Single Umbilical Artery Umbilical Cord Is Associated With High-Grade Distal Fetal Vascular Malperfusion.

PURPOSE AND CONTEXT: Umbilical cord abnormalities with clinical signs of cord compromise are frequently associated with fetal vascular malperfusion (FVM). Single umbilical artery (SUA) has been reported to be associated with high-grade FVM in fetal growth restriction but not in an unselected population; our study aimed to address this issue. METHODS: Clinical and placental phenotypes of 55 consecutive placentas with SUA (Group 1) were compared with those of 655 placentas with 3-vessel umbilical cord (Group 2) from patients who were in the second half of their pregnancy. The placentas were his

Antenatal detection of a single umbilical artery: does it matter?

The presence of a single umbilical artery is recognised as a soft marker for congenital anomalies, aneuploidy, earlier delivery and low birthweight. Most of the available data are derived from case series or highly selected populations and are therefore likely to be unrepresentative. In this retrospective case-comparison study, we firstly aimed to determine the incidence of a single umbilical artery in an unselected population and secondly to examine the clinical significance of this soft marker. Over a 40-month period, 107 cases were identified from a cohort of 35 066 births giving an inciden

Single umbilical artery and other congenital defects in a calf.

Single umbilical artery, a common anomaly of humans is often accompanied by caudal and other vascular defects. A literature search failed to reveal any cases reported in animals. Dissection of a neonatal Holstein bull calf disclosed the following congenital defects: single umbilical artery with anomalous origin, abnormal abdominal aorta, azygos continuation of the caudal vena cava, rectal atresia, rectourethral fistula, abnormally shaped urinary bladder, unilateral testicular agenesis, undescended testis, unilateral renal agenesis, bifid ureter, and hydroureter.

The importance of placental examination in newborns with single umbilical artery.

Examination of placentas from newborns with single umbilical artery (SUA) can furnish valuable data to Perinatologists. Thus, 33.3% of the SUA newborns with gross macroscopic alterations of the placenta were malformed. In the other hand, 75% of the malformed SUA newborns had gross placental alterations. It is pointed out the importance of examination of the placenta, even at unaided eye, as a tool to prevent the Perinatologist in search of probable malformations (both apparent or occult).

The association of single umbilical artery with cytogenetically abnormal pregnancies.

The clinical significance of the absence of one of the two umbilical arteries (single umbilical artery) lies in its association with congenital malformations. Whether this association includes cytogenetic abnormalities is less clear. A retrospective review of all detected chromosomally abnormal pregnancies at the University of Maryland was carried out. Of 109 cytogenetically abnormal pregnancies, the number of umbilical cord vessels could be documented in 53 cases. Six (11.3%) had a single umbilical artery. A single umbilical artery was noted in two of nine fetuses (22.2%) with trisomy 18 and

Isolated single umbilical artery--the case for routine renal screening.

To determine the incidence of silent renal anomalies in infants with isolated single umbilical artery (SUA), all infants with SUA and without other obvious congenital anomalies, identified over a six year period, were screened using renal ultrasonography. Over 35,000 placentas were examined. An isolated single umbilical artery was identified in 112 (0.32%). Nineteen infants had abnormal renal imaging. Eight of 112 (7.1%) had significant persisting abnormalities. Vesicoureteric reflux was found in five infants (4.5%). It is recommended that renal ultrasonography be performed for all infants wit

Middle cerebral artery blood flow velocimetry among healthy fetuses with a single umbilical artery.

OBJECTIVE: A single umbilical artery (SUA) is an independent risk factor for perinatal morbidity and mortality in healthy fetuses. The aims of the study were (1) to determine middle cerebral artery (MCA) blood flow velocimetric values among fetuses without structural or chromosomal anomalies with an SUA and to compare them with nomograms of patients with a 3-vessel cord and (2) to measure the pulsatility index (PI) of the umbilical artery among these patients. METHODS: The PI values of the MCA and umbilical arteries were determined prospectively among 98 healthy fetuses with an SUA. The PI va

Changes in Artery Diameters and Fetal Growth in Cases of Isolated Single Umbilical Artery.

Background-There are conflicting data in the international literature on the risks of abnormal fetal growth in fetuses presenting an isolated single umbilical artery (SUA), and the pathophysiology of this complication is poorly understood. Objective-To evaluate if changes in diameter of the remaining umbilical artery in fetuses presenting an isolated SUA are associated with different fetal growth patterns. Study design-This was a two-center prospective longitudinal observational study including 164 fetuses diagnosed with a SUA at the 20-22-week detailed ultrasound examination and 200 control f

Isolated single umbilical artery: evaluating the risk of adverse pregnancy outcome.

OBJECTIVE: To evaluate if isolated single umbilical artery (SUA) diagnosed on second-trimester ultrasound has an independent risk association with adverse pregnancy outcomes. STUDY DESIGN: We compared 136 singleton pregnancies with isolated SUA with 500 consecutive singleton pregnancies with a three-vessel cord (3 VC). Pregnancies complicated by chromosomal abnormalities and other congenital malformations were excluded. The rates of intrauterine growth restriction (IUGR) defined as birth weight less than the 3rd percentile, small for gestational age (SGA) fetuses, defined as a birth weight lo

Single umbilical artery: fetal and placental histopathological analysis of 24 cases.

AIM: 24 cases of single umbilical artery (SUA) are presented and the related histopathological findings of the fetuses and placentas examined. SUA is the most common congenital anomaly of the umbilical cord, resulting in the absence of one of the two umbilical arteries. It has an incidence of approximately 2.1% in autopsy material and there is evidence that is associated with anomalies of the fetus and placenta. MATERIAL-METHOD: The files were reviewed of 24 cases with SUA, out of 1,570 autopsies of fetuses and placentas performed in the Pathology Laboratory of Aretaieion Hospital, due to spo

Is single umbilical artery an independent risk factor for perinatal mortality?

OBJECTIVE: To evaluate perinatal outcome of fetuses with isolated single umbilical artery (SUA), and specifically to examine whether an isolated SUA is an independent risk factor for perinatal mortality. METHODS: A population-based study was conducted, comparing pregnancies of women with and without SUA. Deliveries occurred between the years 1988-2006, in a tertiary medical center. Multiple gestations, chromosomal abnormalities and malformations were excluded from the analysis. Stratified analysis was performed using multiple logistic regression models to evaluate the association between SUA

Perinatal results following the prenatal ultrasound diagnosis of single umbilical artery.

OBJECTIVE: To study the perinatal results in our population, following the prenatal ultrasound diagnosis of a single umbilical artery (SUA), as this alteration is associated with fetal malformations, chromosomal abnormality, and poor perinatal results. MATERIALS AND METHODS: A retrospective review of all obstetric ultrasounds carried out between October 2000 and December 2003 in our service, obtaining the postnatal results of the fetuses diagnosed with an SUA. RESULTS: From a total of 5,987 pregnant patients examined by ultrasound scan at 20th week, an SUA was found in 40 cases, representing

Single umbilical artery and associated hydronephrosis: a report of 2 cases.

BACKGROUND: A single umbilical artery (SUA) in a fetus may be associated with hydronephrosis. CASES: We describe 2 cases of SUA, one of which had severe hydronephrosis with ascites and was stillborn. The other case was born alive with a unilateral hydroureter and is being managed with close monitoring. CONCLUSION: Detection of SUA on antenatal ultrasound should prompt a careful search for urinary tract abnormalities, as associated severe hydronephrosis may lead to a poor outcome.

Single umbilical artery stenosis associated with intrauterine fetal death post-transfusion.

Single umbilical artery is among the most common funicular vascular anomalies. In contrast, umbilical artery stenosis is rare, and has only been reported in three-vessel cords. We describe a case of single umbilical artery stenosis in a fetus with no associated malformations. Intrauterine fetal death occurred at 28 weeks' gestation following cordocentesis and intravascular transfusion for Rhesus alloimmunization. Single umbilical artery stenosis may place the fetus at increased risk, particularly in cases requiring interventions involving cord manipulation.

Association of single umbilical artery with congenital malformations of vascular etiology.

The possible association of a single umbilical artery (SUA) with malformations of vascular etiology is investigated in this study. Four hundred twelve fetal and embryonic autopsies showing one or more congenital malformations, collected over 7 years, were reviewed. Microscopic confirmation of a SUA was evident in 20 cases (4.85%). The two subgroups with 2 (nA = 20) or 3 umbilical vessels (nB = 392) were compared with each other, in relation to the frequency of malformations per organ system. In the group of fetuses with congenital malformations, no association was observed between SUA and the
