Sphenoid mucocoele as a complication of fibrous dysplasia of the facial bones.

We report on a 28-year-old man with severe headache. Imaging studies included CT and MR. A large sphenoid mucocoele was demonstrated as well as findings compatible with fibrous dysplasia of the facial bones. Both disorders had been previously unknown. Imaging findings suggested that the sphenoid mucocoele was related to an obstruction caused by the expansile bone of fibrous dysplasia. These findings were confirmed surgically. Fibrous dysplasia with subsequent outflow obstruction is an extremely rare cause of sphenoid mucocoele development.

Direct intraoperative sclerotherapy of an aneurysmal bone cyst of the sphenoid.

A young boy presented with a symptomatic aneurysmal bone cyst of the left great wing of the sphenoid bone. Arterial embolization had failed to produce thrombosis, and the lesion could not be resected surgically. Direct intraoperative sclerotherapy resulted in immediate thrombosis of 80% of the volume of the vascular malformation with no progression of symptoms. Two years later, the symptoms had completely resolved, and CT scans showed total ossification of the lesion.

Impact of Sonic Hedgehog-dependent sphenoid bone defect on craniofacial growth.

OBJECTIVES: The main objective of this study was to evaluate how an apparently minor anomaly of the sphenoid bone, observed in a haploinsufficient mouse model for Sonic Hedgehog (Shh), affects the growth of the adult craniofacial region. This study aims to provide valuable information to orthodontists when making decisions regarding individuals carrying SHH mutation. MATERIALS AND METHODS: The skulls of embryonic, juvenile and adult mice of two genotypes (Shh heterozygous and wild type) were examined and measured using landmark-based linear dimensions. Additionally, we analysed the clinical c

Intraoperative frozen cytology of intraosseous cystic meningioma in the sphenoid bone.

Meningiomas in bone are rarely subjected to fine-needle aspiration diagnosis, and those arising in the skull bone with a cystic presentation are rare. A 24-year-old woman presented with subdural hemorrhage, and subsequent radiology depicted an osteolytic mass-like lesion in the sphenoid bone. Intraoperatively, a solid and cystic hemorrhagic lesion mimicking an aneurysmal bone cyst was observed in the sphenoid bone with dural tearing. Frozen cytology showed singly scattered or epithelioid clusters of round to elongated cells intermixed with many neutrophils. Tumor cells had bland-looking round

Two cases of incidental epidermoid cysts: sphenoid sinus and temporal bone.

Epidermoid cysts or tumors are uncommon. When they do occur they are usually large at presentation, and the initial symptoms usually involve the cranial nerves. Congenital epidermoids are believed to arise as a result of cellular dysfunction during embryogenesis, which leads to an abnormal migration of ectodermal cells. We present 2 cases of epidermoids that were identified incidentally at our institution during radiologic evaluations of head trauma; one was found in the sphenoid sinus and the other in the temporal bone. Both patients were treated surgically. We review the pathogenesis, diagno

[Leiomyosarcoma metastatizing to the sphenoid bone presenting exophthalmos: a case report].

A rare subcutaneous leiomyosarcoma metastatizing to the sphenoid bone and presenting exophthalmos is reported. A 56-year-old female presented with protrusion of the right eye and a slowly growing lump on the right temporal region. Six years previously, she had undergone removal of a subcutaneous mass in the back, which was histologically diagnosed in another hospital as leiomyosarcoma. She had undergone four other operations, including removal of local recurrences and a right renal metastasis. On admission, physical examination showed no neurological deficits. Craniogram revealed an osteolytic

Giant-cell tumor of the sphenoid bone. Review of 10 cases.

Giant-cell tumor rarely affects the sphenoid bone. Ten cases of this tumor seen at the Mayo Clinic are reviewed, bringing the number of cases in the world literature to 31. These tumors, which present in the second and third decades of life and are slightly more frequent in women than in men, may present with various symptoms, including headache, visual field defects, blindness, and diplopia. The symptom complex and roentgenographic findings are seldom, if ever, sufficiently characteristic to allow preoperative diagnosis. The microscopic differential diagnosis of giant-cell tumor of the spheno

The Developing Human Sphenoid Bone: Linking Embryological Development to Adult Morphology.

The human sphenoid bone (SB), centrally located at the cranial base, is structurally and developmentally complex. It arises from multiple cartilaginous precursors and undergoes both endochondral and intramembranous ossification, forming essential elements such as the sella, orbital walls, and numerous foramina. This review integrates embryological, anatomical, and radiological findings to present a comprehensive view of SB development and variation. Embryological studies reveal a layered ossification sequence, with accessory centers in the presphenoid and basisphenoid that influence adult morp

Primary Aspergillosis of the Sphenoid Sinus Observed on Bone SPECT/CT.

The bone SPECT/CT allows for precise anatomic delineation of bone activity, which can be valuable for diagnosis of nonmalignant bone diseases. We report an extremely rare case of sphenoid sinus aspergillosis, observed on bone SPECT/CT imaging, as an isolated disease in an immunocompetent individual. A 58-year-old woman with a history of plantar fasciitis was assessed for infection in the tarsal bones. Planar bone scan image revealed incidental focal lesion in skull base, which was considered to be the left sphenoid sinusitis by the bone SPECT/CT scan.

Ossifying fibroma of sphenoid bone with coexistent mucocele: CT and MRI.

A case of a 26-year-old woman with decreasing vision in her right eye, diplopia, headache, and galactorrhea is presented. Both CT and MR studies showed a large sphenoid fibroosseous lesion that had a ground glass appearance on CT and low-to-intermediate proton density and low T2-weighted and low T1-weighted signal intensities on MR. After contrast medium administration, this process diffusely enhanced on CT and MR. There was also an expansile mass in the sphenoid sinus that had intermediate proton density, high T2-weighted and low T1-weighted signal intensities compared with brain. This mass d

Extensive primary Ewings' sarcoma in the greater wing of the sphenoid bone.

We describe a rare case of an extensive primary cranial Ewing's sarcoma located in the greater wing of the sphenoid bone with extension to the orbit, the endocranium, the parapharyngeal and infratemporal space. The patient presented with diplopia, anosmia and prolapse of the left eye. He was given chemo- and radiotherapy and was free of symptoms on re-examination 1.5 years later. The prognosis of Ewing's sarcoma in the absence of surgery is uncertain, but prompt treatment appears to have a satisfactory therapeutic outcome. In the future, more cases should be studied in order to investigate the

[Bone statics of the face: the frontal-sphenoid-pterygoid bone pillars as biomechanic equivalents of the mandible].

In general biomechanics, trabecular osteo-architectonics is considered as a response to the mechanical strains present in the bone (compression or traction). The facial and cephalic skeleton is no exception to this law. The study of the bony architectionic provides the proof, since we have demonstrated in the fixed facial bone mass (at the limits of the facial skeleton and the skeleton of the base of the skull) the biomechanical equivalent of the mandible: the cranio-facial pterygo-spheno-frontal bony pillars. Paired, symmetrical and with a medio-frontal junction, they represent an embryologic

Sphenoid sinus osteitis after endoscopic transsphenoidal surgery: a bone-centered postoperative entity.

PURPOSE: Despite advances in endoscopic skull base surgery, postoperative sinonasal inflammation remains poorly characterized. In particular, sphenoid sinus osteitis has received little attention, as complications after endoscopic transsphenoidal surgery (ETS) have largely been assessed in terms of mucosal pathology. This study aims to determine the incidence of sphenoid sinus osteitis after ETS, describe its clinical and radiological features, and identify associated surgical and demographic risk factors. METHODS: We retrospectively reviewed patients undergoing primary ETS for sellar or para

Sphenoid Bone Dysplasia: A Rare Cause of Compressive Optic Neuropathy Mimicking Glaucoma.

Fibrous dysplasia is a benign, rare bone disease in which bone is replaced by fibro-osseous tissue to varying degrees. It can present differently depending on the amount of compression caused by the fibro-osseous tissue. Patients are usually asymptomatic, but symptoms related to cranial nerve compression may occur. In this case report, we describe a 45-year-old woman with sphenoid bone dysplasia which compressed the optic nerve and caused unilateral optic disc cupping that mimicked glaucoma. Our case highlights the importance of including compressive etiologies associated with optic disc cuppi

Aneurysmal Bone Cyst of the Sphenoid Body Mimicking Craniopharyngioma: A Case Report.

An aneurysmal bone cyst is a locally destructive benign lesion that predominately affects the long bones. Sphenoid body involvement is rare. To date, only 19 primary aneurysmal bone cysts of the sphenoid body have been reported. We describe the case of an 18-year-old male with a one-week history of severe right eye pain and lacrimation, right-sided diplopia, right-sided headache, photophobia, nausea, and vomiting. Magnetic resonance imaging (MRI) demonstrated a lobulated lesion centered in the sphenoid body with expansion into the cavernous sinus, sellar region, and clivus. The lesion had a ho

Sphenoid Bone Pneumatisation on Lateral Cephalograms of Patients With Neurofibromatosis Type 1.

BACKGROUND/AIM: Neurofibromatosis type 1 (NF1) is an autosomal dominant hereditary disease that causes tumors and many developmental disorders, e.g., cranial dysplasia. The purpose of this retrospective study was to analyse the pneumatisation of the sphenoid bone in NF1. PATIENTS AND METHODS: The anonymised lateral cephalograms of 166 NF1 patients and 166 age- and sex-matched controls were examined for anterior-posterior sphenoid pneumatisation. The patient group analysis considered whether the patients had been affected by a facial plexiform neurofibroma (FPNF). RESULTS: Sphenoid pneumatisa
