Hypertonic saline and heart failure: "sodium-centric" or "chlorine-centric"?

Up to 50% of patients admitted for heart failure (HF) have congestion at discharge despite diagnostic and therapeutic advances. Both persistent congestion and diuretic resistance are associated with worse prognosis. The combination of hypertonic saline and loop diuretic has shown promising results in different studies. However, it has not yet achieved a standardized use, partly because of the great heterogeneity in the concentration of sodium chloride, the dose of diuretic or the amount of sodium in the diet. Classically, the movement of water from the intracellular space due to an increase in

Blockers of K+/Cl- transporter/channels diminish proliferation of osteoblastic cells.

In the preset report, we studied if blockers of the K+/Cl- cotransporter, and Cl- and K+ channels modify the proliferation of MC3T3-E1 osteoblastic cells. A blocker of the K+/Cl- cotransporter, DIOA (100 and 500 microM), diminished the cell proliferation. NPPB (100 and 300 microM, a blocker of Cl- channel) and quinine (100 and 500 microM, a blocker of K+ channel) also suppressed the proliferation of MC3T3-E1 cells. These blockers of the K+/Cl- cotransporter/channels (DIOA, NPPB and quinine) would increase the intracellular Cl- concentration ([Cl-]i). On the other hand, our previous study (Bioc

Serum chloride and the response to combining loop and thiazide diuretics for acute heart failure: the ALCALOTIC study.

Chloride may play an important role in the pathophysiology of heart failure (HF) as it is associated with HF prognosis and diuretic resistance. This study evaluates the diuretic response across serum chloride in patients admitted with acute heart failure (AHF) and treated with loop and thiazide diuretics. This is a subanalysis of the ALCALOTIC study, a prospective, multicentre, observational cohort study that included 665 patients admitted for AHF at 30 clinical sites in Spain. We analysed 386 patients after excluding those without baseline chloride or in-hospital weight loss data. The endpoin

Pharmacology of diuretics.

The diuretics in our therapeutic armamentarium have predictable effects based on their nephron sites of action. All but spironolactone must reach the lumen or urinary side of the nephron to exert their effects. Thus, in settings of decreased renal function, doses must be increased to deliver more diuretic into the urine. In other edematous disorders, such as congestive heart failure (CHF) and cirrhosis, adequate amounts of diuretic reach the site of action if renal function is satisfactory. Diminished response in these conditions is caused by a decrease in the sensitivity of the nephron to the

Combination Diuretic Therapy to Counter Renal Sodium Avidity in Acute Heart Failure: Trials and Tribulations.

In contrast to significant advances in the management of patients with chronic heart failure over the past few years, there has been little change in how patients with acute heart failure are treated. Symptoms and signs of fluid overload are the primary reason for hospitalization of patients who experience acute decompensation of heart failure. Intravenous loop diuretics remain the mainstay of therapy in this patient population, with a significant subset of them showing suboptimal response to these agents leading to incomplete decongestion at the time of discharge. Combination diuretic therapy

Compensatory post-diuretic renal sodium reabsorption is not a dominant mechanism of diuretic resistance in acute heart failure.

AIMS: In healthy volunteers, the kidney deploys compensatory post-diuretic sodium reabsorption (CPDSR) following loop diuretic-induced natriuresis, minimizing sodium excretion and producing a neutral sodium balance. CPDSR is extrapolated to non-euvolemic populations as a diuretic resistance mechanism; however, its importance in acute decompensated heart failure (ADHF) is unknown. METHODS AND RESULTS: Patients with ADHF in the Mechanisms of Diuretic Resistance cohort receiving intravenous loop diuretics (462 administrations in 285 patients) underwent supervised urine collections entailing an i

Loop diuretics are K+-sparing in the presence of a low-Na+, high-K+ diet.

Under most conditions, loop diuretics are K+-wasting, requiring potassium supplementation. In this issue, Wang and colleagues demonstrate that in mice fed a low-Na+, high-K+ diet, loop diuretics, in contrast, are K+-sparing. This observation suggests that possible elevations in plasma K+ should be monitored when using a loop diuretic with a low-Na+, high-K+ diet, particularly when in combination with a potassium supplement.

Diuretic management in heart failure.

Many of the primary clinical manifestations of heart failure are due to fluid retention and congestion, and therefore treatments targeting congestion play a central role in heart failure management. Diuretic therapy remains the cornerstone of congestion treatment, and diuretics are prescribed to the majority of heart failure patients. Despite this ubiquitous use, there is limited evidence from prospective randomized studies to guide the use of diuretics. Some observational data have suggested that diuretics may actually be harmful in heart failure, potentially contributing to worsening renal f

Combination of loop diuretics with thiazide-type diuretics in heart failure.

Volume overload is an important clinical target in heart failure management, typically addressed using loop diuretics. An important and challenging subset of heart failure patients exhibit fluid overload despite significant doses of loop diuretics. One approach to overcome loop diuretic resistance is the addition of a thiazide-type diuretic to produce diuretic synergy via "sequential nephron blockade," first described more than 40 years ago. Although potentially able to induce diuresis in patients otherwise resistant to high doses of loop diuretics, this strategy has not been subjected to larg

Sex differences in solute transport along the nephrons: effects of Na+ transport inhibition.

Each day, ~1.7 kg of NaCl and 180 liters of water are reabsorbed by nephron segments in humans, with urinary excretion fine tuned to meet homeostatic requirements. These tasks are coordinated by a spectrum of renal Na+ transporters and channels. The goal of the present study was to investigate the extent to which inhibitors of transepithelial Na+ transport (TNa) along the nephron alter urinary solute excretion and how those effects may vary between male and female subjects. To accomplish that goal, we developed sex-specific multinephron models that represent detailed transcellular and paracell

Loop Diuretics Unique Mechanism of Action.

Loop diuretics, including torsemide, furosemide, bumetanide, and piretanide, act by inhibiting the sodium-potassium-chloride (Na+/K+/2Cl-) cotransporter in the thick ascending limb of the loop of Henle within the nephron. This mechanism is pivotal in managing fluid retention associated with conditions such as heart failure, cirrhosis, chronic kidney disease, and hypertension. A comprehensive understanding of how these diuretics uniquely target this transporter provides crucial insights into effectively addressing fluid overload across diverse clinical conditions.
