Role of sleep-disordered breathing and sleep-wake disturbances for stroke and stroke recovery.

BACKGROUND: Sleep-disordered breathing (SDB) and sleep-wake disturbances (SWD) are highly prevalent in stroke patients. Recent studies suggest that they represent both a risk factor and a consequence of stroke and affect stroke recovery, outcome, and recurrence. METHODS: Review of literature. RESULTS: Several studies have proven SDB to represent an independent risk factor for stroke. Sleep studies in TIA and stroke patients are recommended in view of the very high prevalence (>50%) of SDB (Class IIb, level of evidence B). Treatment of obstructive SDB with continuous positive airway pressure

Sleep-Wake Disorders After Traumatic Brain Injury: Pathophysiology, Clinical Management, and Future.

After experiencing a traumatic brain injury (TBI), the majority of patients will develop sleep-wake disorders (SWD). These can include insomnia, posttraumatic pleiosomnia (increased sleep need), excessive daytime sleepiness (EDS), obstructive and/or central sleep apnea, circadian SWD, and a variety of parasomnias. Untreated SWD may impede the recovery process and can negatively impact mood, metabolic health, cognitive function, and immune function among other processes. Importantly, these patients tend to misperceive their posttraumatic sleep problems. Consequently, interviews performed in sta

EEG microstates during wake and NREM sleep in insomnia disorder.

Insomnia disorder (ID) is prevalent and debilitating, yet its neurophysiological basis remains unclear. Abnormalities in temporal parameters of electroencephalography (EEG) microstates have been linked to diverse neuropsychiatric disorders, but their dynamics across wakefulness and non-rapid eye movement (NREM) sleep in chronic insomnia remain unexplored. In this study, EEG microstate dynamics across wakefulness and NREM sleep were examined in adults with ID (n = 33) and healthy controls (HC; n = 29). Simultaneous EEG and functional magnetic resonance imaging (fMRI) were acquired during noctur

Delayed Sleep-Wake Phase Disorder: Can Polysomnography Be Useful?

BACKGROUND: Delayed sleep-wake phase disorder (DSWPD) is a chronic condition with a multifactorial etiology that primarily affects adolescents, significantly influencing their quality of life. In clinical practice, the contribution of intrinsic and behavioral factors is difficult to determine. The aim of our study was to compare data from clinical interviews, sleep diaries, actigraphy, and nocturnal polysomnography (PSG) in a cohort of adolescents with DSWPD and to assess psychiatric/neurodevelopmental comorbidity. METHODS: Thirty-one patients (22 male; mean age 15.4 ± 2.2 years, range 12 to

Tasimelteon for the treatment of non-24-hour sleep-wake disorder.

Tasimelteon (Hetlioz®), a melatonin receptor agonist, is the first, and, at the time of the publication, the only drug to be approved by the U.S. Food and Drug Administration (FDA) for the treatment of non-24-hour sleep-wake disorder (non-24). This circadian rhythm disorder occurs most commonly in blind individuals without light perception, and it results from their inability to entrain to the 24-hour photoperiod, although the indication does not specify a particular patient population. Non-24 is characterized by a persistent cycle of nighttime insomnia and daytime sleepiness, alternating with

Melatonin receptor agonists: SAR and applications to the treatment of sleep-wake disorders.

Melatonin (N-acetyl-5-methoxytryptamine) is synthesized and released by the pineal gland following a circadian rhythm characterized by high levels during the night. It shows several pharmacological effects on diverse cellular and animal models, mainly related to either its antioxidant activity or to its ability to activate specific receptors (MTr). Melatonin is widely used as a self-administered food additive, but its therapeutic potential needs more investigation and is hampered by its poor pharmacokinetics. This review will focus on the medicinal chemistry of agonist ligands of the two human

Delayed sleep-wake phase disorder and its related sleep behaviors in the young generation.

Delayed sleep-wake phase disorder (DSWPD) is a sleep disorder in which the habitual sleep-wake timing is delayed, resulting in difficulty in falling asleep and waking up at the desired time. Patients with DSWPD frequently experience fatigue, impaired concentration, sleep deprivation during weekdays, and problems of absenteeism, which may be further complicated by depressive symptoms. DSWPD is typically prevalent during adolescence and young adulthood. Although there are no studies comparing internationally, the prevalence of DSWPD is estimated to be approximately 3% with little racial differen

Sleep disorders in children with Smith-Magenis Syndrome - a neurodevelopmental disorder with sleep-wake-rhythm-dysfunction - recommendations for pediatricians.

BACKGROUND: Sleep disorders in children with Smith-Magenis Syndrome (SMS) are common. They affect not only the child but also the entire family. Disturbed sleep in SMS is caused by a sleep-wake rhythm dysfunction. SMS patients mostly have an inverted melatonin secretion rhythm with a peak in melatonin levels during the day. Therefore, children with SMS are often awake at night and sleepy during the day, followed by extremely severe behavioral problems. There are many recent scientific studies with treatment recommendations. PURPOSE: Since SMS is a rare disorder, many doctors are not familiar w

Stability of nocturnal wake and sleep stages defines central nervous system disorders of hypersomnolence.

STUDY OBJECTIVES: We determine if young people with narcolepsy type 1 (NT1), narcolepsy type 2 (NT2), and idiopathic hypersomnia (IH) have distinct nocturnal sleep stability phenotypes compared to subjectively sleepy controls. METHODS: Participants were 5- to 21-year old and drug-naïve or drug free: NT1 (n = 46), NT2 (n = 12), IH (n = 18), and subjectively sleepy controls (n = 48). We compared the following sleep stability measures from polysomnogram recording between each hypersomnolence disorder to subjectively sleepy controls: number of wake and sleep stage bouts, Kaplan-Meier survival cur

Sleep-wake cycle phenotypes in young people with familial and non-familial mood disorders.

OBJECTIVES: Converging evidence identifies that the offspring of parents with bipolar disorder (BD), individuals at clinical high risk of BD, and young people with recent onset BD may differ from other clinical cases or healthy controls in terms of sleep-wake profiles. However, it is possible that these differences may reflect current mental state, subtype of mood disorder, or familial traits. This study aimed to determine objective and subjective sleep-wake profiles in individuals aged 15-25 years with a current major depressive episode, in relation to familial traits. METHODS: Frequency mat

Sleep wake cycle and eating patterns undergo major changes during the adolescent period. Most of the new sleeping and eating patterns come within normal range, and therefore, it is important to differentiate them from pathologicaL eating and sleeping behavior Over the Last decades there has been a dramatic increase in the prevalence of eating disorders (ED) in western society. The main syndromes are anorexia nervosa (AN), buLimia nervosa (BN) and non-specified eating disorders (ED-NOS). These disorders are linked with high morbidity and life-threatening complications. Fluctuations between sleep and wakefulness: wake-like features indicated by increased EEG alpha power during different sleep stages in nightmare disorder.

Although a growing body of research indicates that frequent nightmares are related to impaired sleep regulation, the pathophysiology of nightmare disorder is far from being fully understood. We examined the relative spectral power values for NREM and REM sleep separately in 19 individuals with nightmare disorder and 21 healthy controls, based on polysomnographic recordings of the second nights' laboratory sleep. Nightmare subjects compared to controls exhibited increased relative high alpha (10-14.5Hz) and fronto-central increases in high delta (3-4Hz) power during REM sleep, and a trend of in

Iron Deficiency and Restless Sleep/Wake Behaviors in Neurodevelopmental Disorders and Mental Health Conditions.

Iron deficiency (ID) and restlessness are associated with sleep/wake-disorders (e.g., restless legs syndrome (RLS)) and neurodevelopmental disorders (attention deficit/hyperactivity and autism spectrum disorders (ADHD; ASD)). However, a standardized approach to assessing ID and restlessness is missing. We reviewed iron status and family sleep/ID history data collected at a sleep/wake behavior clinic under a quality improvement/quality assurance project. Restlessness was explored through patient and parental narratives and a 'suggested clinical immobilization test'. Of 199 patients, 94% had ID,

Problems in the Development of the Sleep-Wake Rhythm Influence Neurodevelopmental Disorders in Children.

Development of the sleep-wake rhythm has a significant effect on the physical and mental development of children. The sleep-wake rhythm is controlled by aminergic neurons in the brainstem's ascending reticular activating system, which is associated with synaptogenesis and the promotion of brain development. The sleep-wake rhythm develops rapidly within the first year after birth. At 3-4 months of age, the framework of the circadian rhythm is established. The objective of the present review is to assess a hypothesis concerning problems in the development of the sleep-wake rhythm and their effec
