Neural responses to social threat and predictors of cognitive behavioral therapy and acceptance and commitment therapy in social anxiety disorder.

Previous research has often highlighted hyperactivity in emotion regions to simple, static social threat cues in social anxiety disorder (SAD). Investigation of the neurobiology of SAD using more naturalistic paradigms can further reveal underlying mechanisms and how these relate to clinical outcomes. We used fMRI to investigate responses to novel dynamic rejection stimuli in individuals with SAD (N=70) and healthy controls (HC; N=17), and whether these responses predicted treatment outcomes following cognitive behavioral therapy (CBT) or acceptance and commitment therapy (ACT). Both HC and SA

Impact of cognitive-behavioral therapy for social anxiety disorder on the neural bases of emotional reactivity to and regulation of social evaluation.

UNLABELLED: We examined whether Cognitive-Behavioral Therapy (CBT) for social anxiety disorder (SAD) would modify self-reported negative emotion and functional magnetic resonance imaging brain responses when reacting to and reappraising social evaluation, and tested whether changes would predict treatment outcome in 59 patients with SAD who completed CBT or waitlist groups. For reactivity, compared to waitlist, CBT resulted in (a) increased brain responses in right superior frontal gyrus (SFG), inferior parietal lobule (IPL), and middle occipital gyrus (MOG) when reacting to social praise, and

Social well-being moderates behavioral therapy response for generalized anxiety disorder.

INTRODUCTION: Identifying moderators of treatment outcome for depression and anxiety offers promise for personalizing treatment or providing novel intervention targets. Indices of social well-being have emerged as candidate moderators due to their robust impact on mental health and quality of life. Here, we examine whether measures of social support, isolation, and loneliness moderate outcomes to behavioral activation (BA) and exposure-based therapy (EXP) for individuals with GAD. METHODS: This secondary analysis of a randomized clinical trial included 69 participants (31 EXP, 38 BA). Partici

Cognitive-behavioral therapy for social anxiety disorder: supporting evidence and future directions.

The present paper examines the role of cognitive-behavioral therapy (CBT) in the treatment of social anxiety disorder (SAD). A cognitive-behavioral model of SAD is first presented. Different modalities of CBT for SAD are then described, including exposure, cognitive restructuring, relaxation training, and social skills training, and evidence supporting their efficacy is reviewed. The comparative and combined impact of CBT and pharmacotherapeutic interventions is also explored. CBT appears to be an efficacious treatment for SAD. However, the overall efficacy CBT may be increased by closer exami

Cognitive-behavioral therapy for social anxiety disorder: current status and future directions.

Cognitive-behavioral therapy (CBT) is the most thoroughly studied nonpharmacologic approach to the treatment of social anxiety disorder, and its efficacy has been demonstrated in a large number of investigations. This article summarizes the data on the efficacy of CBT for the treatment of the symptoms of social anxiety disorder and impaired quality of life. The relative efficacy of various CBT approaches, both in the short-term and over extended follow-up periods, is reviewed. Factors associated with more or less positive response to CBT among patients with social anxiety disorder are examined

Neural Circuits of Social Behavior in Autism Spectrum Disorder: From Mechanism to Intervention.

INTRODUCTION: Difficulties in social communication and interaction are core diagnostic features of autism spectrum disorder (ASD), yet interventions targeting these difficulties have modest and variable effects. Understanding mechanistically distinct contributions to social behaviors may identify new therapeutic opportunities. METHODS: This narrative review integrates human neuroimaging studies, supporting rodent circuit neuroscience, clinical intervention research, and developmental studies. We organize the findings into four overlapping functional-computational domains and relate these doma

Testosterone in patients with borderline personality disorder: from hair analyses to social behavior.

Clemens Kirschbaum provided the important inspiration for our team to extend the assessment of steroid hormones beyond blood and saliva samples to include hair analyses. This enabled the investigation of the effects of long-term, cumulative steroid hormone secretion. This novel methodological perspective provided particularly compelling for the study of long-lasting stress-related mental disorders, such as borderline personality disorder (BPD). In an initial study, we found unaltered hair cortisol but increased hair testosterone in women with BPD. Notably, the latter finding inspired us to fur

Changes in Decentering Across Cognitive Behavioral Group Therapy for Social Anxiety Disorder.

To further improve treatments, we need to better understand potential common treatment mechanisms, such as decentering, or the ability to observe thoughts and feelings as objective events in the mind rather than personally identifying with them (Safran & Segal, 1990). Therefore, this study examined whether 12 sessions of Cognitive Behavioral Group Therapy (CBGT) for 63 clients (57.6% female, 50.8% White) diagnosed with social anxiety disorder led to increases in decentering and whether increased decentering was associated with improved outcome. Furthermore, this study examined whether decenter

The behavioral economics of social anxiety disorder reveal a robust effect for interpersonal traits.

Recent evidence suggests that reduced generosity among individuals with social anxiety disorder (SAD) in behavioral economic tasks may result from constraint in changing behavior according to interpersonal contingencies. That is, people with SAD may be slower to be more generous when the situation warrants. Conversely, more global effects on generosity may be related to interpersonal vindictiveness, a dimension only somewhat related to SAD. A total of 133 participants, 73 with the generalized form of SAD, completed self-report instruments and a behavioral economic task with simulated interpers

Prefrontal control and predictors of cognitive behavioral therapy response in social anxiety disorder.

Generalized social anxiety disorder (gSAD) is associated with aberrant anterior cingulate cortex (ACC) response to threat distractors. Perceptual load has been shown to modulate ACC activity such that under high load, when demands on processing capacity is restricted, individuals with gSAD exhibit compensatory activation to threat distractors yet under low load, there is evidence of reduced activation. It is not known if neural predictors of response to cognitive behavioral therapy (CBT), based on such emotional conflict resolution, interact with demands on controlled processes. Prior to CBT,

D-Cycloserine as an augmentation strategy with cognitive-behavioral therapy for social anxiety disorder.

OBJECTIVE: The authors examined whether D-cycloserine, a partial agonist at the glutamatergic N-methyl-d-aspartate receptor, augments and accelerates a full course of comprehensive cognitive-behavioral therapy (CBT) in adults with generalized social anxiety disorder. METHOD: This was a multisite randomized placebo-controlled efficacy study with 169 medication-free adults with generalized social anxiety disorder, of whom 144 completed the 12-week treatment and 131 completed the three follow-up assessments. Patients were randomly assigned to receive 50 mg of D-cycloserine or placebo 1 hour befo

Behavioral inhibition and risk for developing social anxiety disorder: a meta-analytic study.

OBJECTIVE: Behavioral inhibition (BI) has been associated with increased risk for developing social anxiety disorder (SAD); however, the degree of risk associated with BI has yet to be systematically examined and quantified. The goal of the present study was to quantify the association between childhood BI and risk for developing SAD. METHOD: A comprehensive literature search was conducted to identify studies that assessed both BI and SAD. Meta-analyses were performed to estimate the odds ratio (OR) of the association between BI and SAD in children. RESULTS: Seven studies met inclusion crite

Social behavior and autism traits in a sex chromosomal disorder: Klinefelter (47XXY) syndrome.

Although Klinefelter syndrome (47,XXY) has been associated with psychosocial difficulties, knowledge of the social behavioral phenotype is limited. We examined specific social abilities and autism traits in Klinefelter syndrome. Scores of 31 XXY men on the Scale for Interpersonal Behavior and the Autism Spectrum Questionnaire were compared to 24 and 20 control men respectively. XXY men reported increased distress during social interactions and less engagement in specific social behaviors. In the XXY group, levels of autism traits were significantly higher across all dimensions of the autism ph

Relative effects of cognitive and behavioral therapies on generalized anxiety disorder, social anxiety disorder and panic disorder: A meta-analysis.

Although cognitive and behavioral therapies are effective in the treatment of anxiety disorders, it is not clear what the relative effects of these treatments are. We conducted a meta-analysis of trials comparing cognitive and behavioral therapies with a control condition, in patients with social anxiety disorder (SAD), generalized anxiety disorder (GAD) and panic disorder. We included 42 studies in which generic measures of anxiety were used (BAI, HAMA, STAI-State and Trait). Only the effects of treatment for panic disorder as measured on the BAI (13.33 points; 95% CI: 10.58-16.07) were signi

Effects of Computer-Assisted Social Skills Training in Children With Disruptive Behavior Disorders: A Randomized Controlled Trial.

OBJECTIVE: Computer-assisted child-focused interventions are expected to improve efficiency and personalization of therapist-led treatments for children and adolescents. However, therapist-led, outpatient interventions using computer assistance are lacking for children with oppositional defiant disorder (ODD) or conduct disorder (CD). The present randomized controlled trial examined the efficacy of individualized computer-assisted social skills training for children with aggressive behavior compared to a resource activation intervention. METHOD: A total of 100 children aged 6-12 years with a

Relationships between the social communication questionnaire and pragmatic language, socialization skills, and behavioral problems in children with autism spectrum disorders.

The Social Communication Questionnaire (SCQ) is one of the most widely used screening instruments for autism spectrum disorder (ASD). This study examined the relationships between the reciprocal social interaction, communication, and repetitive/stereotyped factors on the Strength and Difficulties Questionnaire (SDQ) and pragmatic, socialization, and behavioral problems in children with ASD and children with typical development (TD). Participants were seven- to 11-year-old children with ASD without intellectual disability (n = 52) and with TD (n = 37). The two groups were matched on age and int

Generalized anxiety disorder symptoms and daily social behavior predict hair cortisol concentration.

BACKGROUND: Generalized anxiety disorder (GAD) symptoms are trait-like and associated with negative emotions and stress biomarkers like cortisol. Both GAD symptoms and cortisol have been linked to interpersonal dominance and affiliation. However, most studies on GAD symptoms and interpersonal processes remain cross-sectional, disconnected from daily stressors, or measure brief cortisol fluctuations. In contrast, hair cortisol concentration (HCC) assesses cortisol over longer periods. OBJECTIVES: This study examined direct and interacting effects of GAD symptoms and interpersonal processes pre

A Systematic Review of Social Validity Trends in Behavior Analytic Research on Major Neurocognitive Disorder.

Social validity is at the heart of applied behavior analysis, examining the extent to which the goals, interventions, and effects of a given treatment are satisfactory to treatment recipients and relevant stakeholders (Wolf, 1978). While social validity measures are critical to include across all populations, persons with Major Neurocognitive Disorder (MND) (formerly Dementia) (Emmady et al., 2022) warrant heightened attention, given both the increasing population of those living with MND and the progressive nature of the disease (WHO, 2023). The current systematic literature review therefore

Cognitive behavioral group therapy for social anxiety disorder - A critical review of methodological designs.

Social anxiety disorder (SAD) is highly prevalent with significant lifetime impacts, especially when left untreated. Cognitive behavioral therapy is the current gold standard treatment with successful patient outcomes. Cognitive behavioral group therapy (CBGT) is one form that provides unique benefits to participants including normalization of symptoms and social support, as well as providing increased access to treatment at a lower per-session cost. Research on this mode of treatment has been ongoing for over 20 years, but we still have much to learn about its overall efficacy. The goals of t

New insights into recalled parental behavior in social anxiety disorder: A cluster analytic approach.

BACKGROUND: Social Anxiety Disorder (SAD) is a highly heterogeneous disorder. To enlighten its heterogeneity, this study focused on recalled parental behavior and aimed to empirically identify if there are subgroups of SAD based on recalled parental behavior by means of cluster analysis. Further, the study investigated whether those subgroups differed on clinical, trauma, and personality variables. METHODS: This study included 505 individuals diagnosed with SAD and 98 adult controls who were asked to fill out the Parental Bonding Instrument (PBI), the Adverse Childhood Experiences Questionnai
