[Chondrosarcomas of skull base treatment].

BACKGROUND AND PURPOSE: Skull base chondrosarcomas are rare. Gross total removal is the treatment of choice, but can be difficult depending on the closeness of noble structures. Proton beam therapy can be associated in most cases. METHODS: Retrospective study of five cases treated in 13 years and study of the literature. RESULTS: Median age of patients was 34 years [28-46]. Cranial nerve palsy was the common clinical presentation. Tumor location was variable but always off midline. Treatment was surgical in all patients with a maximal resection and proton beam therapy associated for two case

Neuronavigation in skull base tumors.

OBJECTIVE: Computer-assisted neuronavigation was used in 87 cases of skull base lesions (SBLs). Preoperative planning and intraoperative identification of anatomic landmarks is especially important in SBLs since it helps to avoid or minimize surgical morbidity and mortality. In this study, we assessed the accuracy and the clinical usefulness of a frameless system based on the optical digitizer in SBLs. PATIENTS AND METHODS: Between April 2000 and March 2003, eighty-seven patients with SBLs were operated on in our department using cranial neuronavigation. A passive-marker-based neuronavigation

Craniofacial and skull base trauma.

BACKGROUND: Traumatic craniofacial and skull base injuries require a multidisciplinary team approach. Trauma physicians must evaluate carefully, triage properly, and maintain a high index of suspicion to improve survival and enhance functional recovery. Frequently, craniofacial and skull base injuries are overlooked while treating more life-threatening injuries. Unnoticed complex craniofacial and skull base fractures, cerebrospinal fluid fistulae, and cranial nerve injuries can result in blindness, diplopia, deafness, facial paralysis, or meningitis. Early recognition of specific craniofacial

Pediatric neurotologic skull base surgery.

The objectives of neurotologic skull base surgery are complete resection of the lesion and high-grade function following surgery. There is a perception that these goals are more difficult to achieve in children than in adults. Skull base disease in children and adolescents is rare. Of the 292 skull base tumors treated from 1970 to 1995 by The Otology Group in Nashville, 15 were in patients 21 years of age or younger, with only 5 patients under 10 years old. In this retrospective study, the authors review these 15 cases and compare them to their adult series. The pathology encountered in the 15

The skull base in plagiocephaly.

Between 1975 and 1992, 426 children with craniofacial malformations were treated in the Department of Pediatric Neurosurgery at the Hôpital des Enfants de la Timone in Marseille. Plagiocephaly was present in 71 (16.6%). The authors present a reproducible analysis of the skull base in plagiocephaly based on these 71 patients. A control group of Mediterranean children (n = 20) was used for comparison. Clinical anthropometric patterns were analyzed in all cases. Comparison with the control group showed a difference only in the nasion-lambda distance. Data obtained from clinical anthropometry were

Neuroradiology of the skull base.

Newer imaging modalities, especially high resolution computed tomographic scanning, have radically altered and expanded the role of radiology in the evaluation and management of lesions affecting the skull base. The location, extent, and tissue characteristics of these lesions can be determined with greater precision than ever before. When necessary, diagnostic and therapeutic angiography can be performed more rapidly and more safely with digital subtraction techniques. In the near future magnetic resonance imaging may prove to be an even more precise imaging tool than computed tomography. Mag

Reconstruction of the skull base.

Tumor surgery of the skull base has been increasingly undertaken in recent years. This report involves a series of 18 consecutive patients who have undergone 22 operations at Ohio State University from July 1980 to January 1983 for removal of tumors either adjacent to or through the skull base. A variety of reconstructive techniques such as pericranial flaps, muscle flaps, temporalis fascial grafts, split-rib grafts, fat grafts, and bone grafts have been found to be reliable. This series also included the successful use of a temporalis myocutaneous flap. The conclusion of this study is that th

Surgery of the skull base.

Tumors involving the base of the skull are often occult and may become quite large before detection is possible. Symptomology varies depending upon the nature of the tumor and its placement. These neoplasms have frequently been considered inoperable simply because of their location, but in recent years microsurgical technique and high speed air drills have allowed the modern temporal bone surgeon to gain improved access to the skull base. A wide variety of surgical approaches to tumors in this area have been described in the past 20 years: translabyrinthine, middle fossa, transcochlear, retrol

Neurosurgery: Skull Base Craniofacial Trauma.

Much of craniofacial trauma involves the frontal sinuses. Because of its response to injury, the frontal sinus mucosa has an innate ability to develop mucoceles, and if infected, mucopyocoeles. This article presents a therapeutic algorithm for all forms of craniofacial trauma with concentration on the most severe injury-the through and through fracture and its surgical remediation.

Complications of skull base reconstruction.

The skull base is an anatomically challenging location that is unique in that it requires three-dimensional considerations when formulating a reconstruction plan. Various methods have been described for repair including open, endoscopic and endoscopic-assisted approaches. Each of these can have complications that can arise either directly from access to the lesion or defect, or indirectly arising from patient characteristics, anesthetic considerations, as well as postoperative care. It is of utmost importance for the surgeon to select the appropriate technique and surgical access depending on

Biomaterials in skull base surgery.

Reconstruction materials and techniques for the base of the skull have undergone rapid developments and differentiation in recent years. While mostly autotransplants, collagens or resorbable alloplastic materials are preferred for duraplasties, pronounced organ-specific differences can be observed in the reconstruction of hard tissues. The use of polymethylmethacryl bone cement, once wide-spread, has decreased greatly due to the release of toxic monomers. Bony autotransplants are still used primarily for smaller skull-base defects, intraoperatively formable titanium nets may be also used for l

Skull base growth in craniosynostosis.

INTRODUCTION: The interrelationship of skull base growth and craniosynostosis is one that is not often taken into account in the clinical management of patients with craniofacial disorders by neurosurgeons and craniofacial surgeons. Very early on in our medical training we are taught the anatomical differences between the calvarial unit and skull base portion of the skull. The inherent differences in both underlying tissue components and the different growth characteristics of these two uniquely different structures are critical in understanding skull base growth in craniosynostosis and the in

[Cementoma of the skull base].

Cementomas result from the odontotic epithelial-mesenchymal complex and are located mainly in the area of large and small molar teeth of the mandible. There was an extremely rare cement location in the analyzed clinical cases. In the first case, cementoma of the frontal sinus spread into the ethmoidal sinus and eye socket in a female patient aged 38 years. The tumor is manifested by right frontalorbital pain and exophthalmos. There were no problems in removing the tumor and repairing the defect of the skull base. The second case was an extensive tumor in a 11-year-old child, which involves the
