Application of the Challenge Point Framework During Treatment of Speech Sound Disorders.

Purpose The purpose of this article is to provide trial-by-trial practice performance data in relation to learning (outcome probe data) as collected from 18 treatment sessions provided to children with severe speech sound disorders. The data illustrate the practice-learning paradox: Specific, perfect practice performance is not required for speech production learning. Method We detailed how nine student speech-language pathologists (SSLPs) implemented and modified the motor learning practice conditions to reach a proposed challenge point during speech practice. Eleven participants diagnosed wi

Rapid movements at segment boundaries.

This paper reports on a one-to-one aspect of the articulatory-acoustic relationship, explaining how acoustic segment boundaries are a result of the rapid movements of the active articulators. In the acceleration profile, these are identified as acceleration peaks, which can be measured. To test the relationship, consonant and vowel segment durations are compared to articulatory posture intervals based on acceleration peaks, and time lags are measured on the alignment of the segment boundaries to the acceleration peaks. Strong relationships and short time lags are expected when the acceleration

Early diagnostic indicators of childhood apraxia of speech in young children with 7q11.23 duplication syndrome: preliminary findings.

Limited evidence for early indicators of childhood apraxia of speech (CAS) precludes reliable diagnosis before 36 months, although a few prior studies have identified several potential early indicators. We examined these possible early indicators in 10 toddlers aged 14-24 months at risk for CAS due to a genetic condition: 7q11.23 duplication syndrome (Dup7). Phon Vocalisation analyses were conducted on phonetic transcriptions of each child's vocalisations during an audio-video recorded 30-minute play session with a caregiver and/or a trained research assistant. The resulting data were compared

Perceptual Classification of Motor Speech Disorders: The Role of Severity, Speech Task, and Listener's Expertise.

PURPOSE: The clinical diagnosis of motor speech disorders (MSDs) is mainly based on perceptual approaches. However, studies on perceptual classification of MSDs often indicate low classification accuracy. The aim of this study was to determine in a forced-choice dichotomous decision-making task (a) how accuracy of speech-language pathologists (SLPs) in perceptually classifying apraxia of speech (AoS) and dysarthria is impacted by speech task, severity of MSD, and listener's expertise and (b) which perceptual features they use to classify. METHOD: Speech samples from 29 neurotypical speakers,

The Effect of Practice on Variability in Childhood Apraxia of Speech: A Multidimensional Analysis.

Purpose Variability has been interpreted in differing ways according to context (e.g., development, speech impairment, and learning). A challenge arises when interpreting variability in the context of learning for children with speech impairment characterized by high movement variability, as in childhood apraxia of speech (CAS). The objective of this study is to investigate changes in variability in CAS with practice in comparison to patterns seen in children with non-CAS speech sound disorders (SSD) and typical development. Method Speech production variability was examined in 24 children (5-6

Apraxia of speech and the study of speech production impairments: Can we avoid further confusion? Reply to Romani (2021).

We agree with Cristina Romani (CR) about reducing confusion and agree that the issues raised in her commentary are central to the study of apraxia of speech (AOS). However, CR critiques our approach from the perspective of basic cognitive neuropsychology. This is confusing and misleading because, contrary to CR's claim, we did not attempt to inform models of typical speech production. Instead, we relied on such models to study the impairment in the clinical category of AOS (translational cognitive neuropsychology). Thus, the approach along with the underlying assumptions is different. This res

Influence of gestures on the intelligibility and comprehensibility of utterances with stuttering events in individuals with Down syndrome.

INTRODUCTION: Individuals with Down syndrome (DS) often stutter, which can affect their speech intelligibility. Previous research has shown that manual signs can enhance speech intelligibility and comprehensibility. It remains unclear to what extent spontaneous hand gestures, such as iconic and beat gestures, may enhance intelligibility and/or comprehensibility during utterances with stuttering events. METHODS: Eleven individuals with DS who stutter provided video-recorded speech samples. From these samples, 60 utterances containing a stuttering event were selected. In half of them, the stutt

A phonetic investigation of single word versus connected speech production in children with persisting speech difficulties relating to cleft palate.

Objective : To investigate the phonetic and phonological parameters of speech production associated with cleft palate in single words and in sentence repetition in order to explore the impact of connected speech processes, prosody, and word juncture on word production across contexts. Participants : Two boys (aged 9 years 5 months and 11 years 0 months) with persisting speech impairments related to a history of unilateral cleft lip and palate formed the main focus of the study; three typical adult male speakers provided control data. Method : Audio, video, and electropalatographic recordings w

Acoustic assessment of dysphonia: Current practice and future directions - A narrative review.

Auditory-perceptual judgment remains the clinical gold standard for characterizing dysphonia; however, its inherent subjectivity has motivated the search for objective acoustic correlates. This narrative review synthesizes the historical and methodological progression of voice acoustics from early time-domain perturbation measures (such as jitter and shimmer) and various implementations of the harmonics-to-noise ratio toward cepstral-spectral analysis capable of robust assessment of connected speech. We summarize the rationale and evidence underpinning cepstral peak prominence and multivariate

Speech rate increase in primary progressive apraxia of speech and its cost on articulatory accuracy.

Impaired articulation (e.g., articulatory accuracy) and prosody (e.g., slow speech rate) are considered primary diagnostic criterions for apraxia of speech both in neurodegenerative and post-stroke contexts. The primary aim of this study was to investigate the ability of participants with primary progressive apraxia of speech (PPAOS), a neurodegenerative disease characterised by initially isolated progressive apraxia of speech, to increase speech rate and the interaction between articulatory accuracy and speech rate. The secondary aim was to investigate the effect of syllable frequency and str

Using ultrasound to quantify tongue shape and movement characteristics.

Objective : Previous experimental studies have demonstrated abnormal lingual articulatory patterns characterizing cleft palate speech. Most articulatory information to date has been collected using electropalatography, which records the location and size of tongue-palate contact but not the tongue shape. The latter type of data can be provided by ultrasound. The present paper aims to describe ultrasound tongue imaging as a potential tool for quantitative analysis of tongue function in speakers with cleft palate. A description of the ultrasound technique as applied to analyzing tongue movements

[Treatment of stuttering].

Stuttering in children will usually pass either with therapy or spontaneously. It remains permanent in approx. 20 to 30% of cases. Psychogenic and neurogenic stuttering have an adulthood onset. Behavioral methods aim to change the stutterer's attitude to his/her own speech or to control the extent of stuttering. By using assistive devices it is possible to slow down speech production and thereby increase its fluency. Due to the lack of suitable drugs, pharmacological therapy is seldom used. Moderate and severe stuttering should be treated also in children under school age.

Interventions That Target or Affect Voice or Speech Production During Public Speaking: A Scoping Review.

OBJECTIVES: Public speaking (PS) is frequently necessary in many professional, educational, and personal settings. Mastering this communication skill is particularly important in today's society. Training techniques for PS have been described in the literature. Given that PS anxiety affects performance, especially voice characteristics and speech fluency, the purpose of this scoping review is to examine, map, and narratively summarize the available evidence on PS interventions that target or affect voice or speech. METHODS: An extensive literature search was conducted in three bibliographic d

Vowel Acoustics as Predictors of Speech Intelligibility in Dysarthria.

PURPOSE: This study sought to determine if alternative vowel space area (VSA) measures (i.e., novel trajectory-based measures: vowel space hull area and vowel space density) predicted speech intelligibility to the same extent as two traditional vowel measures (i.e., token-based measures: VSA and corner dispersion) in speakers with dysarthria. Additionally, this study examined if the strength of the relationship between acoustic vowel measures and intelligibility differed based on how intelligibility was measured (i.e., orthographic transcriptions [OTs] and visual analog scale [VAS] ratings).

Speech rehabilitation in post-stroke aphasia using visual illustration of speech articulators: A case report study.

Recent studies on the remediation of speech disorders suggest that providing visual information of speech articulators may contribute to improve speech production. In this study, we evaluate the effectiveness of an illustration-based rehabilitation method on speech recovery of a patient with non-fluent chronic aphasia. The Ultraspeech-player software allowed visualization by the patient of reference tongue and lip movements recorded using ultrasound and video imaging. This method can improve the patient's awareness of their own lingual and labial movements, which can increase the ability to co

Disfluent whole-word repetitions in cluttering: Durational patterns and functions.

In this article, disfluent word-repetitions are analysed in cluttered and control speech. The main questions are the following: (1) Do different functions of word-repetitions occur in different ratio in cluttered and control speech? (2) Are there any differences between PWC and control speakers in durational parameters of disfluent word-repetitions? Results show that there are differences between the two groups of speakers in the types of word-repetitions, and in their durational parameters. In cluttered speech, the most frequent type of repetitions were covert self-repairs while in control sp
