Public health detailing to increase naloxone access in NYC pharmacies.

BACKGROUND: Drug overdose is a public health crisis in the United States. Opioid overdose deaths are preventable using naloxone, an opioid antagonist that reverses the effect of an opioid and prevents fatal overdose. OBJECTIVES: This study aimed to evaluate changes in naloxone standing order status, attitudes, and practice behavior after an 8-week public health detailing campaign on increasing naloxone access conducted among pharmacists in independent pharmacies in New York City (NYC). METHODS: Campaign recommendations were to (1) enroll in the NYC pharmacy naloxone standing order program, (

Updated Strategies and Therapies for Reducing Ischemic and Vascular Events (STRIVE) unstable angina/non-ST elevation myocardial infarction critical pathway toolkit.

The Strategies and Therapies for Reducing Ischemic and Vascular Events (STRIVE) acute coronary syndromes (ACS) critical pathway toolkit, developed in 2002, has been revised and updated. The updated toolkit is based on the American College of Cardiology/American Heart Association 2007 guidelines for the management of patients with unstable angina/non-ST-segment elevation myocardial infarction (UA/NSTEMI). This review highlights the major revisions to the UA/NSTEMI guidelines and illustrates and describes the revised STRIVE critical pathway tools, which include pathway flowcharts, standing order

Clinical implications of Brivaracetam Cross-Reactivity with levetiracetam immunoassay.

Brivaracetam (BRV) and levetiracetam (LEV) are structurally similar, causing LEV immunoassays (IA) to cross-react with BRV, the real-world clinical implications of which have not been previously characterized. We analyzed 36 patients with 54 serum level measurements, including levels drawn when patients were not receiving the corresponding medication or simultaneous LEV/BRV levels when both ASM were co-administered. LEV levels were measured by IA; BRV levels by LC-MS/MS. Among 14 paired measurements in BRV-only patients, the median cross-reactivity ratio was 14.7. False-positive LEV levels at

The epidemiology of skin tears in the institutionalized elderly.

While skin tears are a common occurrence in the institutionalized elderly population, nothing has been written about this problem. We retrospectively studied all incident reports during a 1-year period at a large, urban, long-term-care facility to identify residents with skin tears. The overall incidence of skin tears was 0.92 per patient per year. The incidence rate for females, but not for males, increased significantly with age (P = 0.012). The mean length of the skin tear was 1.9 cm +/- 1.4 (mean +/- SD). Eighty percent occurred in the upper extremities, with the most frequent location bei

Influenza vaccination rates for hospitalised patients: a multiyear quality improvement effort.

BACKGROUND: Influenza vaccination is the most effective method for preventing influenza virus infection. Adult hospitalised patients form a particularly high-risk group for severe influenza given their advanced age and comorbidities. We sought to improve the influenza vaccination rates of hospitalised patients at the White River Junction Veterans Affairs Medical Center. METHODS: The improvement effort started in 2007 when our baseline vaccination rate was about 60%. An interprofessional team analysed the influenza vaccination process for hospitalised patients. During the course of six influen

Using clinical pathways in patients undergoing cardiac valve surgery.

Clinical pathways, protocols, and standing order sets help organize patients' care and eliminate variations created by practitioners' different preferences. Much attention is being focused on providing the most cost-effective care in the shortest time. Quality of care must be maintained during this process. Clinical pathways, protocols, and standing order sets help ensure that care is consistent and quality of care is maintained with the added benefit of a shorter stay in the hospital.

Intercalation-Directed Programming of Nanomechanics and Mesoscale Topology.

Creating topologically complex structures beyond the molecular scale remains a long-standing challenge in chemistry and materials science. Here, we introduce a self-assembly strategy using snowflake-shaped molecular building blocks, coupled with a molecular intercalation approach that finely tunes fibril-fibril interactions within the assemblies. This dual strategy enables concurrent control over topology, mechanical stiffness, and chiroptical response. Remarkably, the elastic modulus of the assemblies can be modulated across more than 2 orders of magnitude─from a few gigapascals to tens of me

A Quality Improvement Project to Increase Nurse-initiated Care from Triage and Improve Timeliness to Care in the Pediatric Emergency Department.

INTRODUCTION: Initiating care during emergency department (ED) triage is an effective way to decrease time to treatment. Triage nurse-initiated standing orders (SOs) are safe, evidence-based, and allow care to begin for patients before provider evaluation. We describe a multidisciplinary approach to increase the use of SOs across 2 pediatric EDs. METHODS: Interventions for this quality improvement initiative were driven by frontline staff input and included education, fostering competition among nurses, individual feedback, and editing of SOs. The primary outcome measure was the use of SO per

Revealing the bond exchange and network rearrangement mechanism in vitrimers.

Vitrimers combine thermosets stability with thermoplastic reprocessability, yet their rational design is hindered by an insufficient understanding of their viscoelasticity. We investigated the fundamental relationships among segmental dynamics, bond exchange kinetics, and network rearrangements in model poly(propylene glycol)-based vitrimers. Our results show that the bond rearrangement time (τr) and its temperature dependence remain nearly unchanged at high temperatures, where their segmental relaxation time (τα) differs by several orders. This indicates that rate-limited bond exchange kineti

Re-examining the boundary conditions in modeling surface-acoustic-wave-driven acoustofluidic streaming.

Numerical simulations of surface acoustic wave (SAW)-induced acoustic streaming are highly sensitive to the choice of second-order boundary conditions. This study systematically compares the no-slip (NS) and Stokes slip (SD) boundary conditions through different numerical approaches. Two- and three-dimensional simulations based on the Reynolds stress method are performed for standing SAW and traveling SAW devices. Results are validated against particle image velocimetry measurements of streaming patterns and velocities. We show that the SD condition yields Lagrangian velocity fields in signifi

Challenging Spontaneous Quantum Collapse with the XENONnT Dark Matter Detector.

We report on the search for x-ray radiation as predicted from dynamical quantum collapse with low-energy electronic recoil data in the energy range of 1-140 keV from the first science run of the XENONnT dark matter detector. Spontaneous radiation is an unavoidable effect of dynamical collapse models, which were introduced as a possible solution to the long-standing measurement problem in quantum mechanics. The analysis utilizes a model that for the first time accounts for cancellation effects in the emitted spectrum, which arise in the x-ray range due to the opposing electron-proton charges in

Fracture and Fatigue of Al2O3-Graphene Nanolayers.

Al2O3-graphene nanolayers are widely used within integrated micro/nanoelectronic systems; however, their lifetimes are largely limited by fracture both statically and dynamically. Here, we present a static and fatigue study of thin (1-11 nm) free-standing Al2O3-graphene nanolayers. A remarkable fatigue life of greater than one billion cycles was obtained for films <2.2 nm thick under large mean stress levels, which was up to 3 orders of magnitude longer than that of its thicker (11 nm) counterpart. A similar thickness dependency was also identified for the elastic and static fracture behavior,

Lessons learned in implementing SCI clinical practice guidelines.

While clinical practice guidelines (CPGs) were designed as a tool to improve patient outcomes, decrease practice variation, and optimize resource utilization, providers often encounter significant barriers to integrating these into clinical practice. A study was conducted at six spinal cord injury (SCI) centers in the Department of Veterans Affairs (VA) to improve provider adherence and patient outcomes of two CPGs: Prevention of Thromboembolism in Spinal Cord Injury and Neurogenic Bowel Management in Adults With Spinal Cord Injury. To design effective implementation strategies, focus groups w

A compound pendulum for thrust measurement of micro-Newton thruster.

A thrust stand is developed for testing micro-Newton level thrusters on the ground. The stand is composed of a compound pendulum that is symmetrically suspended by two thin beryllium copper strips, and it is precisely calibrated by gravity. The stiffness of the stand can be adjusted in 3 orders of magnitude by a counterweight. When the stiffness is larger than 1 Nm/rad, the stand demonstrates a fast response to thrust. The measured range of the stand reaches 1000 μN, and the noise is less than 0.1 μN/Hz within 1 mHz-1 Hz. To calibrate the resolution of the stand, an electrostatic force is appl

Postural control: toe-standing versus heel-toe standing.

Toe-standing is observed in a number of populations who are able to stand without loss of balance and also those who have balance problems. Intuitively, individuals who stand on their toes are able to successfully regulate their whole body movement in order to keep themselves stable. Force platform data were collected for ten able-bodied subjects during three quiet standing postures, (a) heel-toe standing; (b) half-toe standing and (c) standing en demi pointe (full toe-standing). Differences in control mechanisms with each posture were compared using stabilogram diffusion analysis. During open

Large-scale semidefinite programming with graphics processing units.

Semidefinite programming (SDP) provides a powerful framework in applied mathematics with applications spanning optimization, machine learning, quantum computing, and beyond. However, the computational cost of solving large-scale SDP problems remains a significant practical limitation. We break this long-standing computational bottleneck through a synergistic codesign of low-rank algorithms and graphics processing unit (GPU) architectures, developing accelerated first-order methods that leverage both algorithmic innovations and hardware-aware implementation to achieve up to 4 orders of magnitud
