Epididymitis, orchitis, and related conditions.

Epididymitis is the most common intrascrotal inflammation. Retrograde ascent of pathogens is the usual route of infection. Nonspecific bacterial epididymitis is caused by various aerobic bacteria, is frequently associated with anatomic abnormalities, and represents the most common type of epididymitis in older men. Sexually transmitted epididymitis is usually caused by Chlamydia trachomatis or Neisseria gonorrhoeae and represents the most frequent cause of acute scrotal swelling in men younger than 35 years of age. Underlying urologic abnormalities are uncommon in patients with sexually transm

Sexually transmitted diseases: incidence and distribution.

The incidence of sexually transmitted diseases (STDs) increased from 26.04 per 100000 in 1987 to 104.81 per 100000 in 1993 in selected areas of the country. Gonorrhea is by far the most common STD but its constituent ratio declined because of a rapid increase of nongonococcal uretheritis and genital warts during most recent years. The incidence of syphilis is relatively low and cases of congenital infection are noted. The wide spread of resistant Neisseria gonorrhoeae infection gives a challenge to the therapeutical and control strategies of STDs. Sexually transmitted Chlamydia trachomatis inf

Management of infants born to women with sexually transmitted diseases.

Since infants who acquire sexually transmitted diseases from their mothers are rarely symptomatic at birth, many recommendations are based on the maternal history and results of serologic screening tests. Management of an infant born to a mother with syphilis is based primarily on the mother's history of the disease and treatment. With gonorrhea, a single dose of ceftriaxone should be given to the infant if the mother is infected. Infants born to mothers with chlamydial infection are at risk of conjunctivitis and pneumonitis within the first two to 12 weeks of life. These infants should receiv

[An STI rarely comes alone-An overview of sexually transmitted infections].

Sexually transmitted infections (STI) or sexually transmitted diseases (STD) can be caused by bacteria, viruses, fungi, protozoa and arthropods. The most frequent bacterial STIs include Chlamydia infections, syphilis and gonorrhea; viral STIs include herpes genitalis (herpes simplex virus 1 and 2), human papillomavirus (HPV) and human immunodeficiency virus (HIV). An empathetic medical history taking in cases of a clinical suspicion can provide important information for adequate diagnostics. When an STI is detected, a complete screening for other STIs should be carried out. The STIs result in

Bacterial-viral interactions: a factor that facilitates transmission heterogeneities.

The transmission of infectious diseases is characterized by heterogeneities that are shaped by the host, the pathogen, and the environment. Extreme forms of these heterogeneities are called super-spreading events. Transmission heterogeneities are usually identified retrospectively, but their contribution to the dynamics of outbreaks makes the ability to predict them valuable for science, medicine, and public health. Previous studies identified several factors that facilitate super-spreading; one of them is the interaction between bacteria and viruses within a host. The heightened dispersal of

The 3 major lice that infest humans are Pediculus humanus capitis (head louse), Pthirus pubis (crab louse), and Pediculus humanus humanus (body louse). Patients with louse infestation present with scalp pruritus, excoriations, cervical lymphadenopathy, and conjunctivitis. A hypersensitivity rash, or pediculid, may mimic a viral exanthem. Head lice infestation crosses all economic and social boundaries, whereas body lice infestation preferentially affects the homeless and displaced. Body lice are major vectors of diseases such as typhus, trench fever, and relapsing fever. Pubic lice infestation

Sexually transmitted diseases in adolescents: what's new?

BACKGROUND: Sexually transmitted diseases (STDs) are an important cause of morbidity and, potentially, mortality for adolescents in the United States. Each year, 2.5 million teenagers in this country will contract an STD. An understanding of recent advances in risk factors, epidemiology, diagnosis, and treatment of these important infections is vital for clinicians providing care for adolescents. METHODS: To identify topics for review, a MEDLINE search for the years 1993 to 1997 was conducted, using the general term sexually transmitted diseases. Separate searches were performed for individua

[Management of external genital warts by dermatologists: a French survey].

BACKGROUND: External genital wart is the most frequent sexually transmitted disease. While there are guidelines for management and treatment, no data about primary care of this viral disease is available in France. So, we conducted a survey on French dermatologists'management of external genital warts. METHODS: In July and August 2001, a questionnaire - including questions on physicians and patients, sexually transmitted diseases, partners, clinical and viral evaluations, treatments, and follow-up - was mailed to 652 French dermatologists (randomization of 20 p. 100 of French dermatologists).

Sexual transmission of human T-cell lymphotropic virus type 1.

Human T-cell lymphotropic virus type 1 (HTLV-1) is endemic in many parts of the world and is primarily transmitted through sexual intercourse or from mother to child. Sexual transmission occurs more efficiently from men to women than women to men and might be enhanced by sexually transmitted diseases that cause ulcers and result in mucosal ruptures, such as syphilis, herpes simplex type 2 (HSV-2), and chancroid. Other sexually transmitted diseases might result in the recruitment of inflammatory cells and could increase the risk of HTLV-1 acquisition and transmission. Additionally, factors that

[Chlamydia, herpes, chancre, gonorrhea, syphilis. Are you familiar with the therapy?].

Sexually transmitted diseases (STDs) which today are taken to cover all genital contact infections including the classical venereal diseases, have increased appreciably in recent years. With regard to the penis, STDs may manifest as papules, pustules and ulcers including urethral discharge and swelling of regional lymph nodes. Possible causes may be bacterial, viral or fungal. Depending on the pathogens involved, local or hematogeneous dissemination may occur. Treatment of infectious penile diseases follows the recommendations contained in the guidelines of the German Society for STD.

Understanding the presence of false-positive antibodies in acute hepatitis.

Although false-positive antibodies (FPAs) have been well described in chronic hepatitis C virus (HCV), this has not been evaluated in acute viral hepatitis. Patients with acute viral hepatitis underwent antibody testing for other causes of liver disease and sexually transmitted diseases. Those with antibody positivity underwent confirmatory testing and monitoring. Patients with FPAs were compared with patients with acute hepatitis C infection without FPAs. In total 7 of 24 patients (29%) had FPAs. FPAs during acute viral hepatitis are associated with higher IgM levels and higher ESR in acute H

Human Papillomavirus Epidemiology and Prevention: Is There Still a Gender Gap?

Human papillomavirus (HPV) is sexually transmitted, one of the three most common sexually transmitted infections (STIs) in both males and females, and the most common viral STI. A crucial public health strategy to protect people against HPV is vaccination, which has shown its effectiveness in preventing HPV-related diseases. Presently, three types of vaccines are available (bivalent, quadrivalent, and nonvalent), and they all target the two most oncogenic virus genotypes (HPV 16 and 18). In recent years, the need to implement vaccination programmes that include all genders has been discussed i

Cantharidin is Superior to Trichloroacetic Acid for the Treatment of Non-mucosal Genital Warts: A Pilot Randomized Controlled Trial.

Condyloma Acuminatum is a sexually transmitted viral disease caused by the human papilloma virus (HPV). It is the most common viral sexually transmitted disease. In this randomized controlled trial, cantharidin was found to be more effective and better tolerated than trichloroacetic acid for the treatment of these lesions. Patients treated with cantharidin healed with less scarring than those treated with TCA (P<0.034), had less pain during treatment (P<0.01), and required fewer treatments to eradicate warts (P<0.01) when compared to Trichloroacetic acid.

Trends in molluscum contagiosum in the United States, 1966-1983.

We analyzed data on molluscum contagiosum infection in the United States from two sources: the National Disease and Therapeutic Index Survey of private patients, collected during 1966-1983; and two sexually transmitted disease clinics, collected during 1977-1981. Patient visits to private physicians for molluscum contagiosum increased 11-fold from 1966 to 1983. Over the same time span, the proportion of private patient visits also increased: from 1.2 to 11.0 per 100,000 total clinic visits. Clinic patients, however, showed stable trends in proportions of individuals infected over the period 19

Cervicitis: a review.

PURPOSE OF REVIEW: Sexually transmitted infections impact significantly on global health. Whereas Chlamydia, Neisseria gonorrhoea and syphilis have been extensively examined, there remains a paucity of knowledge of nonchlamydial and nongonococcal cervicitis, an arguably more prevalent but poorly characterized condition with uncertain clinical implications. With increasing application of molecular diagnostic methods for the detection of sexually transmitted infections and a growing body of literature on cervicitis, a review is timely. RECENT FINDINGS: The number of putative aetiological agents
