Spinal cord compression in a patient with a pain pump for failed back syndrome: a chalk-like precipitate mimicking a spinal cord neoplasm: case report.

OBJECTIVE AND IMPORTANCE: The use of intrathecal morphine has been effective with few complications for chronic intractable pain of both benign and malignant origins. A rare but serious problem that exists is the formation of an inflammatory mass at the catheter tip of the pain pump. CLINICAL PRESENTATION: We report the case of a 67-year-old female patient with failed back syndrome who presented with sensory complaints and back pain. INTERVENTION: Magnetic resonance imaging revealed impingement on the thoracic cord by a mass. The mass was originally thought to be a spinal cord tumor; however

Spinal cord Schistosomiasis: A child's case with an unsatisfactory outcome that mimicked an intramedullary neoplasm. A rare case report.

Neuroshcistosomiasis is a life-threatening complication of schistosomiasis. Its prevalence in endemic populations is believed to be underreported at 1-4%. We report a four-year-old child who came to our hospital relatively late since the presentation of weakness and incontinence. MRI revealed long segment (T12-L1) ill-defined intramedullary lesions with cord extension having patchy enhancements. Despite the administration of both praziquantel and pulse prednisolone treatment, there was no convincing result. We thus question the role of steroids and praziquantel in individuals with late-diagnos

Hematological neoplasms with first presentation as spinal cord compression syndromes: a 10-year retrospective series and review of the literature.

CONTEXT: The epidural space is a well-known, albeit uncommon, location for lymphomatous involvement, estimated to occur in less than 3% of all systemic lymphomas. Initial presentation of the patient with disease in this site (i.e. primary spinal epidural lymphoma) has been considered to be "rare". When it has been reported, many studies have emphasized the occurrence of aggressive lymphomas with a poor prognosis. DESIGN: A 10-year retrospective search of our files generated 7 patients who presented initially with back pain, incontinence and/or lower extremity weakness, and by neuroimaging stu

A retrospective cohort analysis evaluating the utility of the spinal instability neoplastic score in screening for spinal instability and vertebral compression fracture risk in plasma cell neoplasm and lymphoma lesions.

BACKGROUND CONTEXT: Spinal column tumors with Spinal Instability Neoplastic Scores (SINS) suggesting instability often trigger referrals to spine surgeons. Plasma cell lesions and lymphoma are highly radiosensitive histologies, and may reossify after radiation therapy. The SINS score, designed to assess the need for surgical stabilization for spinal neoplasms, may therefore overestimate instability in patients with these radiosensitive histologies. PURPOSE: Herein we seek to determine if the SINS score is significantly correlated with lesion instability and vertebral compression fracture prog

Minimizing Intraoperative Blood Loss During Lumbar Vertebrectomy for Spinal Metastasis of Renal Neoplasm: Specific Radiological Embolization Technique.

BACKGROUND: Clear cell renal cell carcinoma is the most common malignant renal tumor, sometimes causing highly vascularized vertebral metastases. If the metastasis is isolated, en bloc resection of the lesion should be discussed. Vertebrectomy for curative oncologic purposes carries a significant risk of bleeding. We report a case in our center of a patient who underwent a special embolization technique prior to vertebrectomy, which significantly reduced intraoperative blood loss. METHODS: We report a case of special embolization technique prior to vertebrectomy. RESULTS: A 62-year-old patie

The plated bayonet: a new instrument to facilitate surgery for intra-axial neoplasms of the spinal cord and brain stem. Technical note.

A new instrument for use in surgically treating intra-axial neoplasms of the spinal cord and brain stem is described. The plated bayonet allows neoplastic tissue in the spinal cord to be separated from functioning neural elements without perforating the adjacent spinal cord. In addition, the plated bayonet facilitates exposure through the very small incision necessary to remove tumors of the brain stem without damaging cranial nerves or other vital structures.

Effect of esketamine combined with pregabalin on acute postsurgical pain in patients who underwent resection of spinal neoplasms: a randomized controlled trial.

Moderate-to-severe acute postsurgical pain (APSP) can prolong the recovery and worsen the prognosis of patients who undergo spinal surgery. Esketamine and pregabalin may resolve APSP without causing hyperpathia or respiratory depression after surgery. However, there are other risks, such as dissociative symptoms. We designed a randomized controlled trial to investigate the effect of the combination of these 2 drugs on the incidence of APSP in patients who underwent resection of spinal neoplasms. Patients aged 18 to 65 years were randomized to receive esketamine (a bolus dose of 0.5 mg·kg -1 an

Esketamine combined with pregabalin on acute postoperative pain in patients undergoing resection of spinal neoplasms: study protocol for a randomized controlled trial.

BACKGROUND: Perioperative pain management is one of the most challenging issues for patients with spinal neoplasms. Inadequate postoperative analgesia usually leads to severe postsurgical pain, which could cause patients to suffer from many other related complications. Meanwhile, there is no appropriate analgesic strategy for patients with spinal neoplasms. METHODS/DESIGN: This is a protocol for a randomized double-blind controlled trial to evaluate the effect of esketamine combined with pregabalin on postsurgical pain in spinal surgery. Patients aged 18 to 65 years scheduled for spinal neopl

Four Cases of the Melanotic Variant of Malignant Nerve Sheath Tumour: a Rare, Aggressive Neoplasm in Young Dogs with a Predilection for the Spinal Cord.

Four cases of a rare melanotic variant of malignant nerve sheath tumour (MNST) in dogs are described. All four cases presented with neurological clinical signs due to multicentric, intradural, intra- and extraparenchymal neoplasms that surrounded the spinal and cranial nerves and infiltrated the adjacent spinal cord and brain. The dogs were young (3 months to 3 years of age), all were female and four different breeds were represented. Characteristic histological features were interweaving fascicles of spindle-shaped cells, sometimes with an architecture reminiscent of Antoni A and B patterns.

Combined Use of Diffusion Tractography and Advanced Intraoperative Imaging for Resection of Cervical Intramedullary Spinal Cord Neoplasms: A Case Series and Technical Note.

BACKGROUND: Intramedullary spinal cord neoplasms (ISCN) pose significant management challenges. Advances in magnetic resonance imaging (MRI) (such as diffusion tensor imaging, DTI) have been utilized to determine the infiltrative nature and resectability of ISCN. However, this has not been applied to intraoperative decision making. OBJECTIVE: To present a case series of 2 patients with ISCN, the first to combine use of DTI, pre- and intraoperative 3-dimensional (3D) virtual reality imaging, and microscope integrated navigation with heads-up display. METHODS: Two patients who underwent surger

Spinal Intradural Escherichia coli Abscess Masquerading as a Neoplasm in a Pediatric Patient with History of Neonatal E. coli Meningitis: A Case Report and Literature Review.

BACKGROUND: Central nervous system abscesses frequently can be seen in the immunocompromised population and most commonly consist of intracranial collections in the adult patient. Spinal intradural abscesses are less commonly encountered, and there are even fewer numbers in the pediatric population with a concordant absence of documentation in the published literature. CASE DESCRIPTION: In this case report, we describe the presentation of a 2-year-old boy with a history of perinatal Escherichia coli meningitis at 2.5 months of age who was found to have an intradural spinal lesion, initially c

Efficacy and safety of human fibrinogen-thrombin patch (Tachosil(®)) in the management of diffuse bleeding after chest wall and spinal surgical resection for aggressive thoracic neoplasms.

Diffuse bleeding after chest wall and spine resection represents a major problem in General Thoracic Surgery. Several fibrin sealants (FS) have been developed over the years and their use has been gradually increasing over time, becoming an important aid to the surgeons, justifying their use across numerous fields of surgery due to its valid haemostatic properties. Among the several FS available, TachoSil(®) (Takeda Austria GmbH, Linz, Austria) stands out for its haemostatic and aerostatic properties, the latter being demonstrated even in high-risk patients after pulmonary resections for prima

Primary tumors of the spine.

Spinal tumors consist of a large spectrum of various histologic entities. Multiple spinal lesions frequently represent known metastatic disease or lymphoproliferative disease. In solitary lesions primary neoplasms of the spine should be considered. Primary spinal tumors may arise from the spinal cord, the surrounding leptomeninges, or the extradural soft tissues and bony structures. A wide variety of benign neoplasms can involve the spine including enostosis, osteoid osteoma, osteoblastoma, aneurysmal bone cyst, giant cell tumor, and osteochondroma. Common malignant primary neoplasms are chord

[Imaging of demyelinating and neoplastic diseases of the spinal cord].

The clinical symptoms of myelopathy are variable and non-specific. Demyelinating as well as neoplastic spinal cord diseases can cause paresthesia, progressive sensomotoric deficits and bowel and bladder dysfunction. Imaging of the spine, especially with magnetic resonance imaging (MRI), is an essential component in the diagnostic assessment of myelopathy and makes a substantial contribution to achieving the correct diagnosis. Although intramedullary neoplasms are far less common than demyelinating spinal cord diseases, radiologists should be familiar with the three most common entities, astroc

A Pain in the Neck - A Case of Intramedullary Spinal Ependymoma.

Spinal cord tumors are sporadic and account for around 2%-4% of central nervous system neoplasms. Ependymoma is one of the most common spinal cord neoplasms and can present with different neurological signs and symptoms. They commonly present with neck or back pain and associated neurological involvement, with sensory symptoms usually antedating the motor symptoms. We now present a rare case of spinal ependymoma in a 26-year-old female who presented with isolated neck pain and the absence of other neurological symptoms. Attention to peculiar characteristics like isolated neck pain might be an

Unilateral hemilaminectomy: The surgical approach of choice for juxta-medullary spinal tumors.

BACKGROUND: The conventional laminectomy may precipitate numerous long-term complications such as the development of kypho-scoliosis, spinal instability, epidural fibrosis and loss of bony shield over the spinal cord, thus increasing the risk of a subsequent myelopathy. The present study evaluates the efficacy of a unilateral hemilaminectomy as the surgical approach of choice for excision of juxtamedullary spinal neoplasms. MATERIALS AND METHODS: From January 2001 to December 2010, 83 patients (male: female ratio: 58: 25, mean age 37.4 years, median age 45 years) harboring a juxta-medullary s

Unusual appearance of spontaneous spinal intradural hematoma mimicking a meningioma.

Spontaneous spinal intradural hematoma is uncommon. It can mimic vascular or hemorrhagic neoplasms, as certain imaging features overlap. The distinction between spinal hematoma and neoplasm is crucial for the approach to management and might determine the outcome. The purpose of this report is to outline the radiological and pathological features of spinal hematoma. We present a case of a 62-year-old male patient presented with a short history of lower-limb weakness, back pain and both fecal and urinary incontinence. An examination revealed that his lower limb power was 0/5 bilaterally with hi
