Spinal trauma. Pathophysiology and management of traumatic spinal injuries.

Spinal trauma can originate from internal or external sources. Injuries to the spinal cord can be classified as either concussive or compressive and concussive. The pathophysiologic events surrounding spinal cord injury include the primary injury (compression, concussion) and numerous secondary injury mechanisms (vascular, biochemical, electrolyte), which are mediated by excessive oxygen free radicles, neurotransmitter and electrolyte alterations in cell membrane permeability, excitotoxic amino acids, and various other biochemical factors that collectively result in reduced SCBF, ischemia, and

[Traumatic injuries in ankylosing spinal diseases].

Ankylosing spinal diseases, such as ankylosing spondylitis (AS) and diffuse idiopathic skeletal hyperostosis (DISH), are highly important in spinal traumatology and are therefore specifically considered in the AO Spine Classification of spinal injuries. These diseases make the spine extremely susceptible to injury and also complicate the diagnosis and treatment, leading to an increased mortality. Concomitant neurological injuries are frequent. The treatment of such injuries requires great attention and careful preparation. Early and precise diagnostics using computed tomography (CT) and magnet

Spinal Injuries in the Overhead Athlete.

Purpose of reviewTreatment of overhead athletes requires a systematic approach that will make an accurate diagnosis, deliver effective treatment, and make timely and safe return to sport.Recent findingsNew data has shown success rates and return to play effectiveness after different types of cervical and lumbar surgery. Cervical foraminotomy has been shown to have the highest rate and fastest return to play, but with the highest incidence of need for revision surgery. Cervical artificial disc replacement has shown promising results in the general population and is being done more commonly in e

Noncontiguous double-level unstable spinal injuries.

PURPOSE: Noncontiguous double-level unstable spinal injuries (NDUSI) are uncommon and have not been well described. In this study, we aimed to better understand the patterns of NDUSI, in order to recommend proper diagnostic and treatment methods, as well as to raise awareness among traumatologists about the possibility of these uncommon injuries. METHODS: A total of 710 consecutive patients with spine fractures were treated for >9 years since 2007 at a single regional trauma center. Of them, 18 patients with NDUSI were reviewed retrospectively. RESULTS: The incidence of NDUSI was 2.5 % of al

Acute complications of spinal cord injuries.

The aim of this paper is to give an overview of acute complications of spinal cord injury (SCI). Along with motor and sensory deficits, instabilities of the cardiovascular, thermoregulatory and broncho-pulmonary system are common after a SCI. Disturbances of the urinary and gastrointestinal systems are typical as well as sexual dysfunction. Frequent complications of cervical and high thoracic SCI are neurogenic shock, bradyarrhythmias, hypotension, ectopic beats, abnormal temperature control and disturbance of sweating, vasodilatation and autonomic dysreflexia. Autonomic dysreflexia is an abru

[Spinal injuries in children and adolescents].

PURPOSE OF THE STUDY: Spinal injuries in children are rare and account for a low proportion of all childhood injuries. Due to anatomical and biomechanical properties of the growing spine, there are great differences between spinal injury in childhood and adulthood. Because of higher mobility and elasticity of the spine and a lower body mass in children, spinal injuries are not frequent and represent only 2 to 5 % of all spinal injuries. In this retrospective study, the effectiveness of conservative and surgical treatment of injured spines in children is evaluated in a 10-year period. MATERIAL

Retrospective analysis of spinal missile injuries.

One hundred six patients with spinal missile injury from war zones were admitted to our department from 1994 to 2000. Functional recovery and complications in surgical and conservative treatment groups were evaluated. Sixty-five were treated surgically, of whom 55 (84%) had incomplete injuries (Frankel scores B, C, and D). In the conservative group, 28 (68%) had incomplete injuries. A total of 81 patients (53 in the surgical group, 28 in the conservative group) could be monitored for functional recovery. In the surgical group, 34 (64%) showed improvement, 15 (28%) were unchanged, and four (7%)

Spinal cord injuries in Ilorin, Nigeria.

BACKGROUND: Spinal Cord Injuries (SCI) usually result from road traffic accidents (RTA), falls, sports and some misadventures. This study was carried out to examine the aetiology of SCI in Ilorin, Nigeria; factors contributory to morbidity and mortality and to suggest measures for reducing them. METHODS: Age, sex, mechanism of injury, complications, duration of treatment and eventual outcome of patients admitted for SCI from 1995 to 1999 were restrospectively studied. RESULTS: Thirty-nine patients, age 19 to 60 years (mean 37.3), 36 males and three females were seen. Cervical spine injuries

[Principles of treatment of spinal injuries.].

This paper is meant for those colleagues who are faced with spinal injuries only sporadically or not at all, to make them familiar with contemporary principles of treatment of these injuries. The treatment of spinal injuries has the objective to restore the anatomical set-up of the spine, stabilize it against surrounding forces which act on it and to preserve its function. Thus the same principles are involved as in the treatment of other fractures. Treatment involves reposition, releasing of nerve structures and stabilization which can be achieved by a conservative procedure or surgery. An in

[Unstable spinal injuries in craniocerebral trauma].

Estimates of the incidence of injury to the cervical spine among patients suffering blunt trauma to the head vary widely. In an attempt to quantify the risk, the records of 593 consecutive patients with blunt craniocerebral injuries admitted to our trauma center in 1991 were reviewed. The overall incidence of cervical spine injuries was 5.8% and for the unstable cervical spine injuries 2.9%. In the management of head trauma with associated cervical spine injuries realignment and immobilisation of the spine and surgery of the head were carried out as first step, followed by decompression and st

[Mechanism of formation of spinal injuries].

It was stipulated that in medicolegal examination of deaths due to mechanical trauma accompanied by vertebral column separation it is necessary to investigate the surface of separation, which as a rule passes along the verge of the osseous and cartilagnous locking laminae. At the initial stage of avulsion a fragment of osseous lamina of the adjacent vertebral corpus remains fixed to cartilaginous lamina. Taking into account the localization of this fragment one may judge upon the direction of spine flexure.

[Classification and prognosis of spinal injuries].

ABCD-0123-CLASSIFICATION: Taking into consideration the known classifications, a new scale of spiral injuries has been developed in which the three osteoligamentous columns (= three column spine) of the axial organ are designated by the letters A, B, and C. The letter D denotes the discoligamentous structures. This classification includes constriction of the spinal canal, whereby constriction of 1/3 is given the number 1, 2/3 the number 2, 3/3 the number 3, and no constriction 0. The prognosis of spinal injuries depends on many factors, especially the following: 1. Severeness of injury, 2. Tim

Radiological assessment of thoracolumbar spinal injuries.

The vertebrae of the thoracolumbar junction are extremely vulnerable to serious injury. Spontaneous reduction of these unstable fractures or dislocations often occurs, making radiological diagnosis of the full extent of these injuries difficult. The technique of radiological examination applicable is described, and important radiological signs are emphasized. The anatomical factors predisposing the thoracolumbar junction to trauma and the mechanism of injury are briefly described.

[Computer tomographic diagnosis of spinal injuries].

The examination of patients with spinal trauma by computed tomography offers a direct view into the spinal canal. A stenosis of the spinal canal by fragments of a fracture or haematoma is recognized easily; myelography has become redundant. For the injured patient computed tomography is connected with only a minimal risk.

Traumatic and Nontraumatic Spinal Cord Injuries.

OBJECTIVE: Owing to the aging of the population in the United States, it is anticipated that injury mechanisms, treatment, and outcomes of spinal cord injuries (SCI) will change. There is a scarcity of literature on nontraumatic SCI. Our goal in this study was to evaluate the causes, management, complications, and outcomes after SCI. METHODS: In a retrospective review, patients with traumatic and nontraumatic SCI admitted to the inpatient rehabilitation unit at a level 1 trauma center from 2003 to 2013 were reviewed. RESULTS: In all, 757 entries were identified, and 685 unique patients met o

Repair therapies in spinal cord injuries.

Spinal cord repair therapies (SCRT) are experimental treatments which are attracting a growing interest among both spinal cord injury sufferers and their families as well as physicians and physiotherapists. Basing on current professional literature, this article presents the most important SCRT strategies. The majority of currently developed SCRTs are at the stage of experimental in vitro or animal studies. Few of these studies are in the early clinical trial stage or are being offered as non-standard commercial health care services. Basing on a review of the literature, it can be stated that
