. Multiple logistic regression analysis was used to determine the independent influence of each variable on anatomic failure. RESULTS: Anatomic reattachment of the retina was achieved in 86% of eyes after a single procedure, and in 90% of eyes after a second surgical procedure, with a mean follow-up period of 5.27 months

Management of Delayed Complications of Hydrogel Scleral Buckles.

(1) Background: hydrogel scleral buckles (HSB)-related complications can happen decades after implantation, although this material has been retrieved for a long time. Due to its fragile texture, ensuring the complete removal of this material and avoiding complications are challenging. Incomplete removal, iatrogenic complication, recurrent retinal detachment, and infection could occur. (2) Methods: chart review of patients who developed delayed HSB-related complications and received removal of HSB in Taipei Veterans General Hospital from 2004 to 2021. The presenting symptoms, prior diagnosis be

Modified scleral band buckle placement technique.

BACKGROUND: Scleral buckling is considered an art, which has a long learning curve compared to other surgical retina techniques. Unfortunately, this beautiful surgical technique is getting less attention as new retina surgeons don't get much exposure to this age-old surgical technique, reason being less interest shown by young retina surgeons and tremendous advancement in micro-incision vitrectomy system. This video is a modification of the present technique of placement of band buckle, which uses science rather than art.The most important aspect in band buckle is getting the optimal buckle hei

Scleral buckling for retinal separation: with and without encirclement.

Two similar patient populations with previously unoperated, atraumatic, rhegmatogenous retinal separations were surgically treated using the same technique with encircling elements in one series (290 eyes) and without encircling elements in the other (463 eyes). Nine percent of the encircled group remained detached compared with 7.6% in the other group. The reason for ultimate surgical failure in both groups was proliferative vitreoretinopathy. Encirclement offered no advantage with respect to surgical success.

Incidence of ocular motility problems following scleral buckling surgery.

The incidence and severity of extraocular muscle imbalance after conventional scleral buckling surgery was determined for 70 eyes of 68 patients with primary rhegmatogenous retinal detachment. Fifty-eight eyes had circumferential silicone explants, 10 eyes had radial sponges and 2 had both. Sixty-five per cent of eyes showed some restriction of ocular motility and 72% of patients had diplopia within their field of binocular single vision (BSV). The more extraocular muscles the explant was placed under, the more directions of gaze were likely to be restricted (p = 0.032). In 84% of eyes the res

Diplopia was not predictable and not associated with buckle position after scleral buckling surgery for retinal detachment.

PURPOSE: The possibility of postoperative binocular diplopia is seen as an important drawback of conventional scleral buckling surgery for rhegmatogenous retinal detachment. The goal of this study was to evaluate the occurrence and pattern of binocular diplopia after scleral buckle procedures in patients with rhegmatogenous retinal detachment. METHODS: In a retrospective study of 1,030 patients with primary rhegmatogenous retinal detachment who were treated by scleral buckle surgery between January 2001 and July 2008, the postoperative occurrence of binocular diplopia was retrieved from the m
