Social Support in Group Medical Visits for Chronic Pain in Primary Care.

PURPOSE: Social support is associated with improved outcomes for people with chronic pain and is an important but understudied component of biopsychosocial pain care. We sought to explore how group medical visits facilitate 4 distinct types of social support-emotional, appraisal, informational, and instrumental-among primary care patients with chronic pain. METHODS: We collected qualitative data within a randomized trial of integrative group medical visits for chronic pain, focusing on 2 cohorts of English-speaking patients randomized to group medical visits. Data came from 16 observed group

A Novel Approach for Patients with Risky Drinking or Tobacco Smoking and Comorbid Cardiovascular Concerns: Applying Interdisciplinary Conjoint Appointments in an Integrated Primary Care Setting.

Complex concerns, such as tobacco use and risky drinking in patients with cardiovascular disease (CVD) who report not being ready to change, may require multi-dimensional approaches to intervention. In this Notes from the Field, an interdisciplinary, integrated conjoint appointment is described wherein primary care providers (PCPs) and behavioral health providers (BHPs) meet together briefly with the patient, bringing varying expertise in an effort to increase readiness to change and cessation of problem behaviors such as smoking. First, the protocol for this appointment, which is part of an i

Implementing group visits for opioid use disorder: A case series.

Background: Group-based models of Office-Based Opioid Treatment with buprenorphine-naloxone (B/N) are increasingly being implemented in clinical practice to increase access to care and provide additional therapeutic benefits. While previous studies reported these Group-Based Opioid Treatment (GBOT) models are feasible for providers and acceptable to patients, there has been no literature to help providers with the more practical aspects of how to create and maintain GBOT in different outpatient settings. Case series: We present 4 cases of GBOT implementation across a large academic health care

Paediatric-Pharmacy Shared Care Improves Access and Efficiency in Developmental Paediatrics.

BACKGROUND: Rising prevalence of neurodevelopmental disorders, including attention-deficit/hyperactivity disorder (ADHD) and autism spectrum disorder (ASD), has increased demand for developmental paediatric services. Ongoing medication review requirements place increasing pressure on paediatrician-led models. Pharmacist-led shared care may improve capacity and access; however, evidence in paediatric outpatient settings remains limited. AIM: To evaluate the impact of a paediatric-pharmacy shared-care model on wait times, service capacity and efficiency compared with medical-only care. METHODS

Group clinic for chemoprevention of squamous cell carcinoma: a pilot study.

Topical 5-fluorouracil (5-FU) is a valuable treatment of actinic keratosis (AK), but its use is limited by bothersome side effects. To evaluate patient satisfaction with a regimen of 5-FU for AK in group clinics, we offered participation in shared medical appointments (SMAs) to dermatology clinic patients diagnosed with AK at the Providence VA Medical Center in Rhode Island. Approximately 3 to 4 patients attended each pair of sessions spaced 2 weeks apart. At each visit, photographs and feedback were obtained; at the second visit, clinicians graded the patients' reactions to 5-FU according to

Usability and Feasibility of the Antipsychotic Medication Decision Aid in a Community Program for First-Episode Psychosis.

OBJECTIVE: Although antipsychotic medications are considered first-line treatment for psychosis, rates of discontinuation and nonadherence are high, and debate persists about their use. This pilot study aimed to explore the usability, feasibility, and potential impact of a shared decision making (SDM) intervention, the Antipsychotic Medication Decision Aid (APM-DA), for decisions about use of antipsychotic medications. METHODS: A pilot randomized controlled trial was conducted with 17 participants in a first-episode psychosis program. Nine participants received the APM-DA, and eight received

Impact of digital versus faxed e-referrals on triage decisions in a medical retina clinic.

CLINICAL RELEVANCE: Efficient triage is essential for managing increasing demand in resource-constrained ophthalmic services. Referral systems that enable structured clinical information and high-quality ocular imaging may support prioritisation according to clinical need and improve service efficiency. BACKGROUND: This study compared triage decisions made using traditional faxed referrals to those made using digital referrals with high-resolution retinal imaging in an Australian public hospital medical retina outpatient clinic. METHODS: Two ophthalmologists independently reviewed referrals

Digital Informed Consent: Modernising Information Sharing in Surgery to Empower Patients.

BACKGROUND: Despite the 2015 Montgomery Ruling highlighting key requisites for informed consent, little has changed to modernise data-sharing and documentation of the consent process. It can be difficult to gauge patient understanding and address all patient concerns in time-limited appointments. We aimed to assess the feasibility of a digital information-sharing platform to support a move towards a digital informed consent process. METHODS: All adult patients referred to a single centre with symptomatic gallstones were invited to use a digital information-sharing platform to support the info

Medication initiation and optimisation in attention deficit hyperactivity disorder: A practical approach for general practice.

BACKGROUND: Attention deficit hyperactivity disorder (ADHD) is increasingly managed within general practice in Australia, supported by evolving shared-care models and expanded general practitioner (GP) involvement. OBJECTIVE: Medication plays an important part in improving day-to-day functioning in patients significantly affected by ADHD. It should be used in conjunction with robust and evidence-based psychosocial supports and psychoeducation. This article aims to provide a practical guide for GPs in Australia seeking to initiate stimulant or non-stimulant medication. DISCUSSION: Medication

A qualitative exploration of barriers to efficient and effective structured medication reviews in primary care: Findings from the DynAIRx study.

INTRODUCTION: Structured medication reviews (SMRs), introduced in the United Kingdom (UK) in 2020, aim to enhance shared decision-making in medication optimisation, particularly for patients with multimorbidity and polypharmacy. Despite its potential, there is limited empirical evidence on the implementation of SMRs, and the challenges faced in the process. This study is part of a larger DynAIRx (Artificial Intelligence for dynamic prescribing optimisation and care integration in multimorbidity) project which aims to introduce Artificial Intelligence (AI) to SMRs and develop machine learning m

Barriers to Hepatitis C Treatment and Interest in Telemedicine-Based Care Among Clients of a Syringe Access Program.

BACKGROUND: Sharing equipment for injection drug use is the most common mode of hepatitis C virus (HCV) transmission in the United States, yet people who inject drugs (PWID) historically have low rates of HCV treatment. New strategies are needed to expand access to HCV treatment among PWID. Co-locating HCV treatment at syringe access programs (SAPs) reduces barriers to treatment, and telemedicine-based treatment programs could expand access further. METHODS: To evaluate interest in a co-localized or telemedicine-based program at an SAP in Denver, Colorado, we surveyed 171 SAP clients to under

Educating patients at discharge: Perceived responsibilities among nurses and residents at an academic medical center.

OBJECTIVES: Safe discharge requires clear coordination across multiple interprofessional team members. Discharge education roles are often unclear; our study sought to identify the primary person responsible for patient discharge education and compare the perceptions of nurses and residents regarding the patient discharge education process. METHODS: We conducted a cross-sectional survey of internal medicine residents and staff nurses at an urban academic medical center. We asked: (1) who was primarily responsible, and (2) who was best suited for patient education (nurse, intern, resident, or

Fibromyalgia, Sjogren's & depression: linked?

Health care has become increasingly fragmented, partly due to advancing medical technology. Patients are often managed by various specialty teams when presenting with symptoms that could be manifestations of different diseases. Approximately one third of them are referred to specialists, at over half for outpatient appointments. Fatigue, pain, depression, dry mouth, headaches, and arthralgia are common complaints and frequently require referral to specialist physicians. Differential diagnoses include fibromyalgia (FM), Sjogren's syndrome (SS), and depression. Evaluations involve various sub-sp

Multivariate analysis of covariates of adherence among HIV-positive mothers with low viral suppression.

BACKGROUND: As part of efforts to improve the prevention of mother-to-child transmission in Northern Uganda, we explored reasons for poor viral suppression among 122 pregnant and lactating women who were in care, received viral load tests, but had not achieved viral suppression and had more than 1000 copies/mL. Understanding the patient factors associated with low viral suppression was of interest to the Ministry of Health to guide the development of tools and interventions to achieve viral suppression for pregnant and lactating women newly initiating on ART as well as those on ART with unsupp

Effect of interventions for non-emergent medical transportation: a systematic review and meta-analysis.

INTRODUCTION: Transportation is an important social determinant of health. We conducted a systematic review of the associations on health and health care utilization of interventions aimed at reducing barriers to non-emergency transportation and non-medical transportation. METHODS: We searched three databases and the gray literature through mid-January 2022. Included studies needed to assess an intervention targeted at non-emergency or non-medical transportation barriers, report missed (or kept) visits, health care utilization, costs, or health outcomes. Data extraction was performed in dupli

The Top Ten Research Priorities for Outpatient Service Delivery: A South West London Outpatient Transformation Group and James Lind Alliance Priority Setting Partnership.

BACKGROUND: Outpatient appointments account for 85% of hospital activity and are fundamental to the running of the National Health Service (NHS). The COVID-19 pandemic and government initiatives have prompted outpatient services transformation. We aimed to determine research priorities for outpatient transformation, which represent the shared priorities of patients, their families, carers and healthcare professionals. METHODS: We established a national (UK) priority setting partnership (PSP) with the James Lind Alliance (JLA) to capture the top 10 research priorities regarding outpatient serv

Decision making in chronic glaucoma--optometrists vs ophthalmologists in a shared care service.

PURPOSE: Shared care schemes have become popular in glaucoma management in the UK over the past 20 years. Published audits of decision making processes by non-ophthalmologists working in such schemes are however rare. Our aim was to audit the appropriateness of optometrists' decision-making when working in an in-house shared care glaucoma scheme. METHODS: A single masked comparison study utilising retrospective consensus decisions by two ophthalmologists compared with 'live' optometrist decisions from three optometrists who reviewed patients with glaucoma, suspect glaucoma and ocular hyperten

Mobile health treatment support intervention for HIV and tuberculosis in Mozambique: Perspectives of patients and healthcare workers.

BACKGROUND: Studies have been conducted in developing countries using SMS to communicate with patients to reduce the number of missed appointments and improve retention in treatment, however; very few have been scaled up. One possible reason for this could be that patients or staff are dissatisfied with the method in some way. This paper reports a study of patients' and healthcare workers' (HCW) views on an mHealth intervention aiming to support retention in antiretroviral therapy (ART) and tuberculosis (TB) treatment in Mozambique. METHODS: The study was conducted at five healthcare centres
