The drug abuse problem in Sri Lanka.

Sri Lanka's experience in dealing with drugs of abuse and with the drug abuse problem is unique in many ways. From the sixteenth century until the middle of the twentieth century, the colonial powers regulated the use of opium as a revenue earning measure. Since independence was achieved in 1948, various measures have been taken to scale down the abuse of opium, cannabis and certain pyschotropic substances, but not all these efforts have been successful. In the wake of ethnic problems, there has been an increase in the involvement of Sri Lankan nationals in smuggling drugs across national fron

Vitamin A status of children in Sri Lanka.

A representative country-wide rural nutrition status survey determined the extent and distribution of vitamin A deficiency in Sri Lanka in children 6 through 71 months of age. Trained paramedical personnel recorded the presence or absence of selected ophthalmological signs and symptoms associated with vitamin A deficiency in 13,450 children. The results of the country-wide clinical survey indicate that a vitamin A deficiency problem of public health importance may exist in two of 15 health areas. Serum vitamin A levels were determined on 346 survey children from two of 15 health areas and comp

Abortion in Sri Lanka: the double standard.

In Sri Lanka, women do not have access to legal abortion except under life-saving circumstances. Clandestine abortion services are, however, available and quite accessible. Although safe specialist services are available to women who can afford them, others access services under unsafe and exploitative conditions. At the time of this writing, a draft bill that will legalize abortion in instances of rape, incest, and fetal abnormalities awaits approval, amid opposition. In this article, I explore the current push for legal reform as a solution to unsafe abortion. Although a welcome effort, this

Incidence of brucellosis in Sri Lanka: an overview.

Infection by Brucella abortus seems to be a major cause of abortions among cattle and buffaloes in Sri Lanka. The incidence of this disease is more prominent among the animals in the Dry zone of the country raised under extensive management systems. The present low incidence of this disease and the small size of the country may facilitate launching of an effective disease control scheme. The milk ring test (MRT) has proven to be usable in testing milk for the infection at farm level. An ELISA technique could be employed to test the seroprevalence of infection among MRT-positive animals. A prog

A study of cutaneous myiasis in Sri Lanka.

BACKGROUND AND OBJECTIVES: Cutaneous myiasis (CM) due to Diptera fly larvae shows different patterns in different regions. Many modalities of treatment have been described. The objectives of our study were to identify the species causing CM in Sri Lanka, the common sites of infestation, and the contributory factors, and to assess some treatment modalities, in particular mineral turpentine and certain herbal preparations. METHODS: All patients with CM admitted or referred to the Dermatology Unit at the General Hospital, Kalutara, over 18 months starting from July 1997, and all patients with CM

Drugs--a growing problem in Sri Lanka.

Cannabis, hashish and opium were the drugs generally abused in Sri Lanka. The laws relating to drugs were consolidated in 1935. Heroin is the most widely abused drug today. Tourists are mainly responsible for the introduction of heroin into Sri Lanka. The separatist terrorists of Sri Lanka are engaged in trafficking in drugs to find the money for the purchase of arms and ammunition.

Dextrocardia and situs inversus in Sri Lanka.

Transposition of the viscera is an uncommon condition but no figures are available as to its prevalence in a tropical country. 31 cases of dextrocardia were seen among 110,603 persons examined at two chest clinics in Sri Lanka. A further 11 cases were detected by the reading of films taken at a tuberculosis prevalence survey in the general population in which 27,095 persons were examined after random selection. The incidence at the chest clinics was 1 in 3567, while the prevalence in the general population was 1 in 2500, perhaps the highest rate to be reported from any country. The prevalence

Chronic musculoskeletal ankle disorders in Sri Lanka.

BACKGROUND: Musculoskeletal disorders of the lower extremities are commonly affected by chronicity and disability. One of the most commonly affected areas is the ankle. Epidemiological information is limited for chronic musculoskeletal ankle disorders in the general community, particularly in the developing world. This study aimed to determine the prevalence and impact of chronic musculoskeletal ankle disorders in the Sri Lankan community. METHODS: A cross-sectional stratified random sample of people (n = 1000) aged 18 to 85 years in Sri Lanka was undertaken by questionnaire in the general co

Hypokalaemic periodic paralysis in central Sri Lanka.

We describe 14 Sinhalese male patients with hypokalaemic periodic paralysis (HPP). The age at onset was between 10 and 32 years. Each paralytic episode lasted from 6 to 48 hours (except in one patient who had an attack lasting 5 days). The frequency of attacks varied from 8 to 10 per month in one patient to only 2 attacks over a period of 16 years in another. Four patients (28.6%) had a family history of the disease. Hypokalaemia (serum potassium 1.5 to 3 mmol/l) was documented during an attack in 11 patients. No cause for hypokalaemia was evident in any of them. Investigations including EMG,

Lyssavirus in Indian Flying Foxes, Sri Lanka.

A novel lyssavirus was isolated from brains of Indian flying foxes (Pteropus medius) in Sri Lanka. Phylogenetic analysis of complete virus genome sequences, and geographic location and host species, provides strong evidence that this virus is a putative new lyssavirus species, designated as Gannoruwa bat lyssavirus.

Frigophobia: a case series from Sri Lanka.

Frigophobia is a condition in which patients report coldness of extremities leading to a morbid fear of death. It has been reported as a rare culture-related psychiatric syndrome in Chinese populations. An extensive survey of the literature yielded only six case reports. The present paper describes a series of 109 patients with frigophobia in Sri Lanka. The common clinical presentation was of patients initially examining their extremities for cold sensations, and then, with the onset of fear, covering themselves in layers of clothing, applying emollients, and staying near an open fire in an ef

Non-dermatophyte mold onychomycosis in Sri Lanka.

Dermatophytic and non-dermatophytic onychomycosis (NDM) was indistinguishable clinically in our case series. Making a clinical diagnosis of onychomycosis without mycology is the routine practice in Sri Lanka. The prevalence of NDM (45.8%) was very high in our patient population, followed by yeasts (34.1%); dermatophyte infection made up only 20%. Therefore, the treatment of onychomycosis with griseofulvin seems futile. Close contact with soil, the habit of walking barefoot, frequent emersion of hands in water, and a hot, humid climate partly explain the variation in causative pathogens in this

Leishmania donovani and cutaneous leishmaniasis, Sri Lanka.

To investigate the relationship of cutaneous leishmaniasis isolates from Sri Lanka to known species, we performed DNA sequencing and microsatellite analyses. We identified Leishmania donovani as the agent of Sri Lanka cutaneous leishmaniasis and showed that these parasites are closely related to those causing visceral leishmaniasis in the Indian subcontinent.

Haemoglobin E beta thalassaemia in Sri Lanka.

Haemoglobin E beta thalassaemia is the commonest form of severe thalassaemia in many Asian countries, but little is known about its natural history, the reasons for clinical diversity, or its management. We studied 109 Sri Lankan patients with the disorder over 5 years. 25 patients were not receiving transfusion; transfusion was stopped with no deleterious effect in a further 37. We identified several genetic and environmental factors that might contribute to the phenotypic diversity of the disorder, including modifiers of haemoglobin F production, malaria, and age-related changes in adaptive

Intestinal lactase deficiency in Ceylon (Sri Lanka).

Lactose tolerance tests (LTT) in 200 normal adult Ceylonese have shown that 145 (72.5%) had a flat LTT, indicating a population prevalence of lactase deficiency of 66.2 to 78.8%. Jejunal lactase estimations in a smaller sample (41) confirmed this. Twelve of 55 subjects (21.8%) with a normal LTT had intestinal symptoms after lactose and intestinal lactase was low in most of them. It is suggested that little lactase is required to elevate the blood sugar but that more may be required to prevent diarrhea. On the other hand, 65.5% had no symptoms despite a flat LTT, and the possible reasons for th
