A case study of two adults who sustained severe burst fractures of the thoracolumbar spine (T12, L1) after trivial accidents using plastic sledges. In one case a transient conus medullaris lesion was encountered. The fractures were reduced indirectly by internal fixation as well as by direct anteposition of the fragments from the spinal canal after laminectomy. The fracture mechanism is probably a result of hyperflexion combined with very little shock absorbtion from this sledgetype. Plastic sledges seem particularly hazardous for adults.

Sixty consecutive patients with spinal injuries and encroachment upon the spinal canal of greater than 20% were assessed for neurological outcome. The patients were divided into two groups, those undergoing posterior surgery alone, and those undergoing anterior surgery for formal decompression with or without anterior or posterior instrumentation. In those patients undergoing posterior surgery, an improvement rate in the neurological function of 83% was noted in patients with incomplete lesions, whereas an 88% improvement rate was found in those undergoing the anterior procedure. There was no

During the years 1962 through 1978 a total of 42 children under the age of 15 years with fractures or dislocations of the spine were admitted to the Royal Brisbane Hospital Complex. Sixty-three percent of these children were males. The predominate causes of injury were fall/jump (40%), road traffic accidents (29%), and water-related injuries (19%). Half the injuries occurred to the cervical spine, while injuries at more than one level occurred in 35% of patients. Cord injuries occurred in 14% of patients, while skeletal and head injuries occurred in over half of the spinal injuries associated

BACKGROUND: Ankylosing spondylitis (AS) is a chronic inflammatory disease marked by spinal rigidity, which predisposes patients to unstable fractures even after minor trauma. While isolated spinal fractures in AS are well described, multilevel fractures across the cervical, thoracic, and lumbar spine following a single traumatic event are exceedingly rare and pose substantial diagnostic and surgical challenges.
OBSERVATIONS: The authors present the case of a 41-year-old male with advanced AS and a history of alcohol use and diabetes who sustained six unstable spinal fractures spanning the cer

Fracture geometry, particularly a jagged bone spike, can present a physical barrier in closed reduction of pediatric distal radius-ulna fractures. When closed reduction of the fracture is not possible, accepting an incomplete reduction and hoping for remodeling, or open reduction, which poses a greater risk for infection and potential physeal injury, are alternative treatment options. The objective of this study was to describe a technique, coined as Percutaneous Skeletal Traction Aided Reduction (P_STAR), for reducing these fractures, thereby eliminating the acceptance of an incomplete reduct

Meningiomas are the most common intradural extramedullary tumors, followed by nerve sheath tumors that can also grow extradurally. Metastases are the most frequent extradural tumors and most commonly affect the thoracic vertebrae. Spinal fractures with column dislocation and/or instability require surgical fixation. Spine surgery for an extramedullary tumor or fracture usually involves decompression of neural elements and instrumentation for stabilization. These procedures risk spinal cord and nerve root injury. The incidence of nerve root deficits after resection of nerve sheath tumors is par

BACKGROUND: Ankylosing spondylitis (AS) is a rheumatologic condition that affects the axial skeleton. Structural changes render the spine susceptible to fractures, which can be treated operatively or non-operatively. The preferred surgical approach is not well established. The objective of this study is to evaluate trends in the surgical treatment of AS patients with cervical and thoracolumbar spine fractures.
METHODS: Using the nationwide inpatient sample (NIS) database, we identified 961 (4,683 weighted) AS patients from 2003 to 2014 who had fusion surgery for vertebral fractures. Our prima

OBJECTIVES: To report on unique cases of spinal compression fractures due to pregnancy-associated osteoporosis (PAO) and to suggest a satisfactory treatment modality.
MATERIALS AND METHODS: A single-center retrospective study. We reviewed the data of 535 patients with osteoporotic spinal compression fractures over a period of 5-year. Two patients who developed spinal compression fractures due to PAO were identified and treated.
RESULTS: The clinical presentation and blood investigations ruled out other causes of osteoporosis. Dual-energy X-ray absorptiometry was used to confirm the diagnosis

Minimally invasive vertebral augmentation-based techniques have been used for the treatment of spinal fractures (osteoporotic and malignant) for approximately 25 years. In this review, we try to give an overview of the current spectrum of percutaneous augmentation techniques, safety aspects and indications. Crucial factors for success are careful patient selection, proper technique and choice of the ideal cement augmentation option. Most compression fractures present a favourable natural course, with reduction of pain and regainment of mobility after a few days to several weeks, whereas other

AIM: The aim of this study was to report whether coccyx fractures are present in spinal cord injury (SCI) patients, who suffered from painful symptoms in the low back, gluteal, hip and thigh regions; and to determine the pain characteristics of coccydynia in these patients.
METHODS: Twenty males and six females with traumatic SCI (mean age: 31.57+/-12.23 years) who described painful symptoms in the low back, hip, gluteal or thigh regions were included in the study. Pain assessment was done by using visual analogue scale (VAS) and the short form of McGill pain questionnaire. Radiological asses

BACKGROUND AND PURPOSE: Percutaneous vertebroplasty can aggravate spinal canal narrowing, especially in patients with preoperative retropulsion. The purpose of this study was to evaluate changes in spinal canal dimension during percutaneous vertebroplasty for osteoporotic fractures with retropulsion.
MATERIALS AND METHODS: We reviewed all cases of osteoporotic vertebral fractures treated with vertebroplasty. Twenty-one patients (25 vertebrae) had a retropulsed fragment that compromised the dimension of the spinal canal on preoperative imaging. We measured the degree of retropulsion before and

BACKGROUND AND PURPOSE: Spinal fractures in ankylosing spondylitis (AS) were difficult to diagnose before CT and MR imaging were available. The purpose of our investigation was to characterize spinal fractures and determine the value of different imaging modalities in AS.
METHODS: Twelve successive cases of spinal fractures were identified in MR imaging files of AS patients. Conventional radiographs were available for 12, CT scans for 7, and 3D-CT scans for 4. We carefully reviewed clinical histories and imaging presentations.
RESULTS: Fractures were found in the cervical spine in 3 patients

Approximately twenty percent of women will suffer a compression fracture sometime in their later years. Almost always the result of postmenopausal osteoporosis, they create dramatic and lasting consequences that lead to pain, reduced physiologic function and increased frailty. Recognition and treatment of compression fractures is an important step in maintaining an acceptable level of comfort and an optimal functional ability.

We present a male infant with congenital fractures and features typical of spinal muscular atrophy. Mid-shaft fractures of the left humerus and both femurs occurred in utero. The patient died at 8 months of age following recurrent respiratory tract infections. His peripheral electrophysiologic studies were consistent with anterior horn cell disease. Molecular genetic studies were negative for the SMN gene. At autopsy, extensive anterior horn cell loss was present. There was extensive disease of skeletal muscle with relative sparing of the diaphragm. This patient represents a further case of th

As long as simple and efficient operative techniques for stabilizing of injured segments of the vertebral column were missing naturally conservative methods dominated. The fixateur interne and the transpedicular spongiosa bone graft, however, provide operative techniques of high efficiency and technical simplicity, which make a revision of old indicational rules- operation versus conservative treatment - necessary. The example of a stable spine fracture points out that the calculated final result has to be clearly better if risk and effort of an operation should be advantageous for the patient

Our results and experience with computer tomography (CT) were analyzed with respect to 41 patients with fresh bone injuries in both the cervicothoracic and the lumbar region of the vertebral column. Besides differentiated representation of the involvement of various parts of the vertebral body, the arch, transverse and spinous processes in 46 fractures, and four luxation fractures, bone fragments were found in the spinal cavity in 12 patients. Two patients displayed spinal bleeding and 3 showed a traumatic vertebral disc protrusion; 5 patients had paravertebral hematomas. Comparison of the CT-

Clinical guidelines for the treatment of vertebral fractures associated with ankylosing spondylitis are derived from case reports and a review of literature. The coincidence of paravertebral calcifications and fracture formations leads to problems in the establishment of a proper initial diagnosis. Therefore computed tomography and magnetic resonance imaging have to be employed to define the extent of fracture and the presence of spinal lesions. As a rule vertebral fractures based upon spondylitic alterations are extremely unstable and tend to secondary dislocation with a high risk of spinal c

The management of narrowing spinal fragments in the operative treatment of spinal fractures remains an open question, in particular when the procedure is performed by a posterior approach. This article describes the use of intraoperative ultrasonography during spinal surgery. From 1990 to 1997, 116 spinal fractures were treated operatively at our clinic. Stabilization of the spine was achieved with the AO fixateur interne and the AO USS, respectively (Synthes, D-79224, Umkirch, Germany). For 60 cases who had a fractured posterior vertebral surface dislocated into the spinal canal, we used intr
