A 66 year old woman with a 9 year history of chronic lymphocytic leukaemia developed intermittent rectal bleeding for 9 months; sigmoidoscopic biopsy proved that this was due to large bowel infiltration by leukaemia. This is a very rare occurrence.

A study of 37 patients with sigmoid volvulus is presented. Subsequent bouts of recurrent volvulus are very common after sigmoidoscopic intubation, and overall mortality rates are high. Surgical resection is the preferred method of treatment.

The aim of this prospective study was to assess patient comfort during nonsedated screening sigmoidoscopy with the use of a standard 60-cm sigmoidoscope compared with a thinner 100-cm upper endoscope. The role of sigmoidoscopic examination in the diagnosis, evaluation, and treatment of minor lower gastrointestinal bleeding was investigated. One hundred ten consecutive patients undergoing 35-cm flexible sigmoidoscopy were enrolled in a study to compare the efficacy of one vs two enemas (Fleet's) in achieving adequate bowel preparation. We report a case of necrotizing amebic colitis with the sigmoidoscopic finding of a confluent, grayish pseudomembrane. The patient was referred from a chronic mental institution, which represents the patient population most susceptible to fulminant amebic dysentery in the United States. The pseudomembrane was a reflection of extensive mucosal necrosis and resolved with amebicidal therapy.

During the 40 years from 1945 to 1984, 159 occurrences of sigmoid volvulus in 140 patients were diagnosed and managed. The 130-cm colonoscope was utilized to determine whether a deeper insertion could be accomplished after the usual enema preparation for routine flexible sigmoidoscopy and, if so, to what extent that would enhance the yield of neoplastic findings. An enema that contained a complex of bismuth citrate and polyacrylate was compared with 5-aminosalicylic acid (5-ASA) enemas for treatment of distal ulcerative colitis. The multicentre trial involving 63 patients was randomised and double blind with enemas given over four weeks; clinical, sigmoidoscopic, and histological assessments were made.
