Can preoperative spirometry be omitted for HBP surgery?

PURPOSE: Postoperative pneumonia (POP) is a major complication of hepatobiliary and pancreatic surgery that significantly affects patient outcomes. However, the necessity of routine preoperative spirometry to assess the risk of POP remains uncertain. We therefore evaluated whether or not omitting routine preoperative spirometry affects the postoperative outcomes, particularly the incidence of POP. METHODS: This retrospective study included 691 patients who underwent hepatobiliary and pancreatic surgeries at our institution. Patients were categorized into two groups: the pre-COVID-19 group (Apr

Quality of spirometry tests in the field of occupational health.

BACKGROUND: The spirometry test is a valuable test to evaluate the performance of the respiratory system. The interpretation of the results is highly dependent on the quality of its performance, while the inappropriate quality results in unwanted consequences for individuals and the healthcare system. This study investigated the quality of spirometry tests performed in occupational health. METHODS: In this cross-sectional study, the quality of 776 spirometry tests in different occupational centers by the specialists in Rasht, Iran, in 2020, based on the guidelines of the American Thoracic Soc

Inhaled Medication Use in Smokers With Normal Spirometry.

BACKGROUND: The objective of our study was to identify variables associated with inhaled medication use in smokers with normal spirometry (GOLD-0) and to examine the association of inhaled medication use with development of exacerbations and obstructive spirometry in the future. METHODS: We performed a retrospective multivariable analysis of GOLD-0 subjects identified in data from the COPDGene study to examine factors associated with medication use. Five categories were identified: (1) no medications, (2) short-acting bronchodilator, (3) long-acting bronchodilator; long-acting muscarinic anta

[Spirometry in the Medical Practice - Part 1: Measuring].

Spirometry in the Medical Practice - Part 1: Measuring Abstract. Today, every practitioner, as well as every hospital emergency, should be able to perform spirometry. However, the measurement requires solid basic knowledge as well as a certain amount of experience so that consistent and reproducible measurements are possible. Since the measurement depends heavily on the cooperation of the patient, typical errors must be recognised and corrected immediately. A rounded forced expiratory flow volume curve, or a sudden drop of the curve towards zero at the end of exhalation as well as clearly devi

[Spirometry in the Medical Practice - Part 2: Interpretation].

Spirometry in the Medical Practice - Part 2: Interpretation Abstract. The way in which spirometry should be assessed correctly was defined internationally in 2005. Compared to before, it was newly defined that the FEV1/VC ratio is used as a parameter to detect an obstruction. An obstruction is of significance if this measured value is below the 5 % percentile, which is usually indicated as a Z-Score of minus 1,645 in the automatically generated protocol. To generate this value, the software must contain the modern GLI standard values and the patient data (gender, age, height). A proven obstruc

Interstitial Lung Abnormalities, Emphysema, and Spirometry in Smokers.

BACKGROUND: Most pulmonary conditions reduce FVC, but studies of patients with combined pulmonary fibrosis and emphysema demonstrate that reductions in FVC are less than expected when these two conditions coexist clinically. RESEARCH QUESTION: Do interstitial lung abnormalities (ILAs), chest CT imaging findings that may suggest an early stage of pulmonary fibrosis in individuals with undiagnosed disease, affect the association between emphysema and FVC? STUDY DESIGN AND METHODS: Measures of ILA and emphysema were available for 9,579 and 5,277 participants from phases 1 (2007-2011) and 2 (201

Handgrip strength is associated with improved spirometry in adolescents.

INTRODUCTION: Pulmonary rehabilitation, including aerobic exercise and strength training, improves function, such as spirometric indices, in lung disease. However, we found spirometry did not correlate with physical activity (PA) in healthy adolescents (Smith ERJ: 42(4), 2016). To address whether muscle strength did, we measured these adolescents' handgrip strength and correlated it with spirometry. METHODS: In 1846 non-smoking, non-asthmatic Germans (age 15.2 years, 47% male), we modeled spirometric indices as functions of handgrip strength by linear regression in each sex, corrected for fac

The impact of spirometry on pediatric asthma diagnosis and treatment.

Research has shown that spirometry is underutilized in the clinical setting. This study profiles the use of spirometry in an asthma management program at an inner-city community health clinic. Eligible subjects included 56 children who presented with an acute asthma exacerbation. Physicians recorded patient diagnosis before and after viewing spirometry. Bivariate and multivariate analysis was used to determine associations between symptoms and forced expiratory volume in 1 second (FEV1). Physicians changed 30.4% of patients' treatment plans after viewing spirometry results. Wheezing was signif

Effects of spirometry standards in two occupational cohorts.

Characteristics of subjects with nonreproducible lung function tests (test failures) are described in two studies of occupational respiratory disease. According to current guidelines for spirometry, subjects with test failure are excluded from analyses of epidemiologic pulmonary function data. Among 415 Chinese cotton textile workers, the prevalence of byssinosis was 5.8% for subjects with repeatable tests and 13.3% for subjects with test failure. In regression analysis, the estimate of the association between cotton dust exposure and FEV1 decreased when subjects with test failure were exclude

Peripheral eosinophilia and preserved ratio impaired spirometry (PRISm).

Preserved ratio impaired spirometry (PRISm) may be in the pre-stage of chronic obstructive pulmonary disease. However, little is known about peripheral eosinophils in PRISm. This study ultimately enrolled 7301 community-dwelling participants aged 20-79 years without airflow obstruction. Participants were categorized based on spirometry into two groups: normal spirometry and PRISm. Peripheral eosinophilia is defined by peripheral eosinophil counts ≥300 cells/µL. Multivariable logistic regression model was employed to examine the association between peripheral eosinophil counts and PRISm, as app

Incentive spirometry in postoperative abdominal/thoracic surgery patients.

BACKGROUND: Postoperative patients have higher incidences of respiratory complications. Patients undergoing abdominal or thoracic surgical procedures are at greater risk of having such complications. Incentive spirometry is an inhalation-based prophylactic technique that encourages patients to mimic a natural deep sigh to periodically increase lung volume. As this technique is the prophylactic method of choice for many hospitals, several studies have tested its efficacy. METHODS: Five articles, including 4 systematic reviews and 1 clinical practice guideline, are analyzed and summarized. Each

How often does spirometry testing induce cardiac arrhythmias?

AIMS: To describe the frequency and type of arrhythmia induced by forced expiratory and inspiratory flow-volume loop manoeuvres. METHODS: 735 subjects (548 men) aged from 10 to 98 years old (mean 54 years, SD +/- 15) were submitted to a conventional medical examination and spirometry testing prior to a maximal cardiopulmonary exercise test (CPET). A continuous digital electrocardiogram (ECG) was recorded during spirometry and CPET, and later reviewed and interpreted by the same physician (who supervised all the procedures). RESULTS: 470 subjects (64%) had cardiac arrhythmias during one or bo

Unsupervised home spirometry versus supervised clinic spirometry: analysis of longitudinal participant-level data from the CLEAR trial in patients with bronchiectasis.

BACKGROUND: Lung function measurement is central to assessing disease severity and monitoring progression in bronchiectasis. Although home spirometry is convenient for remote monitoring, there is limited evidence for its performance. AIMS: In an a priori defined substudy in the CLEAR trial (a multicentre, randomised, open-label study of hypertonic saline and carbocisteine versus usual care in bronchiectasis), we assessed the performance of home spirometry in adults with bronchiectasis using the PANACEA framework (seven domains: test Performance, disease mANAgement, Cost, patient Experience, c

Unsupervised home spirometry is not equivalent to supervised clinic spirometry in children and young people with cystic fibrosis: Results from the CLIMB-CF study.

BACKGROUND: Handheld spirometry allows monitoring of lung function at home, of particular importance during the COVID-19 pandemic. Pediatric studies are unclear on whether values are interchangeable with traditional, clinic-based spirometry. We aimed to assess differences between contemporaneous, home (unsupervised) and clinic (supervised) spirometry and the variability of the former. The accuracy of the commercially available spirometer used in the study was also tested. METHODS: Data from participants in the Clinical Monitoring and Biomarkers to stratify severity and predict outcomes in chi

Spirometry after treatment completion for pulmonary tuberculosis - a necessity?

Pulmonary tuberculosis often results in permanent lung damage, such as fibrosis, bronchiectasis, and emphysema, despite a successful microbiological cure. These structural changes lead to post-tuberculosis lung disease (PTLD), which is characterized by chronic respiratory symptoms and impaired lung function. This study aimed to evaluate functional and radiological impairments in patients after completion of anti-tubercular therapy to understand the long-term impact of tuberculosis on respiratory health. This prospective study evaluated 175 adults within 1 year of completing anti-tubercular the

Incentive spirometry decreases respiratory complications following major abdominal surgery.

Pulmonary complications are the leading cause of morbidity and mortality following major abdominal surgery. Chest physiotherapy aims to decrease the likelihood of these complications and hasten recovery. Exercises aimed at maximising inspiratory effort are the most beneficial for the patients. The incentive spirometer is a handheld device that patients use to achieve effective inspiration. In a nonrandomised pilot study of 263 patients we have found that the addition of the incentive spirometer, as part of an intensive post-operative physiotherapy programme, decreased the occurrence of pulmona

Influence of incentive spirometry on pulmonary volumes after laparotomy.

Twenty patients undergoing an upper abdominal procedure were randomly assigned in two groups. One group was treated with both chest physical therapy and incentive spirometry and the other was treated with chest physical therapy alone. Spirometric values, respiratory rate, tidal volume and vital capacity were determined during the postoperative course. We observed that patients treated with incentive spirometry bad a faster return to preoperative pulmonary volumes. This observation confirms the prophylactic role of incentive spirometry in the prevention of pulmonary complications after laparoto
