Speech Function Following Deep Brain Stimulation of the Caudal Zona Incerta: Effects of Habitual and High-Amplitude Stimulation.

Purpose Deep brain stimulation (DBS) is often successful in alleviating motor symptoms of essential tremor (ET); however, DBS may also induce adverse speech effects. The caudal zona incerta (cZi) is a promising DBS target for tremor, but less is known about the consequences of cZi DBS for speech. This preliminary study examined how habitual cZi DBS and cZi stimulation at high amplitudes may affect speech function in persons with ET. Method Fourteen participants with ET were evaluated: off stimulation, on habitual cZi DBS, and with unilateral cZi stimulation at increasing stimulation amplitudes

Efficacy of speech intervention using electropalatography with a cochlear implant user.

Electropalatography (EPG) has become relatively well established as a safe and convenient technique for use in the assessment, diagnosis and treatment of children and adults with articulation disorders. EPG's wide applicability is reflected in the range of different cases that has been researched in recent years. Some research has been carried out using EPG therapy for deaf individuals who use hearing aids, however there are no similar studies for cochlear implant users. The purpose of this single case study is to explore the technique of EPG as a therapeutic intervention to treat voiceless ve

Familial aggregation of phonological disorders: results from a 28-year follow-up.

This investigation is a follow-up to a longitudinal speech and educational outcome study involving approximately 400 normally developing children that was initiated in 1960. From this database, two groups of subjects (now aged 32-34), their spouses, and all of their offspring over the age of 3:0 (years:months) completed a battery of cognitive-linguistic and interview measures. One group (probands) consisted of 24 adults with a documented history of a moderate phonological-language disorder that persisted through at least the end of the first grade. The other group (controls) consisted of 28 ad

Assessing the treatment effects in apraxia of speech: introduction and evaluation of the Modified Diadochokinesis Test.

BACKGROUND: The number of reliable and valid instruments to measure the effects of therapy in apraxia of speech (AoS) is limited. AIMS: To evaluate the newly developed Modified Diadochokinesis Test (MDT), which is a task to assess the effects of rate and rhythm therapies for AoS in a multiple baseline across behaviours design. METHODS: The consistency, accuracy and fluency of speech of 24 adults with AoS and 12 unaffected speakers matched for age, gender and educational level were assessed using the MDT. The reliability and validity of the instrument were considered and outcomes compared wit

Sequencing Deficits and Phonological Speech Errors, But Not Articulation Errors, Predict Later Literacy Skills.

PURPOSE: Speech sound disorder (SSD) in conjunction with a language disorder has been associated with poor literacy acquisition; however, no study has evaluated whether articulation, phonological, or sequencing skills are differentially related to reading skills. Therefore, this study examined the relationship between speech error types at ages 5-6 years and literacy at ages 7-9 years. Phonological errors were hypothesized to predict phonological awareness (PA) and literacy even while accounting for other speech error types and language skills. METHOD: One hundred twenty-three children, 86 wi

Sensory integration dysfunction affects efficacy of speech therapy on children with functional articulation disorders.

BACKGROUND: Articulation disorders in young children are due to defects occurring at a certain stage in sensory and motor development. Some children with functional articulation disorders may also have sensory integration dysfunction (SID). We hypothesized that speech therapy would be less efficacious in children with SID than in those without SID. Hence, the purpose of this study was to compare the efficacy of speech therapy in two groups of children with functional articulation disorders: those without and those with SID. METHOD: A total of 30 young children with functional articulation dis

Task-dependence of articulator synergies.

In speech production, the motor system organizes articulators such as the jaw, tongue, and lips into synergies whose function is to produce speech sounds by forming constrictions at the phonetic places of articulation. The present study tests whether synergies for different constriction tasks differ in terms of inter-articulator coordination. The test is conducted on utterances [ɑpɑ], [ɑtɑ], [ɑiɑ], and [ɑkɑ] with a real-time magnetic resonance imaging biomarker that is computed using a statistical model of the forward kinematics of the vocal tract. The present study is the first to estimate th

Brain Cortex Activity in Children With Anterior Open Bite, Speech and Swallowing Disorders.

BACKGROUND: Anterior open bite (AOB) affects 16.5% of children aged 2-16 years, significantly impacting dental occlusion and oral function. Among various contributing factors, improper tongue posture has been implicated. This study aims to assess neuronal cortical activation differences in children with AOB compared to those without, focusing on rest, speech and swallowing disorders. OBJECTIVE: To compare the prevalence of articulation disorders, swallowing occurrences, tongue rate of motion ratio, and brain activation patterns at rest and during deglutition and phonation in children with and

Development of a speech treatment program for a client with essential vocal tremor.

Vocal tremor is characterized by involuntary rhythmic modulations of pitch and loudness and is best perceived during sustained phonation of vowels. It is most often present in individuals affected by neurogenic disorders such as Parkinson's disease, amyotrophic lateral sclerosis, spinal muscular dystrophy, spasmodic dysphonia, and essential tremor. Vocal tremor does not appear to be responsive to systemic pharmaceutical management but may benefit from injection of botulinum toxin (i.e., Botox) into affected musculature. However, many individuals do not tolerate the potential side effects of se

Evaluation of articulation of Turkish phonemes after removable partial denture application.

In this study, the adaptation of patients to removable partial dentures was evaluated related to articulation of Turkish phonemes. Articulation of /t,d.n,l,r/, /g,k/, /b,p,m/ and /s,z,ş,v,f,y,j,h,c/ phonemes were evaluated by three speech pathologists, on records taken from 15 patients before the insertion of a removable partial denture, just after insertion, and one week later. The test consisted of evaluation of phoneme articulation of independent syllables in terms of distortion, omission, substitution, mass effect, hypernasality and hyponasality. Data were evaluated with Cochrane Q. McNema

A further analysis for developmental apraxia of speech in children with defective articulation.

This paper reports a further analysis of behaviors that might distinguish developmental apraxia of speech from "functional" defective articulation. Closely following a study of Yoss and Darley, 30 children with moderately-to-severely defective articulation but with normal hearing, intelligence, and language development and with no apparent organic etiology for their disability were matched with a normal-speaking control group. Both groups were given Yoss and Darley's battery of speech and nonspeech tasks. Yoss and Darley's analysis procedure was applied to the data. Their finding of significan

Influence of an Intensive Speech Therapy Program on the Speech of Individuals with Cleft Lip and Palate.

Introduction  Compensatory articulations are speech disorders due to the attempt of the individual with cleft palate/velopharyngeal dysfunction to generate intraoral pressure to produce high-pressure consonants. Speech therapy is the indicated intervention for their correction, and an intensive speech therapy meets the facilitating conditions for the correction of glottal stop articulation, which is the most common compensatory articulation. Objective  To investigate the influence of an intensive speech therapy program (ISTP) to correct glottal stop articulation in the speech of individuals wi

Integrating perceptual and instrumental procedures in the assessment of velopharyngeal insufficiency.

Although technology for the instrumental assessment of velopharyngeal function has advanced, management decisions still too often depend only on subjective perceptual analyses. Both perceptual and instrumental appraisal are necessary for adequate assessment. Instrumental assessment is essential prior to surgical or other long-term management. The assessment is only as valid as the instruments used. If such instruments are not readily available, clinicians should refer patients to centers where they are available and these examinations are routinely performed.

Palatoplasty: a comparative study.

A comparative analysis of 275 patients who underwent palatoplasty between 1961 and 9171 is presented. Of these, 149 patients had a Langenbeck palatoplasty and 188 a V-Y pushback palatoplasty. Speech results in these patients were carefully assessed by experience speech pathologists using conversational and structured (word articulation tests) speech, manometric ratios, and radiographic evidence of palatal movement. Over all, 73% of the V-Y pushback patients attained acceptable speech compared to 56% in the Langenbeck category. Patients with clefts of the soft palate only had the highest incide

Improving speech characteristics of young adults with congenital dysarthria: An exploratory study comparing articulation training and the Beatalk method.

INTRODUCTION: This exploratory study compared the effects of two speech therapy approaches on speech characteristics of young adults with congenital dysarthria resulting from various etiologies: a) articulation training focusing on consonant articulation exercises at various levels (isolation, syllables, and words), and b) the Beatalk method, based on human beatboxing, i.e., producing various instrumental sounds in an a-cappella musical context. Both interventions were designed to increase participants' speech intelligibility. METHODS: Twelve adults with congenital dysarthria and reduced spee
