This report describes the long-term operative outcome of 72 patients with spina bifida cystica. The period of follow-up was between 4 and 20 years. In our series, 17 patients died, with the mortality rate increasing as the lesions were more rostral. All cases involving only a meningocele are living without handicap. The cases of spina bifida cystica with hydrocephalus had higher morbidity and mortality when compared to those without hydrocephalus. The costs and economic benefits are examined of introducing a programme for the mass-screening of pregnancies for the detection and abortion of fetuses with spina bifida cystica. A benefit-cost index is derived, and the possible effects on it of making different input assumptions are discussed. It is considered that, on economic grounds, screening may be worthwhile only in populations in which the incidence of spina bifida is high.

The results of a policy of "selection" in treating infants with spina bifida cystica over 21 months are presented. Of 37 newborn infants referred on the first day of life 25 were not treated because of the severity of their condition, as defined by exact criteria. All died under 9 months of age. In contrast, only one treated infant died and the rest are either normal or moderately handicapped. Selection for treatment is offered as the best but not a good solution to an insoluble problem.

Twenty couples whose newborn baby had an open myelomeningocoele were given a detailed account of the baby's clinical state and likely future by the paediatrician. They each said how they wanted their child treated. I believe a high percentage of parents would like to be involved in the decision on how to treat their spina bifida baby. The present physical handicaps and social status of 202 survivors of spina bifida cystica attending The Hospital for Sick Children, Great Ormond Street before 1954 are described. Ninety-three of the patients, mostly with uncomplicated meningocele, had escaped serious disability and were living normal lives.
