Sleep: Architecture, Deprivation, and Propofol-Induced Sleep.

The importance of sleep and its role in maintaining health and well-being has garnered much attention in recent years. A large percentage of people suffer from sleep deprivation and chronic insomnia. These derangements can lead to a wide range of adverse health effects. Therefore, the crucial nature of sleep health cannot be overstated. Normal sleep architecture along with adequate sleep hygiene promotes homeostasis by way of positive feedback, which in turn, maintains many crucial physiologic functions. Given their education and area of expertise, certified registered nurse anesthetists shoul

Opioids, sleep architecture and sleep-disordered breathing.

Opioid use whether acute or chronic, illicit or therapeutic is prevalent in Western societies. Opioid receptors are located in the same nuclei that are active in sleep regulation and opioid peptides are suggested to be involved in the induction and maintenance of the sleep state. mu-Opioids are the most commonly used opioids and are recognized respiratory depressants that cause abnormal awake ventilatory responses to hypercapnia and hypoxia. Abnormal sleep architecture has been reported during the process of opioids induction, maintenance and withdrawal. During induction and maintenance of opi

[Sleep disorders and sleep-related breathing disorders].

Normal sleep consists of 4-5 sleep cycles including light-, deep- and rapid eye-movement sleep. Restoration of physical and psychological function are the main effects of sleep. In most cases, disturbances of normal sleep become clinically evident as problems of initiating and maintaining sleep and/or as increased daytime sleepiness. Approximately 10% of adults suffer from pronounced insomnia, a similar percentage from markedly increased daytime sleepiness. Sleep disorders cause high socio-economic costs due to increased accident risk, cardiovascular sequelae and sick leave. Most of the 88 dis

Sleep quality, sleep propensity and academic performance.

We examined associations between measures of sleep propensity on the Epworth Sleepiness Scale, sleep quality on the Pittsburgh Sleep Quality Index and academic performance by GPA and grades in introductory psychology for 414 students. In the total sample, neither sleep propensity nor sleep quality correlated with GPA or introductory psychology grades. However, among students carrying a full course load, those reporting poor sleep quality performed less well on academic measures than those reporting a better quality of sleep. Further research is needed to assess the moderating influence of over

Sleep, sleep disorders and motor vehicle crashes.

There is wide consensus that drowsiness and sleep during driving put the driver and others at high risk for motor vehicle crashes. A large percentage of both private automobile and commercial vehicle crashes and crash fatalities are associated with the sleepy driver each year. Sleepiness in persons without sleep disorders may occur from potentially preventable causes such as sleep deprivation, drinking alcohol or taking sedative medications. Sleepiness also occurs uncontrollably in persons with certain sleep disorders. It is important, therefore, for persons to have a diagnosis of a sleep diso

Sleep studies for sleep-related breathing disorders.

Our understanding of sleep-related breathing disorders is still in a period of rapid change. Considerable uncertainty exists in several major areas of patient assessment. Although early definitions of sleep apnoea enshrined the concept of rigid guidelines, such an approach is no longer helpful, and may constrain potential advances. The recognition of the basic pathophysiological events in obstructive sleep apnoea (OSA) has evolved from purely apnoeas, to include hypopnoeas, and now to increased upper airway resistance alone. Our view as to the significant consequences of such respiratory event

The orexin story, sleep and sleep disturbances.

The orexins, also known as hypocretins, are two neuropeptides (orexin A and B or hypocretin 1 and 2) produced by a few thousand neurons located in the lateral hypothalamus that were independently discovered by two research groups in 1998. Those two peptides bind two receptors (orexin/hypocretin receptor 1 and receptor 2) that are widely distributed in the brain and involved in the central physiological regulation of sleep and wakefulness, orexin receptor 2 having the major role in the maintenance of arousal. They are also implicated in a multiplicity of other functions, such as reward seeking,

Sleep and sleep disorders in children and adolescents.

Pediatric sleep disorders are common, affecting approximately 25% to 40% of children and adolescents. Although there are several different types of sleep disorders that affect youth, each disorder can have a significant impact on daytime functioning and development, including learning, growth, behavior, and emotion regulation. Researchers are only beginning to uncover the interaction between sleep and psychiatric disorders in children and adolescents, including depression, attention-deficit/hyperactivity disorder, and autism. This article reviews normal sleep and sleep disorders in children an

The sleep-wake cycle and sleeping pills.

Sleeping pills are drugs which are used world-wide to combat sleep disturbances, and to prevent symptoms due to maladjustment to shiftwork or jet-lag. Today, benzodiazepines and the so-called "non-benzodiazepines", such as zolpidem, which both act on benzodiazepine receptors, are drugs of first choice and they are substitutes for barbiturates. Their use as sleeping pills in insomniacs is established after appropriate medical diagnosis. Symptoms from shiftwork or jet-lag are due to an internal desynchronisation of biological rhythms, and there is ample evidence that benzodiazepines are not effe

Sleep disorders and sleep problems in childhood.

Sleep problems are common in childhood. A distinction is made between problems in which polysomnography is abnormal (i.e., the parasomnias, sleep apnea and narcolepsy) and problems that are behavioral in origin and have normal polysomnography. The parasomnias--sleep terrors, somnambulism and enuresis--appear to be related to central nervous system immaturity and are often outgrown. Obstructive sleep apnea syndrome (OSAS) is frequently missed in children and can often be cured through surgery. Behavioral sleep problems may be overcome after parents make interventions. Physicians can be of great

Sleep and sleep disorders in older adults.

Complaints of sleep disturbance increase with age. Objective sleep assessments using polysomnography reveal sleep impairments (increased wakefulness and arousal from sleep; decreased slow wave sleep) even in healthy seniors. Both polysomnographic sleep and subjective sleep worsen in the presence of health impairments related to drug use, pain, cardiovascular disease, diabetes, depression, or other emotional disorders. In addition to normal aging and chronic disease, sleep complaints can also result from poor sleep habits, specific occult disorders during sleep, or some combination of these fac

Associations between sleep hygiene and secondary objective sleep parameters: moving beyond sleep duration.

UNLABELLED: The present study sought to better understand the relationship between a broad range of sleep hygiene behaviours and important but under researched objective sleep outcomes: total time spent awake after sleep onset (WASO), the number of awakenings (NWAK) during the night, and sleep onset latency (SOL). Secondary data from a 14-day repeated measures design among 74 undergraduate students was analysed using linear mixed models to determine the between and within-person associations of 32 sleep hygiene behaviours with objectively measured sleep outcomes. Later bedtimes and earlier wak

Daridorexant in severe obstructive sleep apnea: effects on sleep-disordered breathing and sleep.

STUDY OBJECTIVES: To evaluate the effect of daridorexant on nighttime respiratory function and sleep in adults with severe obstructive sleep apnea (OSA) without insomnia. MATERIALS AND METHODS: This randomized, double-blind, placebo-controlled, two-period, crossover trial was conducted at a single sleep center in 16 adults (≥18 years) with severe OSA without insomnia. In each period, daridorexant 50 mg or placebo was administered every evening for 5 days. Primary and secondary endpoints were the treatment differences (daridorexant-placebo) for apnea/hypopnea index (AHI) and oxygen saturation

Glycemic burden during nocturnal sleep in patients with sleep bruxism across sleep stages.

OBJECTIVE: To assess nocturnal glycemic burden across sleep stages in adults with polysomnographically confirmed sleep bruxism. METHODS: 95 adults underwent polysomnography, bilateral masseter electromyography, and continuous glucose monitoring. 70 participants with BEI ≥2 events/h formed the sleep bruxism group and 25 with BEI <2 were controls. Glycemic burden was calculated as glucose AUC (mg·h/dL). RESULTS: Fasting glucose, glycated hemoglobin, sleep architecture, respiratory indices, and oxygenation did not differ. Glycemic burden was higher with bruxism during total sleep time (690.06 v

Sleep fragmentation during rapid eye movement sleep and hypertension in obstructive sleep apnea.

OBJECTIVE: Sleep fragmentation determined by repetitive arousals from sleep in obstructive sleep apnea (OSA) is associated with hypertension. We aimed to quantify the independent association of arousals during rapid eye movement (REM)/non-rapid eye movement (NREM) sleep with prevalent hypertension. METHODS: We included adults with 4 h of total sleep time and at least 30 min of REM sleep obtained from overnight in-laboratory polysomnography. Logistic regression models were fitted to explore the association between arousals during REM/NREM sleep and prevalent hypertension. All models controlled

Tilia Tomentosa promotes sleep, preserving sleep structure.

Tilia extracts have been used for centuries for their relaxing and hypnotic properties; however, their impact on the sleep-wake cycle and sleep architecture remained elusive. In this study, we investigated the effects of a chronic treatment with Tilia tomentosa extracts on the sleep-wake cycle and the associated neuronal activity, using chronic electrophysiological recordings in freely moving mice. We found that Tilia treatment induced a significant increase in NREM sleep and a concomitant reduction of wakefulness during the nocturnal (active) hours, in treated mice compared to controls. Furth
