A new minimally invasive technique to occlude ventricular septal defect using an occluder device.

PURPOSE: This study was to evaluate ventricular septal defect occlusion using a lower mini-sternotomy approach. DESCRIPTION: Eleven cases with ventricular septal defect underwent general anesthesia and a 3 to 4 cm lower mini-sternotomy incision was made. Using transesophageal echocardiography, the occluder was released using a mono-tubed unit. EVALUATION: All cases were occluded successfully. No patient required open heart surgery using extracorporeal circulation. There were no major complications and no evidence of residual ventricular shunt. CONCLUSIONS: Ventricular septal defect occlusio

Patent foramen ovale with complex anatomy: Comparison of two different devices (Amplatzer Septal Occluder device and Amplatzer PFO Occluder device 30/35).

BACKGROUND: Patent foramen ovale (PFO) closure after a cryptogenic cerebral ischemic event is a routinely procedure. The most used device is Amplatzer™ PFO Occluder 25 mm, but PFOs with complex anatomy require larger device for closure. We compared Amplatzer™ Septal Occluder (ASO) device versus Amplatzer™ PFO Occluder 30 or 35 mm (A-PFO 30/35) about the safety of procedure and the presence of residual shunt during the follow-up. METHODS: From June 2002 to July 2016, 355 patients (pts) with PFO undergone closure at our institution. Among these ones, 70 pts (19.7%) had a PFO with complex anatom

Migration of atrial septal occluder device to the thoracic aorta: case report.

Atrial septal defect (ASD) is a common cardiac defect with significant implications if left untreated. Although open heart surgery is the traditional approach, transcatheter closure devices, such as the Amplatzer™ Septal Occluder Device, have gained prominence due to advantages like shorter hospital stays and reduced costs. Among potential complications, device migration is a rare complication, with an incidence of 0.5 to 1.1%. We report a rare case of migration of an Amplatzer™ device in an asymptomatic patient, diagnosed 6 months after its implantation. After detecting the issue, the medical

Embolization of an Atrial Septal Defect Occluder Device Into the Left Ventricle.

Amplatzer Atrial Septal Occluder device has been routinely and successfully used as a percutaneous alternative to cardiac surgery for closure of atrial septal defects. It has shown to the safe with a low complication profile. Complications that most commonly occur with atrial septal defect (ASD) closure devices include malposition or embolization, residual shunt, atrial arrhythmias, thrombosis over the vena cava or atrium, erosion and perforation of the heart, and infective endocarditis. The most common complications associated with an ASD occluder device appear to be embolization and malposit

Transcatheter retrieval of Amplatzer Septal Occluder device embolized into the abdominal aorta.

The use of Amplatzer Septal Occluder device has become an alternative to surgical procedure in selected group of patients affected by atrial septal defect. Percutaneous closure of atrial septal defect has emerged as a low morbidity procedure but, at the same time, showed various complications associated to the device itself. Although embolization to the abdominal aorta is only sporadic reported, it could represent a potential vascular disaster and usually is treated by surgery. Herein, we report on the fourth, in English literature, successfully total transcatheter retrieval of an Amplatzer Se

Tricuspid stenosis: A rare and potential complication of ventricular septal occluder device.

Asymmetrical septal occluder device (ASOD) has made percutaneous closure of ventricular septal defect an easy and effective management option. Although there are reports of aortic and tricuspid valvular regurgitation after deployment of ASOD, only few cases of tricuspid stenosis (TS) has been reported so far in the literature. We report a case of malaligned ASOD that occurred after successful device closure resulting in TS along with mild tricuspid and aortic regurgitation requiring surgical retrieval. Transesophageal echocardiography played crucial role in detecting the cause of tricuspid val

Late Migration of Amplatzer Septal Occluder Device to the Descending Thoracic Aorta.

Percutaneous closure of atrial septal defect (ASD) has become an increasingly common procedure. Serious complications of the procedure, such as cardiac migration, are rare, and usually occur <72 hours after device placement. In this report, we present the case of a patient who underwent successful surgical treatment for the migration of an ASD occluder device to the thoracic aorta 12 months after ASD closure.

Transcatheter closure of a complex atrial septal defect after occluder device embolization.

Percutaneous closure is nowadays considered the treatment of choice of ostium secundum atrial septal defects (ASD). However, transcatheter closure can be highly challenging when we face an ASD with complex morphological features. The combination of different imaging modalities can be very helpful. This case shows the great value of using both intracardiac and real time 3D transesophageal echocardiography for the percutaneous closure of a complex iatrogenic ASD after device embolization.

Late embolization of the atrial septal occluder device into the abdominal aorta.

Although early device embolization is a well-known complication of percutaneous atrial septal defect (ASD) closure, late device embolization is rarely encountered and information about management of it are very limited. Herein, we reported a case of late ASD device embolization into the abdominal aorta at the level of the superior mesenteric artery, 8 months after percutaneous closure.

Perventricular closure of perimembranous ventricular septal defects using the concentric occluder device.

This report summarizes the authors' clinical experience with perventricular closure of the perimembranous ventricular septal defect (PmVSD) using the concentric occluder as a minimally invasive technique without cardiopulmonary bypass and following transesophageal echocardiography (TEE) guidance. Between July 2011 and March 2013, 61 patients with PmVSD underwent perventricular concentric device closure using a minimally invasive inferior sternotomy approach. The basal diameter of the PmVSD ranged from 2.5 to 7 mm. The diameter of the occlusion device waist ranged from 4 to 8 mm. A ventricular

Endoscopic Deployment of Cardiac Septal Occluder Device to Close an Enterovaginal Fistula.

Enterovaginal fistulas (EF) are a rare, pathologic connection between the small bowel and vagina. EF commonly present with abnormal passage of stool, flatus, and purulence through the vaginal orifice, leading to increased risk of severe infections, and cause extreme discomfort for patients. A 70-year-old woman with a history of urothelial carcinoma was treated surgically, resulting in a persistent symptomatic EF. Her course was complicated by multiple failed surgical attempts to fistula closure. Ultimately, she was deemed a poor surgical candidate. Gastroenterology successfully closed her fist

Occluder Device Removal and Total Septation of "Swiss-Cheese" Ventricular Septal Defects.

"Swiss-cheese" ventricular septal defects present complex treatment challenges. Despite difficult defect visualization and closure, complete septation is the treatment of choice. We present the case of a 2-year-old with residual apical "Swiss-cheese" ventricular septal defects after failed percutaneous device closure with 2 occluder devices. Surgical removal of 1 device and primary closure of the defects via right apical ventriculotomy resulted in successful complete septation.

Late embolisation of an atrial septal occluder device in the abdominal aorta.

Percutaneous closure of secundum atrial septal defect (sASD) - which is the most common in adult congenital heart disease - is considered to be the first treatment option but can involve early and late complications. We report on the late embolization of a device to the abdominal aorta, 12 months after successful percutaneous closure of sASD. A 63-year-old woman, who suffered from stomach ache, was found to have an ASD occluder device in her abdominal aorta. Although surgical intervention to remove the embolisation may be considered, medical follow-up and re-intervention of percutaneous closur

Surgical revision of an uncommonly dislocated self-expanding Amplatzer septal occluder device.

Open heart surgery is the standard procedure for closure of ostium secundum atrial septal defects. Recently, percutaneous transcatheter procedures emerged as therapeutic alternatives for closure of both atrial septal defects and patent foramen ovale. Unfortunately, however, such percutaneous procedures may require surgical intervention for early or late complications. We report a case with emergent surgery for dislocation of the Amplatzer septal occluder into the aortic arch diagnosed 30 days after percutaneous closure of an atrial septal defect.

Experimental preseeding of the STARFlex atrial septal occluder device with autologous cells.

Devices used in interventional cardiology are permanent implants. However, most of the devices fulfill only a temporary function. For example, atrial septal defect (ASD) occluders serve as mechanical shields until complete in- and overgrowth of the occluding device by endogenous tissue from the defect edges has occurred. Thereafter, the foreign body material of the devices is no longer needed and bears potential long-term adverse effects. The concept of "biodegradable" occluder devices that act as transient mechanical shields to close the defects and as scaffolds for overgrowth with autologous

Preprocedural Transthoracic Echocardiography Can Predict Amplatzer Septal Occluder Device Size for Transcatheter Atrial Septal Defect Closure.

OBJECTIVE: To evaluate whether preprocedural transthoracic echocardiography (TTE) can be used to predict Amplatzer septal occluder (ASO) size for device closure of atrial septal defect (ASD). DESIGN: Retrospective review of patients who underwent ASD device closure at our institution between August 2006 and August 2013 was performed. Patients with complex congenital heart disease, devices other than the ASO, multiple devices, or inadequate TTE images were excluded. Those who had transesophageal echocardiography (TEE)-guided device placement were evaluated. A blinded observer reviewed their pr
