Speech and language therapists' insights into severity of speech sound disorders in children for developing the speech sound disorder severity construct.

BACKGROUND: Children with speech sound disorders (SSD) are at higher risk of communication breakdown, but the impact of having an SSD may vary from child to child. Determining the severity of SSD helps speech-language therapists (SLTs) to recognise the extent of the problem and to identify and prioritise children who require intervention. AIMS: This study aimed to identify severity factors for SSD in order to develop a multifactorial Speech Sound Disorder Severity Construct (SSDSC) using SLTs' views and the International Classification of Functioning, Disability and Health (ICF). METHOD: In

Perceptual Ratings Predict Speech Inversion Articulatory Kinematics in Childhood Speech Sound Disorders.

PURPOSE: This study evaluated whether articulatory kinematics, inferred by articulatory phonology speech inversion neural networks, aligned with perceptual ratings of /ɹ/ and /s/ in the speech of children with speech sound disorders. METHOD: Articulatory phonology vocal tract variables were inferred for 5,961 utterances from 118 children and three adults, aged 2.25-45 years. Perceptual ratings were standardized using the novel 5-point PERCEPT Rating Scale and training protocol. Two research questions examined if the articulatory patterns of inferred vocal tract variables aligned with the perc

Non-speech oral motor treatment for children with developmental speech sound disorders.

BACKGROUND: Children with developmental speech sound disorders have difficulties in producing the speech sounds of their native language. These speech difficulties could be due to structural, sensory or neurophysiological causes (e.g. hearing impairment), but more often the cause of the problem is unknown. One treatment approach used by speech-language therapists/pathologists is non-speech oral motor treatment (NSOMT). NSOMTs are non-speech activities that aim to stimulate or improve speech production and treat specific speech errors. For example, using exercises such as smiling, pursing, blow

Speech and language skills of parents of children with speech sound disorders.

PURPOSE: This study compared parents with histories of speech sound disorders (SSD) to parents without known histories on measures of speech sound production, phonological processing, language, reading, and spelling. Familial aggregation for speech and language disorders was also examined. METHOD: The participants were 147 parents of children with SSD (58 fathers and 89 mothers) who were directly tested and interviewed for family history of disorders. RESULTS: Thirty-six parents (18 mothers and 18 fathers) reported enrollment in speech therapy as children for SSD. Parents with a history of s

Speech and motor speech disorders and intelligibility in adolescents with Down syndrome.

The goal of this research was to assess the support for motor speech disorders as explanatory constructs to guide research and treatment of reduced intelligibility in persons with Down syndrome (DS). Participants were the 45 adolescents with DS in the prior paper who were classified into five mutually-exclusive motor speech classifications using the Speech Disorders Classification System. An ordinal index classified participants' percentage of intelligible words in conversation as High (≥ 85%), Moderate (80% - 84.9%), or Low (< 80%). Statistical analyses tested for significant differences in i

Speech and voice disorders in patients with psychogenic movement disorders.

Psychogenic speech and voice disorders (PSVDs) may occur in isolation but more typically are encountered in the setting of other psychogenic disorders. We aimed to characterize the phenomenology, frequency, and correlates of PSVDs in a cohort of patients with psychogenic movement disorders (PMDs). We studied 182 consecutive patients with PMDs, 30 of whom (16.5 %) also exhibited PSVD. Stuttering was the most common speech abnormality (n = 16, 53.3 %), followed by speech arrests (n = 4, 13.3 %), foreign accent syndrome (n = 2, 6.6 %), hypophonia (n = 2, 6.6 %), and dysphonia (n = 2, 6.6 %). Four

[Speech and Language Disorders in the Elderly].

Cerebrovascular insults and neurodegenerative diseases are the main causes of acquired speech and language disorders. A massive increase in the prevalence of both is evident from the age of 65 years. By the age of 90 years, more than 10 % of the population would have experienced a stroke and 30 % would have suffered from neurodegenerative diseases. Common risk factors are adiposity, hypertension and diabetes. The term aphasia subsumes acquired language disorders that have to be further classified in order to optimize diagnostics and therapy. Also prognosis depends on whether the disease is pro

[Speech apraxia--an apraxic disorder?].

This article presents arguments supporting the view that apraxia of speech is an apraxic disorder. In a first part the most salient clinical signs of the syndrome are reviewed. Among these, the symptom of inconsistent distortions and categorical aberrations of speech sounds is emphasized for its relevance in differentiating apraxia of speech from the dysarththrias on the one hand and from aphasic phonological disturbances on the other. In a second part, the diagnostic criteria of apraxia of speech are compared to those of oral (nonverbal) apraxia. In this comparison, a full correspondence betw

Genetic pathways involved in human speech disorders.

Rare genetic variants that disrupt speech development provide entry points for deciphering the neurobiological foundations of key human capacities. The value of this approach is illustrated by FOXP2, a transcription factor gene that was implicated in speech apraxia, and subsequently investigated using human cell-based systems and animal models. Advances in next-generation sequencing, coupled to de novo paradigms, facilitated discovery of etiological variants in additional genes in speech disorder cohorts. As for other neurodevelopmental syndromes, gene-driven studies show blurring of boundarie

[Speech fluency disorders in childhood and adolescence].

Persistent speech fluency disorders, mostly stuttering and less frequently cluttering, occur in approximately 1% of children and adolescents. They considerably impair the social participation and quality of life of those affected. The German interdisciplinary evidence-based S3 guidelines provide information about the pathogenesis, diagnostics and treatment of speech fluency disorders and contain a systematic review on the efficacy of stuttering treatment. For preschool children the Lidcombe therapy shows the best evidence of efficacy. Strong evidence also exists for an indirect approach. For a

Motor and speech disorders in classic galactosemia.

Purpose To test the hypothesis that children with classic galactosemia and speech disorders are at risk for co-occurring strength and coordination disorders. Method This is a case-control study of 32 children (66% male) with galactosemia and neurologic speech disorders and 130 controls (50% male) ages 4-16 years. Speech was assessed using the Percentage of Consonants Correct (PCC) metric from responses to the Goldman-Fristoe Test of Articulation-2 and from a 5-min recorded speech sample, hand and tongue strength using the Iowa Oral Performance Instrument, and coordination using the Movement As

Oral breathing and speech disorders in children.

OBJECTIVE: To assess speech alterations in mouth-breathing children, and to correlate them with the respiratory type, etiology, gender, and age. METHOD: A total of 439 mouth-breathers were evaluated, aged between 4 and 12 years. The presence of speech alterations in children older than 5 years was considered delayed speech development. The observed alterations were tongue interposition (TI), frontal lisp (FL), articulatory disorders (AD), sound omissions (SO), and lateral lisp (LL). The etiology of mouth breathing, gender, age, respiratory type, and speech disorders were correlated. RESULTS:

[Speech disorders in right-sided brain stroke].

The clinical picture revealed that right-sided brain stroke is often accompanied by some speech disturbances. The present paper aims at the analysis of literature devoted to speech disturbances in right-sided brain stroke. The paper addresses issues on lateralization of speech functions, in particular the role of the right hemisphere in speech activity and structure of speech pathology in right hemispheric foci are specified. Such clinical variants of speech disturbances as aphasia, disprosodia, dysartria, mutism and stammering are analyzed in depth. Types of speech disorders as well as possib

Relations among speech, language, and reading disorders.

In this article, we critically review the evidence for overlap among three developmental disorders, namely speech sound disorder (SSD), language impairment (LI), and reading disability (RD), at three levels of analysis: diagnostic, cognitive, and etiological. We find that while overlap exists at all three levels, it varies by comorbidity subtype, and the relations among these three disorders are complex and not fully understood. We evaluate which comorbidity models can be rejected or supported as explanations for why and how these three disorders overlap and what new data are needed to better

[Speech and motor disorders in Rett syndrome].

Rett syndrome (RS) is a severe genetically conditioned disorder of an early childhood with an definite clinical phenotype in girls. Motor and speech disturbances are noted as the essential part of RS clinical picture. The variability of motor dysfunction and degree of speech deterioration were noted at different stages of the illness. The aim of the present investigation was to study dynamics of both speech and motor disturbances during 2-5 years of the course of the illness and to analyze a correlation of motor and speech functions' disorders. The study was performed in 50 girls with classica
