[Changes in the surgical approach of impacted lithiasis in the terminal choledochus].

Transduodenal sphincterotomy used to be was a common procedure when the surgeon couldn't remove stones from the terminal choledochus. We treated 6 patients in whom instead of a sphincterotomy, the stone was left in place and the duct drained with a T-tube. The first three cases were so managed because an emergency left no other option in the other three the decision was elective. Operative cholangiographies showed the impacted stones and in the postoperative cholangiographies, the stones had passed down to the stones had moved upward because of the decompression with the T-tube. The problem in

Experience with sphincteroplasty and sphincterotomy in pancreatobiliary surgery.

Review of a 26-year experience with transduodenal sphincteroplasty and sphincterotomy was undertaken (1) to analyze critically the indications for and results of these procedures and (2) to determine which preoperative factors correlate with a good or poor outcome. Of 109 patients, 78 underwent sphincteroplasty, whereas 31 had a transduodenal sphincterotomy. Surgical indications included: group 1, 53 patients with common duct stones; group 2, 28 patients with dyskinesia or stenosis of the sphincter of Oddi (without choledocholithiasis or recurrent pancreatitis); and group 3, 28 patients with r

[Transcutaneous chemical cholelitholysis with methyl tert-butyl ether (MTBE). lst results in man].

Three patients have been treated, with instillation of MTBE through a transcutaneous biliary drainage after transduodenal sphincterotomy. The first had gallbladder stones, the second common bile duct stones and the last intrahepatic stones (V segment). While in the first case there has been only a partial success in the other two we have obtained a total dissolution of the stones, with complete clinical recovery. This treatment has not caused immediate side effects but the absence of long term damage has not yet been proven. We think that the use of MTBE is not indicated in gallbladder stone d

[Perduodenal Sphincterotomy (author's transl)].

In spite of immaculate surgical technique conventional transduodenal sphincterotomy is attended by a non-lethal complication rate of about 5.8% and a mortality rate of about 4.5%, the most frequent cause being dehiscence of the duodenal suture. The primary and secondary pathogenesis hereof is explained by the particular predisposition duodenal laceration on account of its special anatomy and operative vulnerability. Bearing these facts in mind, the method of so-called perduodenal sphincterotomy (p. sph.) seemed to offer more promising results for the following reasons: 1. Splitting of the sphi

[Surgical therapy of benign obstructive jaundice (author's transl)].

The Mirizzi syndrome is the cause of one-third of all cases of benign obstructive jaundice; the therapy is cholecystectomy. Cholangiography is indispensible, manometry and choledochoscopy optional. Choledocholithiasis occurs in 50%. First, the stones are washed out and a Fogarty catheter is used, followed by spoon and forceps. Stenoses of the papilla are mostly dilated; therefore, transduodenal sphincterotomy in fibrotic stenoses should be used for impacted stones and suspected carcinoma of the papilla. The goal is definitive therapy during the first procedure, preserving anatomy and physiolog

[Early and late results of transduodenal sphincteroplasty (author's transl)].

70 patients who underwent transduodenal sphincterotomy for stenosis of the papilla of Vater 2 months till 15 years ago, were re-examined. The lab results as well as the histological examination of liver biopsies and the ERC gave no reference of bile duct-or liver damage.

Operative treatment of periampullary retroperitoneal perforation complicating endoscopic sphincterotomy.

BACKGROUND: Evidence-based strategies are lacking regarding the appropriate management of periampullary retroperitoneal perforations complicating endoscopic retrograde cholangiopancreatography (ERCP) combined with endoscopic sphincterotomy (ES). We propose a transduodenal operative repair of periampullary retroperitoneal perforation. METHODS: Six patients with duodenal periampullary perforation induced by endoscopic sphincterotomy underwent operation after failure of an attempt of conservative management. After mobilization of the second and the third part of the duodenum, a minimal transvers

[Status determination in discussion of benign stenosis of Vater's papilla].

Inflammatory and cicatrical alterations are the morphological basis of papillary stenosis in most cases due to canalicular or lymphatic spreading of bacterial infection in gallstone disease. The consequences of papillary stenosis are elevated pressure in the ducts and reduced bile flow. At surgery you find dilatation of the bile ducts, increase of residual pressure, decrease of outflow, and--in cicatrical stenosis--stop for the probe. Only the inflammatory stenosis is reversible, but permanent alteration is not to determine. Therefore therapy is necessary, at first instrumental dilation to 4-6

Sphincterotomy treatment for biliary tract stones. A retrospective review.

A series of 106 patients with biliary tract stones is reported. The patients underwent supraduodenal choledochotomy, with an attempt to carry out conventional sanitation of the biliary tract, before a transduodenal sphincterotomy was done. Indication for sphincterotomy was mainly presence of wedged papillary stones or multiple common duct stones. Of the entire series, 52 attended for follow-up study after a period of at least 2 years postoperatively, and underwent intravenous cholangiography, 51 patients had papillary stones removed via the sphincterotomy. Stones were missed in 8 of 102 patien

[Endoscopic papillotomy (author's transl)].

Endoscopic sphincterotomy (EST) is a useful alternative to re-intervention of the bile ducts in cases of choledocholithiasis in the aged patient. The rate of success of EST is 87.7%, the rate of complications 8.4%, and the rate of mortality 1.1%. Mortality from EST is lower than that from transduodenal papillotomy or reoperation of the bile ducts. Late complications after EST are unknown and not to be expected.

[The "sump syndrome": a case treated by surgical endoscopy. A discussion about biliodigestive diversions and endoscopic sphincterotomies].

A case of "sump syndrome", occurred in our department, suggested the discussion about the opportunity of endoscopic operative procedures in choledocholithiasis and in biliary tract surgery complications. Improvement of endoscopic operative procedures allow extensive indication of ERCP sphincterotomy instead of traditional choledochoduodenostomy or transduodenal papillosphincterotomy. Additionally, operative endoscopic procedures are good alternative to reoperation for traditional biliary tract surgery complications.

Transduodenal Sphincteroplasty: A Visual Case Study.

BACKGROUND: Sphincter of Oddi dysfunction is a challenging and rare clinical entity resulting in pancreatobiliary pain and stasis of bile and pancreatic juice. This problem was classically treated with surgical therapy, but as classification of the disease has changed and newer methods of endoscopic evaluation and therapy have evolved, operative transduodenal sphincteroplasty is now generally reserved as a final therapeutic option for these patients. In this video and manuscript, we describe our approach to operative transduodenal sphincteroplasty in a patient with type I Sphincter of Oddi dys

Transduodenal extended sphincteroplasty and removal of ventral duct pancreatic calculi.

The surgical treatment of chronic pancreatitis and associated ductal calculi typically involves drainage of the main pancreatic duct or parenchymal resection. Treatment of isolated symptomatic pancreatic duct calculi is usually approached by endoscopic techniques. Herein is described a case report of operative transduodenal extraction of symptomatic pancreatic calculi of the ventral pancreas in a patient with a prior distal pancreatectomy. This case represents an uncommon yet valuable option in selected patients in whom endoscopic treatments have failed or are unavailable.

Surgery for pancreas divisum.

Recurrent acute pancreatitis has a documented association with pancreas divisum. The intermittent inflammation of the gland is caused by episodic obstruction of pancreatic secretions at the minute dorsal duct orifice. Selection of appropriate patients for operative treatment is paramount if long-term success is to be achieved. Dorsal duct sphincterotomy alone, performed by a transduodenal approach, requires precise surgical technique. Long-term freedom from recurrent attacks of pancreatitis can be achieved by operative treatment.

[Lesions after sphincterotomy].

Within the last years 70 transduodenal sphincterotomies at stringent indication have been carried out at the Department of Surgery of the RWTH Aachen. This means 9.2% of all operations caused by cholelithiasis. The lethality was 4.2%. The following postoperative complications where stated: a) the bleeding from the divided duct of sphincter with or without haemobile (1.4%), b) the post-sphincterotomy pancreatitis (9.5%) owing to lesion of the pancreatic duct, c) the retroperitoneal abscess with biliary peritonitis (1.8%), d) insufficiency of the duodenal wall or duodenal fistula (1.9%), e) post

[Lithiasis of the common bile duct and transduodenal papillosphincterotomy. Analysis of our cases].

The results obtained from 1978 to 1987 with the use of transduodenal papillosphincterotomy in bile duct lithiasis are reported. The operation was performed on 126 patients and produced 4.7% immediate mortality. The indications and the most frequent post-operative complications were studied. Routine intra-operative cholangiography had been carried out. The long-term results were very good in 86% of the cases, average in 13% and poor in 1%. Provided one limits its indications to cases when it is really necessary, papillosphincterotomy does not involve a higher risk than other biliary operations.
