In 2005 the American Academy of Sleep Medicine (AASM) published a revised form of the International Classification of Sleep Disorders (ICSD-2). Goals of the ICSD-2 are: A) To describe all currently recognized sleep and arousal disorders, and to base the descriptions on scientific and clinical evidence. B) To present the sleep and arousal disorders in an overall structure that is rational and scientifically valid. C) To render the sleep and arousal disorders as compatible with ICD-9 and ICD-10 as possible. In this article, sleep-disordered breathing disorders, as classified in the ICSD-2 are pr

Sleep disorders are widespread in society and are prevalent in military personnel and in Veterans. Disturbances of sleep and arousal mechanisms are common in neuropsychiatric disorders such as schizophrenia, post-traumatic stress disorder, anxiety and affective disorders, traumatic brain injury, dementia, and substance use disorders. Sleep disturbances exacerbate suicidal ideation, a major concern for Veterans and in the general population. These disturbances impair quality of life, affect interpersonal relationships, reduce work productivity, exacerbate clinical features of other disorders, a

INTRODUCTION: The term arousal usually refers to transient, brief arousal during nocturnal sleep or to a state of vigilance which is maintained during the day.
DEVELOPMENT AND CONCLUSION: In this review we consider the various accepted meanings and clinical disorders of 'arousal'. Insomnia (hyperarousal), parasomnias (partial arousal) and the sudden death syndrome of infancy are all attributed to disorders of arousal. It is concluded that diurnal hyperarousal leads to difficulty in getting off to sleep and staying asleep, whilst nocturnal arousal leads to excessive somnolence during the day,

The parasomnias comprises a group of disorders that intrude into the sleep process and are not primarily disorders of sleep and wake states per se. These disorders are manifestations of central nervous system activation usually transmitted through skeletal muscle or autonomic nervous system channels (ICSD 1990). The parasomnias are divided into four groups: Arousal disorders (sleepwalking, night terrors, and confusionnal arousal), sleep-wake transition disorders (rhythmic movement disorder, sleep starts), REM sleep parasomnias (nightmares, sleep paralysis, hallucinations, and REM sleep behavio

STUDY OBJECTIVES: To establish whether the recent EEG and behavioral criteria of arousal disorders apply to sexsomnia.
METHODS: EEG and behavioral markers upon N3 sleep interruptions in videopolysomnography were retrospectively compared in 24 participants with sexsomnia, 41 participants with arousals disorders, and 40 healthy controls. The specificity and sensitivity of previously suggested EEG and behavioral cutoffs for supporting arousal disorders diagnosis were measured in the sexsomnia vs. control groups.
RESULTS: Participants with sexsomnia and arousals disorders showed a higher N3 frag

Parasomnias are abnormal behaviors and/or experiences emanating from or associated with sleep typically manifesting as motor movements of varying semiology. We discuss mainly nonrapid eye movement sleep and related parasomnias in this article. Sleepwalking (SW), sleep terrors (ST), confusional arousals, and related disorders result from an incomplete dissociation of wakefulness from nonrapid eye movement (NREM) sleep. Conditions that provoke repeated cortical arousals, and/or promote sleep inertia, lead to NREM parasomnias by impairing normal arousal mechanisms. Changes in the cyclic alternati

Diagnosing sleep phases, arousal problems, and apnea episodes using Polysomnography (PSG) signals is often time-consuming. However, automated approaches have demonstrated promising results. Early detection of sleep disturbances can facilitate the diagnosis of neuropathologies before they progress. Given the significance of sleep events in diagnosing and treating sleep disorders, automated arousal disorder classification is increasingly crucial. Timely intervention for arousal disorders, if detected early, can potentially slow the progression of neuropathological illnesses such as Multiple Syst

Arousals from sleep and consequent sleep disruption may be a causal link between sleep-disordered breathing and its sequellae in children. Quantification of arousals therefore makes an important contribution to the overall assessment of the sleep of a child with suspected obstructive sleep apnea (OSA) or other sleep disorders. Arousals are classically defined by changes in the electroencephalographic (EEG) channels, but most arousals in children involve body movement in addition to EEG changes. Several methods of quantifying arousals without the use of EEG have been proposed, with the aim of s

PURPOSE OF REVIEW: This article discusses the clinical manifestations, diagnosis and differential diagnosis, pathophysiology, and management of parasomnias occurring in non-rapid eye movement (REM) sleep.
RECENT FINDINGS: Disorders of arousal are characterized by dissociated sleep, with wake and sleep phenomena intermingling, and local sleep, in which different areas of the brain exist simultaneously in different states of wakefulness or sleep. The frequency of arousals from slow-wave sleep with delta or mixed-frequency activity has a high sensitivity but relatively low specificity for the di

BACKGROUND: Parasomnias are disruptive motor episodes during sleep transitions, classified into rapid eye movement (REM) and non-rapid eye movement (NREM) types. NREM parasomnias (arousal disorders) include confusional arousals, sleepwalking, and sleep terrors. Despite their potential effect on mental health, limited research exists among university students.
OBJECTIVES: This study aims to ascertain the frequency of arousal disorders, their association with sleep quality, and their impact on the academic performance of university students.
METHODS: A quantitative cross-sectional study was co

Increasing respiratory effort is the likely stimulus for arousal in patients with sleep-disordered breathing. Changes in the phase angle waveform (an indirect measure of respiratory effort) may provide a useful non-EEG indicator of respiratory-related arousal. The aim of this study was to investigate the relationship between phase angle change (using a continuous measurement technique) and EEG arousal. Polysomnographic sleep recordings (including: EEG, EOG, EMG, respiratory effort [ribcage and abdominal movement], respiratory paradox [continuous phase angle measurement], oral-nasal airflow, an

The role of arousals in sleep is gaining interest among both basic researchers and clinicians. In the last 20 years increasing evidence shows that arousals are deeply involved in the pathophysiology of sleep disorders. The nature of arousals in sleep is still a matter of debate. According to the conceptual framework of the American Sleep Disorders Association criteria, arousals are a marker of sleep disruption representing a detrimental and harmful feature for sleep. In contrast, our view indicates arousals as elements weaved into the texture of sleep taking part in the regulation of the sleep

Sleep state instability is a potential mechanism of central apnea/hypopnea during non-rapid eye movement (NREM) sleep. To investigate this postulate, we induced brief arousals by delivering transient (0.5 second) auditory stimuli during stable NREM sleep in eight normal subjects. Arousal was determined according to American Sleep Disorders Association (ASDA) criteria. A total of 96 trials were conducted; 59 resulted in cortical arousal and 37 did not result in arousal. In trials associated with arousal, minute ventilation (VE) increased from 5.1 +/- 1.24 minutes to 7.5 +/- 2.24 minutes on the

We consider the disorders of arousal and sleep-related hypermotor epilepsy as genetic twin-conditions, one without, one with epilepsy. They share an augmented arousal-activity during NREM sleep with sleep-wake dissociations, culminating in sleep terrors and sleep-related hypermotor seizures with similar symptoms. The known mutations underlying the two spectra are different, but there are multifold population-genetic-, family- and even individual (the two conditions occurring in the same person) overlaps supporting common genetic roots. In the episodes of disorders of arousal, the anterior cing

Parasomnia is a common pediatric sleep disorder that can cause parents or caregivers distress when experienced by their children. Based on the International Classification of Sleep Disorders, parasomnias can be divided into two subgroups: non-rapid eye movement (NREM) parasomnias and rapid eye movement (REM) parasomnias. REM sleep parasomnias include nightmares, REM behavior disorder, and sleep paralysis, whereas NREM sleep parasomnias include disorders of arousal such as confusional arousals, sleepwalking, sleep talking, night terrors, and sleep-related eating disorder. BACKGROUND AND PURPOSE: Narcolepsy is a sleep disorder with clinical symptoms attributed to a reduced activation of the arousal system. Cyclic alternating pattern (CAP) is the expression of rhythmic arousability during non-rapid eye movement (NREM) sleep. CAP parameters, arousals and conventional sleep measures were studied in narcoleptic patients with cataplexy.
PATIENTS AND METHODS: Data were collected from all-night polysomnographic (PSG) recordings and the multiple sleep latency test (MSLT) on the intervening day of 25 drug-naive patients (10 males and 15 females; mean age: 34+/-16 years)

This review discusses recent developments in classifying the sleep disorders. Several classification systems are presented, and the Diagnostic Classification of the Sleep and Arousal Disorders (DCSAD), published by the Association of Sleep Disorders Centers in 1979, is used as the basis for reviewing their differences. The four main sections of the DCSAD are presented: the disorders of initiating and maintaining sleep, the disorders of excessive somnolence, the sleep-wake schedule disorders, and the dysfunctions associated with sleep, sleep stages, or partial arousals. Disorders discovered sin

STUDY OBJECTIVES: To construct and evaluate the inter-rater reliability of the Structured Clinical Interview for DSM-5-TR Sleep Disorders - Kid (SCISD-Kid).
METHOD: The SCISD-Kid was modeled on the adult SCISD-R and accounted for pediatric developmental and sociocultural factors. Fifty sleep-disturbed children (Mage = 11.9, SD = 2.9) and 50 caregivers responded to the final SCISD-Kid. Video recordings were double-scored to evaluate inter-rater reliability.
RESULTS: The final SCISD-Kid contained approximately 90 questions. Eight of the nine covered disorders had prevalence rates sufficient fo

OBJECTIVE: Periodic limb movements in sleep (PLMS) may or may not be associated with restless legs syndrome (RLS). The number of PLMS is commonly used to assess the clinical severity and sleep quality of patients with RLS. It is still unclear whether the sleep disorder of periodic limb movement disorder (PLMD) is different from the sleep disorder in RLS.
METHODS: We compared the polysomnograms (PSGs) of 27 prospectively recruited RLS patients and 26 retrospectively recruited age- and sex-matched PLMD patients without RLS symptoms.
RESULTS: The PLM index and the index of arousal-associated PL

Delirium is a disturbance of consciousness and cognition that results in a confusional state. It tends to fluctuate in intensity and is often observed in older patients. Sleep is a window of vulnerability for the occurrence of delirium and sleep disorders can play a role in its appearance. In particular, delirious episodes have been associated with obstructive sleep apnoea syndrome, which is reported to be frequent in the elderly. Hereby, we present a case-report documenting the sudden onset of a confusional state triggered by obstructive sleep apnoea-induced arousal, together with a review of
