A rare phenomenon, primary pulmonary smooth muscle tumor and its management.

Smooth muscle tumors of uncertain malignant potential, the borderline tumors arising from the smooth muscle cells, usually grow slowly and do not fulfill the diagnostic criteria of leiomyosarcoma and its variants, but may behave in a malignant manner. A 15-year-old female patient with an endobronchial mass in the left main bronchus on thoracic computed tomography underwent thoracotomy and tracheobronchoplasty with a wide and safe margin. Histopathological evaluation revealed a smooth muscle tumor of uncertain malignant potential. There were no complications related to the operation during the

The PTEN tumor suppressor inhibits human airway smooth muscle cell migration.

Airway remodeling in asthma is characterized by increased airway smooth muscle (ASM) mass, accompanied by cell migration. It is well known that the proliferation and migration of ASM cells (ASMCs) play a key role in airway remodeling, but the precise mechanism modulating these cellular events remains unclear. One of the genes most likely to be involved in this process is the phosphatase and tensin homolog (PTEN) gene, whose deletion from chromosome 10 can inhibit the proliferation and migration of many cell types. In this study, we investigated the effects of PTEN on human ASMCs. The cells wer

EXPRESSION OF ALPHA-SMOOTH MUSCLE ACTIN IN ODONTOGENIC CYSTS AND TUMORS.

BACKGROUND: Odontogenic cysts and tumors exhibit different degrees of aggressiveness in their biological behavior. There has been evidence that the presence of myofibroblasts (MFs) at the invasion front promotes tumor invasion. Our study is based on the fact that MFs are important in the biological behavior of odontogenic cysts and tumors. AIM: To assess immunohistochemically expression of alpha-smooth muscle actin (α-SMA) of MFs in odontogenic cysts and tumors and correlate this expression to their biological behavior. MATERIALS AND METHODS: The archival tissues collected for 1.5 years were

Immunohistochemical analysis in the diagnosis of uterine myometrial smooth muscle tumors.

Uterine myometrial smooth muscle tumors sometimes raise concerns related to their histopathological typing, and that especially for benign entities with areas of atypia. In this study, we analyzed a group of myometrial smooth muscle tumors in what it regards their differential diagnosis utilizing a panel of antibodies consisting of α-SMA, Ki-67, p16, p53, PR and bcl-2. The study included a total of 16 cases of cellular leiomyomas, five cases of atypical leiomyomas and six of leiomyosarcomas, which were all analyzed for age, risk factors, mitotic activity, cellular atypia and tumor staging. In

Smooth muscle tumors of the esophagus: clinicopathological findings in six patients.

Preoperatively, it is difficult to discriminate leiomyoma and leiomyosarcoma of the esophagus, which are rare smooth muscle tumors. The objective of this study was to evaluate the clinicopathological findings of this unusual lesion. A search of the surgery archives of the Toyama Prefectural Central Hospital of Pathology revealed six cases of esophageal smooth muscle tumors. Clinicopathological findings were reviewed retrospectively. Only three patients (50%) presented with dysphagia, and the remaining three patients were asymptomatic. These patients underwent surgical excision. Histologically

[DNA flow cytometry in smooth muscle tumors of the gastrointestinal tract].

DNA ploidy of 45 smooth muscle tumors of the G.I. tract was determined by flow cytometry and correlated with clinical features and prognosis. The sites of the tumors were: esophagus (1), stomach (24), small intestine (12), large intestine (6), liver (1) and pancreas (1). The histologic type was leiomyoma in 14, leiomyosarcoma in 29, and leiomyoblastoma in 2. DNA aneuploidy was more frequent in leiomyosarcoma (17/29) than leiomyoma (5/14), but the difference was not statistically significant. One leiomyoblastoma was diploid and the other was aneuploid. No patients with leiomyoma died. In patien

Smooth muscle tumors of the inferior vena cava and right heart.

Four Chinese patients with extensive smooth muscle tumors of the inferior vena cava (IVC) and right heart are described. Two patients had primary IVC leiomyosarcomas and one had both IVC and cardiac metastases from uterine leiomyosarcoma. The remaining patient had extra-uterine intravenous leiomyomatosis, with extensive tumor calcification. Clinical presentation was variable, consisting of the Budd-Chiari syndrome, a new heart murmur, and signs of cerebral embolism. All the lesions were defined radiologically prior to surgical resection of cardiac tumors.

Smooth muscle in atypical lipomatous tumors. A report of three cases.

Three cases of atypical lipomatous tumor with areas of smooth muscle are presented. The patients were a 49-year-old man, a 45-year-old woman, and a 63-year-old man; their neoplasms were located in the spermatic cord, the mediastinum, and the retroperitoneum, respectively. The smooth muscle foci were well differentiated in all cases. Smooth muscle was present in the primary tumor in the first and second cases (spermatic cord and mediastinum) and in a recurrence in the second case (there has been no recurrence in the first case). In the third case (retroperitoneum), smooth muscle was found only

An Update on Uterine Smooth Muscle Tumors: Is It a Leiomyoma, a STUMP, or a Leiomyosarcoma?

Uterine smooth muscle tumors (USMTs) represent a diagnostically and clinically challenging subset of uterine mesenchymal neoplasms. Up to 5% of these tumors exhibit ambiguous histological features that preclude definitive classification as either benign leiomyomas or malignant leiomyosarcomas. This review provides a comprehensive synthesis of the evolving diagnostic criteria, histopathological variants, and recent advancements in immunohistochemical and molecular profiling of smooth muscle tumors with uncertain malignant potential (STUMPs). The review traces the historical development of diagn

The role of hypoxia related angiogenesis in uterine smooth muscle tumors.

Mechanisms of hypoxia-related angiogenesis are important for uterine smooth muscle tumors. Factors that are related to angiogenesis during hypoxia include vascular endothelial growth factor (VEGF), hypoxia inducible factor 1α (HIF1α), T-cell intracellular antigen1 (TIA1), eukaryotic translation initiation factor 2α (eIF2α) and thrombospondin 1 (TSP1). We investigated immunoreactivities of VEGF, HIF1α, TIA1, eIF2α and TSP1 using an indirect immunoperoxidase method for formalin fixed, paraffin embedded tumors that had been diagnosed as leiomyoma (LMY), cellular leiomyoma (CLM) or leiomyosarcoma

Immunohistochemical analysis of p16 expression in uterine smooth muscle tumors.

The expression of p16 as a tumor suppressor protein was evaluated in a retrospective analysis of paraffin-embedded tissue specimens of leiomyosarcoma (LMS), leiomyoma (LM) and normal myometrium. In this study, we investigated by immunohistochemistry p16 expression in 15 LMSs, 15 LMs and ten normal myometrium. Strong expression of p16 was found in 12 of the 15 LMSs and in three cases weak expression; three LMs had focal and weak p16 staining but none of the normal myometrium. A statistically significant difference regarding the frequency of p16 protein expression was observed between LMS and LM

Gastrointestinal smooth muscle tumors and iron deficiency anemia in children.

Smooth muscle tumors are rarely seen in the pediatric population. We present a child with smooth muscle tumor of low malignant potential in the ileocecal valve region in whom iron deficiency anemia was the only presenting sign. Abdominal computed tomographic (CT) scan, barium enema, and colonoscopy revealed the mass. Following resection of the tumor the anemia was corrected and the child feels well.

Uterine Smooth Muscle Tumor of Uncertain Malignant Potential (STUMP): A Case Report.

Uterine smooth muscle tumors of uncertain malignant potential (STUMP) are rare mesenchymal neoplasms that occupy a diagnostic gray zone between benign leiomyomas and malignant leiomyosarcomas. Owing to their low incidence and overlapping clinical, radiological, and histopathological features, STUMPs pose significant diagnostic and therapeutic challenges and are frequently identified only after surgical intervention for presumed benign disease. We report the case of a 39-year-old Mexican woman with a two-year history of abnormal uterine bleeding (AUB) and severe dysmenorrhea. Preoperative imagi

Uterine Smooth Muscle Tumor of Undetermined Malignant Potential (STUMP): A Diagnostic Challenge.

Smooth muscle tumors of the uterus can range from benign leiomyoma (LM) to malignant leiomyosarcoma (LMS) histologically. Between these two entities lies a gray zone of smooth muscle tumors of undetermined malignant potential, commonly known as STUMP. Their diagnosis has always been a challenge for the pathologists. Once diagnosed, clinicians face difficulties in managing STUMP because of the lack of discrete treatment guidelines. Here, we report a case of a 52-year-old lady who presented in an emergency with severe menorrhagia. On examination, the patient was pale and there was a 4x3 cm size

Smooth Muscle Tumor of Uncertain Malignant Potential (STUMP): A Comprehensive Multidisciplinary Update.

Background and Objectives: The uterine smooth muscle tumors of uncertain malignant potential (STUMP) are tumors with pathological characteristics similar to leiomyosarcoma, but that do not satisfy histological criteria for leiomyoma. These are problematic lesions with intermediate morphologic features; thus, diagnosis and treatment are difficult. This narrative review aims to review data in the literature about STUMPs, particularly focusing on management and therapeutic options and strategies for women who desire to preserve fertility. Material and Methods: authors searched for "uterine smooth

Rare Case of Smooth Muscle Tumor of Uncertain Malignant Potential - Clinical Case.

Introduction: Smooth muscle tumor of uncertain malignant potential (STUMP) still represents a medical and surgical challenge, being rare and hard to diagnose. Normally STUMP consists in a multitude of uterine tumors that do not meet the current histologic criteria for a specific malignant or benign tumor. Clinical, imagistic and laboratory investigations seem to only raise the presumptive diagnosis in these cases, the diagnosis being confirmed during histological examination of the post hysterectomy/myomectomy specimen. Case report: We present the case of a 50-year-old female patient who was e

Laryngeal Epstein-Barr Virus-Associated Smooth Muscle Tumor in an Undernourished Child.

Smooth muscle tumors associated with Epstein-Barr virus infections (EBV-SMT) of laryngeal origin are exceedingly rare and have been reported in few adult patients, but not in children. This reported case describes a lesion found in the larynx of an 8-year-old Guatemalan undernourished girl. Microscopically, the lesion showed a highly cellular mesenchymal spindle cell tumor, containing frequent lymphocytes. The immunohistochemical analysis revealed positivity for α-smooth muscle actin and h-caldesmon. In addition, most of the tumor cells were positive for EBV by in situ hybridization. To the be

Biomarker resolution of uterine smooth muscle tumor necrosis as benign vs malignant.

Uterine leiomyosarcomas are rare malignant tumors with a poor prognosis while leiomyomas are common benign tumors unrelated to their malignant counterparts. Diagnostic features commonly present in leiomyosarcoma include cytologic atypia, high mitotic index, and a sarcoma-specific geographic cell death designated 'tumor cell necrosis (TCN)'. TCN has a sharp viable-nonviable boundary lacking inflammation, fibrosis or granulation tissue seen in nonspecific infarction. These characteristics are sometimes difficult to interpret on routine hematoxylin and eosin slides, and can lead to diagnostic err

Rare Presentations of Epstein-Barr Virus--Associated Smooth Muscle Tumor in Children.

Epstein-Barr virus (EBV) has oncogenic potential and has been implicated in the etiology of a wide range of malignancies. Certain EBV-driven neoplasms, such as smooth muscle tumors (SMTs), manifest typically in immunocompromised patients. In children, these neoplasms have been encountered in the setting of primary immune disorders, specifically severe combined and common variable immunodeficiency syndromes. Human immunodeficiency virus infection and posttransplant immunosuppression, in particular liver and kidney transplantation, likewise increase the risk in the pediatric population. The loca
