Several investigations have indicated that rupture of the thoracic aorta is one of the leading causes of immediate death in victims of road traffic accidents. In Finland in 1983, 92% of front-seat passengers were seat belt wearers on highways and 82% in build-up areas. The mechanisms of rupture of the aorta have been intensively investigated, but the relationship between seat belt wearing and injury mechanisms leading to aortic rupture is still largely unknown. Six women who sustained injury to a breast from seatbelt straps during road traffic accidents are presented. In three the breast contained a carcinoma. In view of the overlap of the appearance of fat necrosis and carcinoma by clinical and radiological examination all such lesions should be biopsied to exclude carcinoma as the history of the lesion can be dangerously misleading.

The personal experience of a general practitioner who sustained a fracture-dislocation of the atlanto-axial joint is recorded. The inefficient application of an unyielding (non-inertial) lap and diagonal seat belt permitted this injury, although one does not know what other injuries might have occurred had the belt not been worn.

Eighteen-year-old restrained male driver involved in a flip over motor vehicle accident resulting in a seatbelt injury triad ( rectus abdominis muscle disruption, injury to the sigmoid colon and infra-renal aortic dissection). The patient did well after the surgical resection of the sigmoid colon, repair of the rectus abdominis muscle and endovascular repair of the aorta. Our objective is to shed light on this potentially fatal injury.

Seat-belt fractures of the lumbar spine in adolescents and adults are well recognised but there are few reports of these injuries in young children. We reviewed all seat-belt injuries in skeletally immature patients (Risser 0), seen at a tertiary referral centre between 1974 and 1991. A 3-year prospective study of road traffic accidents in which injuries of the chest wall were accurately recorded and related to the use of seat belts was performed in Oxfordshire by a combined team of surgeons and engineers. An increased incidence of soft tissue injuries was noted in restrained front seat occupants though this was not the case for fractures of the clavicle or sternum. The overall incidence of broken ribs in front seat passengers was not affected by the wearing of belts but the incidence of those with four or more broken ribs was considerably reduced by wearing belts.

From November 1974 to June 1978 60 severely injured car-occupants wearing seatbelt were treated at the department of surgery, University of Kiel. Ten of them sustained pelvic fractures.
