Necrotizing sialometaplasia: literature review and presentation of five cases.

Necrotizing sialometaplasia is a benign, ulcerative, inflammatory process of the minor salivary glands, primarily found in the hard palate. It is often mistaken clinically and histologically for a malignant tumor (that is, mucoepidermoid or squamous cell carcinoma). The lesion heals spontaneously in six to 12 weeks. It occurs primarily in persons between the ages of 40 and 60 and has been reported more frequently in men than women. In necrotizing sialometaplasia the squamous metaplasia and tissue necrosis is confined to the existing ductal and lobular pattern of the salivary glands, a unique c

Necrotizing sialometaplasia of the lip simulating squamous cell carcinoma.

A case of necrotizing sialometaplasia of the lip in an 68-year-old pipe smoker is described. Necrotizing sialometaplasia is a self-healing non-neoplastic disease probably of ischaemic nature. Thirty-nine cases of sialometaplasia are described in the literature up to early 1979. These cases appeared in the palate, nasal cavity, gingiva, lip, hypopharynx and maxillary sinus. Six cases have also been reported from major salivary glands. Histologically there is necrosis of mucous cells with partial replacement by squamous epithelium. This entity has often been mistaken for squamous or mucoepidermo

Hypoxia Contributes to the Early-Stage Progression of Necrotizing Sialometaplasia.

Necrotizing sialometaplasia (NSM) is a nonneoplastic lesion listed in the World Health Organization Classification of Tumours-Head and Neck Tumours. In early NSM lesion, there is infarction and necrosis of the acinar cells, and squamous metaplasia of the salivary ducts occurs as the lesion matures. Differentiation from squamous cell carcinoma and other malignancies is sometimes required clinically and histopathologically. Local hypoxia caused by trauma and vascular compromise is a proposed etiology of NSM. However, the mechanisms underlying the pathogenesis are unclear. This study focused on t

Necrotizing sialometaplasia: A report of two cases and review of the literature.

Necrotizing sialometaplasia (NS) affects salivary glands, and despite being a benign condition, its clinical and histopathological features sometimes mimic other malignant pathologies of epithelial origin. This article presents two cases of NS and discusses clinicopathological features and the differential diagnosis of this condition. The first case, a 76-year-old woman with a 6-month history of painful oral thrush. Intraoral examination showed an ulcerative lesion located on the hard palate. The clinical hypothesis was squamous cell carcinoma. Second, a 26-year-old man with a 40-days ulcerati

Necrotizing sialometaplasia: review of the literature and report of nonulcerative case.

A review of the literature and a report of a case of necrotizing sialometaplasia of the hard palate is presented. Because necrotizing sialometaplasia may be misdiagnosed as squamous cell carcinoma or mucoepidermoid carcinoma, it is important that the clinician and pathologist take the necessary precautions to prevent any further mutilative surgery for this benign disease entity.

Non-ulcerated necrotizing sialometaplasia may mimic a salivary gland tumor.

Necrotizing sialometaplasia (NS) is a benign, self-limiting inflammatory entity that mainly affects the minor salivary glands located in the hard palate. Classically, NS is characterized as a nodule that evolves to a central ulcer. The most widely recognized triggering factor is an ischemic event. The diagnosis becomes a challenge in non-ulcerated NS cases which is essential to rule out the possibility of salivary gland tumors, especially the malignant ones. Here, we presented a case of a 32-year-old male patient with a 1-month complaint of a painful, slightly elevated erythematous area on the

[Pathohistological findings in infarcted parotid glands (necrotizing sialometaplasia) (author's transl)].

In 1973 Abrams et al. described a nonneoplastic lesion named "necrotizing sialometaplasia". Since this date additional cases have been reported. The lesion might be misdiagnosed either mucoid tumor or squamous carcinoma. The cases reported in the following years were localised on the hard palate, lower lip, nose and in the major salivary glands. By the "Speicheldrüsenregister" of the Institute of Pathology of the University of Hamburg five cases of infarcted parotid glands were registered during the years of 1965--1977. All infarcts are observed in patients with tumors in the parotid region. N

Necrotizing squamous metaplasia in herpetic tracheitis following prolonged intubation: a lesion similar to necrotizing sialometaplasia.

In the last year we have encountered three cases of clinically inapparent herpes simplex virus (HSV) tracheitis in biopsies from patients undergoing tracheostomy following prolonged intubation. The diagnosis was based on finding characteristic inclusion-bodies on haematoxylin and eosin stained sections and was confirmed by immunohistochemical staining of infected cells with antibodies to HSV. A prominent mononuclear infiltrate, originally described in HSV oesophagitis, was recognized in two cases. Diffuse squamous metaplasia of both surface and glandular epithelium adjacent to necrotic ulcerat

Palatal Nodule in Bulimia Patient: An Early Sign of Necrotizing Sialometaplasia.

Necrotizing sialometaplasia (NS) is a rare inflammatory condition. Despite rare, chronic self-induced vomiting in patients with eating disorders has been reported as a risk factor. This study aims to report a case of NS in a patient with bulimia nervosa and review the literature. A 25-year-old woman presented with a crateriform painful ulcer, affecting the palatal mucosa, measuring 3 cm in its largest extension. The main diagnosis hypothesis was NS. On the third visit, the patient mentioned suffering bulimia with episodes of self-induced vomiting. Consequently, the diagnosis was NS associated

Atypical Presentation of Necrotizing Sialometaplasia of the Hard Palate: A Diagnostic Dilemma.

Necrotizing sialometaplasia refers to a benign, uncommon, and self-limiting inflammatory reaction concerning the salivary gland tissue, which both clinically and histologically may be easily mistaken for mucoepidermoid carcinoma or squamous cell carcinoma. This may cause irrelevant surgical intervention. Minor salivary glands are the most commonly affected salivary gland, with the hard palate being the most usual site. However, it can involve the other areas in which salivary gland tissue is present in the other oral subsites and pharyngeal areas. Due to the lack of knowledge about this entity

Necrotizing sialometaplasia of the parotid gland associated with facial nerve paralysis.

INTRODUCTION: Necrotizing sialometaplasia is a benign inflammatory lesion involving most frequently the minor salivary gland of the hard palate. Involvement of the parotid gland is rare, involvement of the parotid gland associated with facial palsy is exceptional. CASE REPORT: A 56-year-old male patient with Marfan syndrome presented with swelling and inflammation of the left parotid gland associated with progressively complete facial nerve paralysis. CT scan and MRI showed a parotid collection with hyper signal of the nearest tissues associated with erosion of the styloid process. A malignan

Necrotizing sialometaplasia in a patient who is HIV positive: a case report.

Necrotizing sialometaplasia (NS) is a self-limiting, benign, inflammatory disease of the minor salivary glands of the hard palate. The main significance of the NS lesion lies in the fact that it may be mistaken for mucoepidermoid or squamous cell carcinoma. A case report is presented of a patient with NS who was HIV positive; the lesion was located in the minor salivary glands of the hard palate. Clinically, the lesion presented as a deep ulcer with slightly elevated irregular borders and a necrotic base in the hard palate. Histologically, the tissue was characterized by squamous metaplasia of

Necrotizing sialometaplasia accompanied by adenoid cystic carcinoma on the soft palate.

Necrotizing Sialometaplasia (NS) is a benign, self-limiting inflammatory disease of the mucus-secreting glands, and this illness mainly involves the minor salivary glands. The significance of NS resides in its clinical and histopathological resemblance to malignancy. We present here a case of necrotizing sialometaplasia on the soft palate, and this was accompanied by adenoid cystic carcinoma. We report here on this case to draw attention to the difficulty for deciding the extent of resecting a malignancy, and especially when the malignancy is simultaneously accompanied by necrotizing sialometa

[Necrotizing sialometaplasia--a specific differential diagnosis of an ulcer of the hard palate].

BACKGROUND: Ulcers of the hard palate are mostly caused by malignancies. In addition, rare diseases should be included in the differential diagnosis. Beside specific inflammations (lues, tuberculosis) necrotizing sialometaplasia belongs to these uncommon medical conditions. PATIENTS: A 51-year-old male was admitted to the hospital with an algetic ulcer of the hard palate. Malignancy was ruled out by pathohistological examination. Inflammation or infection was excluded by serological examination. A second biopsy was investigated by the pathologist suspecting necrotizing sialometaplasia and con

Necrotizing sialometaplasia: a sheep in wolf's clothing. Healing as a diagnostic test.

Necrotizing sialometaplasia is a benign self-limited intraoral lesion that is easily confused both clinically and histologically with squamous cell carcinoma. It presents as a painless ulceration, frequently on the hard palate, that histologically shows necrosis, inflammation, squamous metaplasia, and granulation tissue. It is thought to be due to infarction of minor salivary glands and heals spontaneously in six to 12 weeks. A brief period of observation for evidence of healing can be an important diagnostic clue in distinguishing this entity from cancer, thus saving the patient unnecessary s

Paroxetine-induced black hairy tongue and necrotizing sialometaplasia: a case report.

BACKGROUND: Black hairy tongue, also known as lingua villosa nigra, is a reversible benign disease on the dorsum of the tongue characterized by elongated and hypertrophied filiform lingual papillae. Necrotizing sialometaplasia; on the other hand, is a benign reactive inflammatory disorder involving a minor salivary gland. The pathophysiology driving black hairy tongue and necrotizing sialometaplasia remains unclear; however, common predisposing factors for both conditions include smoking, alcohol, and drugs, and in this case, paroxetine, a selective serotonin reuptake inhibitor. Multifocal Necrotizing Sialometaplasia in the Tongue Surgical Specimen: An Immunohistochemical Study.

BACKGROUND: Necrotizing sialometaplasia (NSM) is a rare self-limiting inflammatory lesion, with most cases affecting the minor salivary glands, especially those located in the palate (70%). To date, about 261 cases of NSM have been reported. Of them, 7 cases affected the tongue; 37 cases occurred after surgery; and 17 cases showed association with neoplasms. METHODS: A 50-year-old male patient was diagnosed with squamous cell carcinoma (SCC) of the tongue. After surgical excision, the tumor diagnosis was well-differentiated SCC with a close surgical margin. Re-excision of the margin was indic
