Sotalol in neonates for arrhythmias: Dosing, safety, and efficacy.

INTRODUCTION: Various agents may be utilized to manage supraventricular tachycardia (SVT) in neonates and infants. Recently, sotalol has piqued interest given its reported success in managing neonates and infants with SVTs, especially with the intravenous formulation. While the manufacturer recommends using an age-related nomogram in neonates and young infants to guide doses, clinical reports describe various dosing based on weight (mg/kg) or on body surface area (BSA) in mg/m2 . Given the reported variation in clinical practice with regard to dosing in neonates, there is a gap in the literatu

Sotalol: a new agent for the treatment of ventricular arrhythmias.

Sotalol was developed as a nonselective beta-blocker in the 1960s for the treatment of hypertension and later for cardiac risk management after myocardial infarction. Extensive research has since well described class III type electrophysiologic effects on the repolarization of myocardial fibers. Sotalol prolongs and homogenizes ventricular refractoriness, resulting in good antifibrillatory/antitachycardia protection. The unique combination of beta-blockade and antiarrhythmic effects probably will promote sotalol's use in postmyocardial infarction patients with ventricular tachycardia and sudde

Electrophysiologic effects of sotalol on the immature mammalian heart.

Sotalol is a beta-blocker with class III antiarrhythmic properties that has recently been used in children for the treatment of supraventricular and ventricular arrhythmias. However, little is known about its electrophysiologic effects on the immature heart. Using intracardiac electrocardiographic recordings and stimulation techniques, 15 canine neonates (8-15 days) and 15 adult mongrel dogs were studied with cumulative doses of sotalol (0.5, 1, 2, and 4 mg/kg plus an additional dose of 8 mg/kg for neonates). Heart rate decreased significantly in the two groups, but more in adult dogs (-43% in

[Intravenous sotalol in the acute treatment of supraventricular tachycardias].

The effects of intravenous sotalol (0.5 mg/kg in 6 minutes) were studied in 23 patients with supraventricular tachycardia (ventricular rate greater than 120 bpm) after failure of placebo (isotonic dextrose). Thirteen patients had atrial flutter or fibrillation and 10 a junctional tachycardia of recent onset. Sinus rhythm was restored in 4 of the patients with atrial flutter or fibrillation and the ventricular rate was slowed significantly in 3 patients (less than 100 bpm). In 3 other patients the ventricular rate decreased (31 to 35 p. 100) but remained above 100 bpm. Sinus rhythm was restored

Long-term sotalol therapy in patients with arrhythmias.

Sotalol is a pure beta-adrenergic receptor antagonist. The present study was divided into a first and second therapy period. A total of 38 patients with atrial or ventricular arrhythmias were included in the first therapy period. After a drug-free period of approximately two months, 14 of the 38 patients entered the second therapy period and were given oral sotalol. During the two treatment periods, oral sotalol was given in doses ranging from 40 to 480 mg/day for 0.5 to 11 months in the first period and for four to nine months in the second period. Oral sotalol decreased or abolished arrhythm

[Transplacental cardioversion of fetal supraventricular tachycardia using sotalol].

Intrauterine fetal supraventricular tachycardia (FSVT) is a rare condition which is connected with organic heart disease in 4-10 per cent. Intrauterine pharmacologic cardioversion with digoxin and verapamil has been recommended previously. However, during pregnancy standard doses of digoxin result in suboptimal concentrations in the maternal serum and fetal therapeutic concentrations might demand doses inconvenient to the mother. Verapamil is not always effective. In this report we describe a case of FSVT. The arrhythmia was treated by administering sotalol to the mother after verapamil had fa

Expedited loading with intravenous sotalol is safe and feasible-primary results of the Prospective Evaluation Analysis and Kinetics of IV Sotalol (PEAKS) Registry.

BACKGROUND: Loading of oral sotalol for atrial fibrillation requires 3 days, frequently in the hospital, to achieve steady state. The Food and Drug Administration approved loading with intravenous (IV) sotalol through model-informed development, without patient data. OBJECTIVE: We present results of the first multicenter evaluation of this recent labeling for IV sotalol. METHODS: The Prospective Evaluation Analysis and Kinetics of IV Sotalol (PEAKS) Registry was a multicenter observational registry of patients undergoing elective IV sotalol load for atrial arrhythmias. [Sotalol--atrial fibrillation, reduced renal function and sudden death].

Sotalol is a betablocker with additional antiarrythmic properties. Its pharmacokinetic properties may lead to increased serum levels of the drug in patients with reduced kidney function. We report on a case in which kidney failure in a patient using sotalol for paroxysmal atrial fibrillation gave rise to torsade de pointes ventricular tachycardia and, eventually, sudden death. Gradual prolongation of the QT interval suggested an overdose of sotalol. Sotalol has been commonly used in cases of angina pectoris and hypertension. The recently published SWORD study has, however, changed the indicati

The acute cardiac electrophysiological effects of intravenous sotalol hydrochloride.

The cardiac electrophysiological effects of sotalol were studied in ten patients (pts) aged 20--65 years undergoing intracardiac stimulation studies for palpitations (7 pts) or dizzy spells (3 pts). The following measurements were made: 1. basic sinus cycle length (SCL): 2. SINUS NODE RECOVERY TIME (SNRT) following overdrive pacing; 3. intra-atrial (PA), atrio-His (AH) and His-ventricular (HV) conduction intervals during regular atrial pacing; 4. effective refractory periods of the atria (AERP), AV node (AVERP) and ventricular myocardium (VERP). AV nodal functional refractoriness (AVFRP) was a

Comparison of sotalol versus amiodarone in maintaining stability of sinus rhythm in patients with atrial fibrillation (Sotalol-Amiodarone Fibrillation Efficacy Trial [Safe-T]).

The Sotalol-Amiodarone Fibrillation Efficacy Trial (SAFE-T) is a randomized, double-blind, multicenter, placebo-controlled trial in which the effects of sotalol and amiodarone in maintaining stability of sinus rhythm are being examined in patients with persistent atrial fibrillation at 20 Veterans Affairs medical centers. The time to the occurrence of atrial fibrillation or flutter in patients with atrial fibrillation converted to sinus rhythm is the primary outcome measure, with a number of parameters as secondary end points. SAFE-T had randomized 665 patients when enrollment terminated on Oc

Treatment of ventricular arrhythmias with oral sotalol in four horses.

Sustained ventricular arrhythmias (VAs) in horses can require medical intervention. Prompt pharmacological treatment is indicated when the arrhythmia leads to hemodynamic or electrical instability. Established antiarrhythmic treatment of VA in horses includes lidocaine and magnesium, administered parenterally. Other antiarrhythmic agents are used; however, concerns regarding adverse effects, financial restrictions, logistics of administration, and lack of access often limit their use. While anecdotal reports of oral administration of sotalol for treatment of VA in horses exist, sotalol has pri

Managing acute supraventricular tachycardia and the role of intravenous sotalol.

PURPOSE OF REVIEW: The initial approach to stable supraventricular tachycardia (SVT) in children is either adenosine or vagal maneuvers. While automatic tachycardias do not respond to such an approach, even reentrant arrythmias will fail to terminate with adenosine or vagal maneuvers. Little consensus exists on the ideal second line antiarrhythmic medication for stable SVT. This article reviews the current approach to stable SVT and early pediatric studies using intravenous (i.v.) sotalol. RECENT FINDINGS: The development of i.v. sotalol has spawned enthusiasm for a novel antiarrhythmic for r

Arrhythmias in Patients With Heart Failure Prescribed Dofetilide or Sotalol.

BACKGROUND: Heart failure (HF) with reduced ejection fraction (HFrEF) is a risk factor for drug-induced arrhythmias. It is unknown whether HF with preserved ejection fraction (HFpEF) also increases the risk. OBJECTIVES: The purpose of this study was to determine if the risk of ventricular tachycardia (VT) and sudden cardiac arrest (SCA) is increased in patients with HFpEF prescribed dofetilide or sotalol. METHODS: Using Medicare claims and pharmacy benefits from 2014 to 2016, we identified patients taking dofetilide or sotalol and non-dofetilide/sotalol users among 3 groups: HFrEF (n = 26,17

Efficacy and safety of dofetilide and sotalol in patients with hypertrophic cardiomyopathy.

BACKGROUND: Professional society practice guidelines conflict regarding their recommendations of dofetilide (DOF) and sotalol (STL) for treatment of arrhythmias in hypertrophic cardiomyopathy (HCM), and supporting data is sparse. We aim to assess safety and efficacy of DOF and STL on arrhythmias in HCM. METHODS: This was an observational study of HCM patients treated with DOF or STL for atrial fibrillation (AF) and ventricular arrhythmias (VA). Outcomes of drug discontinuation and arrhythmia recurrence were compared at 1 year and latest follow-up by Kaplan-Meier analysis. Predictors of drug f

Treatment of fetal tachycardia with sotalol: transplacental pharmacokinetics and pharmacodynamics.

OBJECTIVES: The aim of this study was to investigate the pharmacokinetics and pharmacodynamics of sotalol in the treatment of fetal tachycardia. BACKGROUND: Maternally administered, intrauterine therapy of fetal tachycardia is dependent on the transplacental passage of the antiarrhythmic agent. METHODS: In a prospective study of patients treated for fetal tachycardia with sotalol, concentrations of sotalol were determined in maternal and umbilical blood and in amniotic fluid, and the relationship between these concentrations and the occurrence of conversion to sinus rhythm was investigated.

Usefulness of oral sotalol for the treatment of junctional ectopic tachycardia.

An infant with junctional ectopic tachycardia diagnosed at three weeks of age was treated with oral sotalol. At the first echocardiogram the shortening fraction was decreased. After one day of treatment the ventricular rate decreased from 230 to 130 beats per minute. Five months later sinus rhythm was restored, shortening fraction was normal and sotalol was withdrawn without relapse.
