Acceptability ratings and intelligibility scores of alaryngeal speakers by three listener groups.

Acceptability and intelligibility of three groups of alaryngeal speakers were examined: four oesophageal speakers, four tracheo-oesophageal puncture speakers using a tracheostoma valve and four tracheo-oesophageal puncture speakers using digital occlusion. Speakers were video-taped producing samples from the Assessment of Intelligibility of Dysarthric Speech, in addition to a section of spontaneous speech. Three groups of listeners were used: nine speech and language therapists, five ENT surgeons and ten naïve listeners. Specific variables including fluency, rate, quality, intensity, extraneou

Clinical application of electrolaryngograph for speech rehabilitation in near-total laryngectomy with myo-mucosal valved neoglottis.

Near-total laryngectomy with myomucosal valved neoglottis is the most recent of the various surgical procedures for speech rehabilitation. The electrolaryngographic wave form (Lx), which was recorded initially to explore the site and mechanism of phonatory function of the neoglottis, has been successfully used in speech rehabilitation of these patients. In a pilot study it was tried as a visual feedback. The results confirm its utility for speech rehabilitation of these laryngectomees.

[Surgical rehabilitation of speech after total laryngectomy: the Staffieri techniques (author's transl)].

Surgical speech rehabilitation after total laryngectomy by the three procedures of Prof. Staffieri is based on the principle of establishing a short fistula lined by pharyngeal mucosa between trachea and hypopharynx. It permits voluntary air pressure control for voice production while preventing aspiration during deglutition. In a single-stage procedure the "neoglottis phonatoria" is established on the occasion of the laryngectomy at the top of the trachea. In cases of prevoius laryngectomy a shunt is established between the trachea dorsal wall and the oesophagus, either cranially (direct inte

Electrolarynx in voice rehabilitation.

OBJECTIVE: Patients of laryngeal cancer who have undergone the surgical removal of the entire larynx suffer the loss of phonation. Electrolarynx (EL) speech is the most commonly adopted alaryngeal phonation. However, EL speech is notorious of the sound quality being monotonic and robotic with the lack of pitch control and the presence of the radiated noise. This paper provides a review of modalities in EL speech as well as introducing the technologies to control the pitch and reduce the noise of the device. METHODS: Improvements of EL speech quality have been divided into two parts: improving

Aerodynamic characteristics associated with oesophageal and tracheoesophageal speech of Cantonese.

The present study investigated the aerodynamic differences between standard oesophageal (SE) and tracheoesophageal (TE) speech. Airflow and air pressure values below the pharyngoesophageal segment were obtained from 10 SE and 12 TE superior speakers of Cantonese. Airflow data were directly measured from sustained vowels, and sub-pharyngoesophageal segment pressure was estimated from /ip(h)ip(h)i/ syllables produced by the alaryngeal speakers. Results indicated that SE speech was associated with a lower rate of airflow and a higher pressure below the pharyngoesophageal segment than TE speech. S

Voice restoration after laryngectomy.

Speech therapists have traditionally played a major part in voice restoration after laryngectomy. Nurses have a potentially complementary role to play, which is explored in this article. The three main methods of voice restoration are outlined and the nurse's role in the management of laryngectomy patients is discussed.

Silent Speech Recognition as an Alternative Communication Device for Persons with Laryngectomy.

Each year thousands of individuals require surgical removal of their larynx (voice box) due to trauma or disease, and thereby require an alternative voice source or assistive device to verbally communicate. Although natural voice is lost after laryngectomy, most muscles controlling speech articulation remain intact. Surface electromyographic (sEMG) activity of speech musculature can be recorded from the neck and face, and used for automatic speech recognition to provide speech-to-text or synthesized speech as an alternative means of communication. This is true even when speech is mouthed or sp

Practice of laryngectomy rehabilitation interventions: a perspective from Hong Kong.

PURPOSE OF REVIEW: To review the current practice of rehabilitation for laryngectomees in Hong Kong. RECENT FINDINGS: Factors affecting the quality of life of laryngectomees include their performance in speech restoration, the presence of complications of treatment, as well as the availability of psycho-social support. In Hong Kong, more than 90% of laryngectomees have speech restoration by various means, the commonest of which being tracheo-oesophageal puncture and electrolaryngeal speech. However, they face special problems in communication using the current alaryngeal speech modalities, as

Rate-dependent perception of aspiration and pre-aspiration in Icelandic.

Speech segments are highly context-dependent and acoustically variable. One factor that contributes heavily to the variability of speech is speaking rate. Some speech cues are temporal in nature-that is, the distinctions that they signify are defined over time. How can temporal speech cues keep their distinctiveness in the face of extrinsic transformations, such as those wrought by different speaking rates? This issue is explored with respect to the perception, in Icelandic, of Voice Onset Time as a cue for word-initial stop voicing, word-initial aspiration as a cue for [h], and Voice Offset T

Articulatory Contact Pressure during Bilabial Plosive Production in Esophageal and Tracheoesophageal Speech.

OBJECTIVES: The present study investigated the articulatory contact pressure during the production of bilabial plosives by esophageal (ES), tracheoesophageal (TE), and laryngeal speakers. METHODS: The peak contact pressure (PCP) during bilabial plosive production of /CVCVCVCVCV/ syllable strings of /p/ and /ph/ was obtained from 10 ES, 10 TE, and 10 laryngeal speakers of Cantonese. PCP values were obtained by using a pressure transduction system (Iowa Oral Performance Instrument) during speech production. RESULTS: The results showed that ES speakers exhibited a significantly greater PCP valu

Neoglottic reconstruction after total laryngectomy. Staffieri's techniques: a report of 18 cases.

Mario Staffieri and associates developed a technique for reconstruction of a phonatory neoglottis in 1970. Since then Staffiere has performed over 100 of these procedures with approximately a 90% success rate. The technique has been used in 18 cases at The University of Texas Medical Branch in Galveston with certain variations. All patients received a wide field total laryngectomy for T3 or T4 endolaryngeal or pyriform sinus carcinomas. Of these 18 patients, two did not have neoglotic fistulas created. Of the 16 patients who remained in the series, 12 have a good voice. One died of carcinoma o

The Performance of Automatic Speaking Valves and Fixation Devices Enabling Hands-Free Speech Following Total Laryngectomy-A Systematic Review.

BACKGROUND: The percentage of daily automatic speaking valve use (AU) in laryngectomized patients has remained at ~25% since their invention. To increase this percentage, understanding the performance of existent devices is essential. Therefore, we reviewed studies reporting AU, fixation device lifetime (FDL), compliance, and voice and speech assessment outcomes (VSAOs). METHODS: Studies published until 23 April 2025 were included. Quality assessment was performed using RoB2, ROBINS-I, and JBI tools. RESULTS: Twenty studies were included, which were of poor methodological quality. AU and FDL
