Sleep-to-wake transition movement disorders.

Consciousness and vigilance level are important factors for the manifestation and variability of many disorders, including movement disorders. Usually lumped together into unspecified "Wakefulness," the transition between wakefulness and sleep--the pre-dormitum, and between sleep and wakefulness--the post-dormitum, possess intrinsic cerebral metabolic patterns and mental, behavioural, and neurophysiological characteristics which make these peculiar states of vigilance independent. Moreover, the pre- and post-dormitum, with the relative state-dependent changes in firing patterns of many neurona

Obstructive sleep apnoea and depression--diagnostic and treatment implications.

Sleep apnoea (OSA), a common sleep disorder, is well recognised as a cause of morbidity including psychiatric disorders. There is increasing recognition of the link between OSA and depression. Sleep changes are intrinsic to depressive disorders, most notably disturbances of REM sleep; OSA causes predominantly REM sleep disturbances. The neuro-vegetative features of depression are similar or identical to the symptoms of OSA-an issue which has not achieved wide clinical recognition. A growing number of studies confirm the statistical link between the two conditions. The implications are twofold:

Cardiovascular consequences of sleep apnea.

Sleep apnea is a common health concern that is characterized by repetitive episodes of asphyxia. This condition has been linked to serious long-term adverse effects such as hypertension, metabolic dysregulation, and cardiovascular disease. Although the mechanism for the initiation and aggravation of cardiovascular disease has not been fully elucidated, oxidative stress and subsequent endothelial dysfunction play major roles. Animal models, which have the advantage of being free of comorbidities and/or behavioral variables (that commonly occur in humans), allow invasive measurements under well-

Childhood epilepsy with occipital paroxysms: difficulties in distinct segregation into either the early-onset or late-onset epilepsy subtypes.

The Commission on Classification and Terminology of the International League Against Epilepsy Childhood rigidly segregated epilepsy with occipital paroxysms into 2 separate syndromes with different predominant seizure types: early-onset seizure susceptibility type consisting of prolonged infrequent, nocturnal autonomic seizures and accompanied by eye deviation and ictal vomiting and late onset with short diurnal frequent seizures and visual ictal manifestations along with throbbing headaches. Epileptic clinical manifestations and electroencephalographic data were analyzed in 28 patients with s

Sleep-wake control with age and neurodegenerative diseases.

Aging causes dramatic alterations in bodily functions. Among them, sleep quality declines with age, particularly in individuals with neurodegenerative diseases. In this review, we first describe alterations in sleep-wake architecture and discuss potential mechanisms underlying sleep disorders that arise with age. We discuss evidence linking sleep disorders with neurodegenerative diseases such as Alzheimer's disease (AD), Parkinson's disease (PD), progressive supranuclear palsy (PSP), Huntington's disease (HD), and amyotrophic lateral sclerosis (ALS) syndrome. Even though the causes of AD, PD,

Daytime sleep state misperception in a tertiary sleep centre population.

STUDY OBJECTIVES: Sleep state misperception is common in various sleep disorders, especially in chronic insomnia with a prevalence ranging between 9-50%. Most prior studies used nocturnal polysomnography (PSG) for the identification of sleep state misperception during nighttime. Our objective was to assess sleep state misperception during daytime in people with sleep disorders with excessive daytime sleepiness (EDS). METHODS: In this prospective observational study, we assessed the occurrence of, and factors influencing sleep state misperception in consecutive patients undergoing a routine mu

Sleep problems in midlife and beyond.

Good sleep quality is important for good health, both physical and mental, and indeed for quality of life, performance and productivity. Sleep problems increase with age in both sexes, but women are more susceptible to them at all ages. Although menopause is considered an important milestone (the decrease in both oestrogen and progesterone has been shown to reduce sleep quality), an increase in sleep problems is already evident in midlife, as there is an increased incidence of other diseases as well as mood symptoms, which may exert an effect on sleep quality either directly or via the side-ef

Sleep disorders in Parkinson's disease.

Sleep disorders in Parkinson's disease (PD) are frequent and have numerous etiologies. Both nighttime sleep disturbances and daytime sleepiness can occur. The key to effective treatment is appropriate diagnosis. A careful interview of the patient and his or her bed partner provides direction for additional evaluations. Referral to a sleep specialist for quantitative studies is necessary to evaluate for rapid eye movement (REM) sleep behavior disorder, sleep apnea, periodic limb movements, and other sleep disorders. Excessive daytime sleepiness may be attributed to interrupted nighttime sleep o

Transcranial magnetic stimulation and sleep disorders: pathophysiologic insights.

The neural mechanisms underlying the development of the most common intrinsic sleep disorders are not completely known. Therefore, there is a great need for noninvasive tools which can be used to better understand the pathophysiology of these diseases. Transcranial magnetic stimulation (TMS) offers a method to noninvasively investigate the functional integrity of the motor cortex and its corticospinal projections in neurologic and psychiatric diseases. To date, TMS studies have revealed cortical and corticospinal dysfunction in several sleep disorders, with cortical hyperexcitability being a c

Impact of inadequate sleep on unintentional injuries in adolescents.

Excessive daytime sleepiness is a growing societal problem in adolescents. Empirical evidence demonstrates that adolescents experience significant sleepiness as a result of inadequate or disrupted sleep. Although the functional impairment caused by sleepiness in the pediatric population is less studied, children and adolescents do experience changes in behavior and a decline in performance as seen in the adult population. The end result is functional impairment in adolescents, which then places them at higher risk for unintentional injury.

Ondine's curse: anesthesia for laparoscopic implantation of a diaphragm pacing stimulation system.

PURPOSE: Central alveolar hypoventilation syndrome (CAHS) is a rare disease characterized by the loss of autonomic control of breathing. This condition causes hypoventilation and obstruction during sleep. Throughout their lives, these patients require ventilatory assistance by means of positive pressure ventilation to their lungs via mask, tracheotomy, or other means, such as phrenic nerve pacers. The diaphragm pacing stimulation system (DPSS) is a new treatment where electrodes are implanted into the diaphragm and cause contraction on stimulation. The DPSS has been used successfully in tetrap

Aspects of parasomnias in childhood and adolescence.

The many parasomnias now officially described often occur in young patients. They are at risk of being confused with each other if their characteristic features are not well known and if they are not carefully assessed, mainly clinically. Accurate diagnosis is essential for choice of treatment, which varies considerably for different parasomnias. These points are illustrated mainly by reference to arousal disorders (including sleepwalking and sleep terrors) and the other parasomnias, such as nightmares and sleep-related epilepsies, from which they must be distinguished.

Sleep organization in children with partial refractory epilepsy.

Although it is currently known that sleep can influence epilepsy and epilepsy can influence sleep organization, few data have been published on this mutual interaction concerning the pediatric population. The objective of this study was to verify the eventual presence of sleep alterations in children with partial refractory epilepsy. Seventeen patients with partial refractory epilepsy were submitted to whole-night polysomnography as part of their epilepsy investigation. Polysomnographic recordings were performed on a digital video-electroencephalography (EEG) system and consisted of the regist

[Sleep medicine: development, contributions and perspectives. Report of the work group on sleep medicine].

Sleep is a basic biological process that has an impact on all the functions of the body, and interacts bidirectionally with virtually all of the body systems, so that the sleep disorders are associated with disturbances in other systems, either respiratory, neurological, cardiovascular, endocrine, immune, etc., and vice versa. The complexity of the regulatory mechanisms of sleep and the variety of their disorders, together with the clinical evidence accumulated in recent decades, have led to the birth of a new branch in medicine: the Sleep Medicine, with well defined intrinsic disorders. The c

Parasomnias and movement disorders in children and adolescents.

Childhood parasomnias and movement disorders arise from a variety of etiologic factors. For some children, psychopathology plays a causal role in sleep disorders; in other cases, recurrent parasomnia episodes induce psychopathology. Current research reveals complex interconnections between sleep and mental health. As such, it is important that clinicians consider the impact psychiatric disorders have on childhood parasomnias. This article describes common parasomnias and movement disorders in children and adolescents, with emphasis on psychologic and behavioral comorbidities.

Mechanistic insights into cortical network entrainment by tACS and tDCS: A computational modeling approach.

Non-invasive brain stimulation techniques such as transcranial alternating current stimulation (tACS) and transcranial direct current stimulation (tDCS) are increasingly explored for modulating large-scale brain oscillations associated with cognition, sleep, and neuropsychiatric disorders. However, the network-level mechanisms underlying their differential effects on synchronization and excitatory-inhibitory (E/I) dynamics remain poorly understood. This study investigates how tDCS, tACS, and algorithmically guided tACS (ALGO-tACS) influence phase synchronization, intrinsic frequency modulation
